Epstein-Barr virus (EBV) infection and p53 protein expression in gastric carcinoma.

In the presented studies p53 protein expression was evaluated in samples of gastric carcinoma originating from 32 selected adult patients (with documented diagnosis of adenocarcinoma of the stomach and without the presence of Helicobacter pylori infection). Among the patients 14 individuals carried EBV-positive gastric carcinoma (group 1) while the 18 remaining patients carried EBV-negative gastric carcinoma (group 2). EBV infection was detected testing the tissue material for the presence of EBER by RNA in situ hybridization (ISH) and testing sera of the patients for EBV-specific antibodies. Expression of p53 protein was analysed using immunohistochemistry. Presence of p53 protein was noted in 9 (64.3%) cases of EBV-positive gastric cancer (group 1) and in 10 (55.5%) cases of EBV-negative gastric cancer (group 2). No significant differences were detected in the frequencies of p53 protein expression in the two studied groups. The results permit to conclude that abnormalities in p53 in gastric cancer are independent of EBV infection, even if EBV may participate in development of the tumour.

Ionic liquids as an alternative to formalin in histopathological diagnosis.

Asymmetry of cations and the type of anions play a key role in the properties of ionic liquids (ILs) as fixatives for tissue preservation. 1-Methyl-3-octyloxymethylimidazolium tetrafluoroborate has proven to be a very good fixative, with similar effects as formalin. Our study shows that it is applicable for both histological and immunohistochemical purposes. After treatment with 1-methyl-3-octyloxymethylimidazolium tetrafluoroborate, tissue sections are more intensely stained. With respect to expression patterns and staining intensity, immunohistochemical staining is comparable in tissues fixed in formalin and the selected ILs. The present study demonstrates the properties of 1-methyl-3-octyloxymethylimidazolium tetrafluoroborate for tissue preservation in histopathological procedures, eliminating the requirement of formalin.

Enzyme production and biotypes of vaginal Candida albicans.

Candidial vulvovaginitis is one of the most common forms of vaginal infection. However, the origin of the infecting organism is sometimes doubtful. Therefore, epidemiological investigation can help to recognize routes of infection spreading. The aim of the present study was to determine the ability to produce esterases by clinical isolates of C. albicans and to find the relationship between their serotypes. Also, it was intended to determine the ability of these strains to produce proteases and lipases as well as the ability of the strains to assimilate carbohydrates. 46 strains of C. albicans isolates from the vagina of women suffering from vulvovaginitis were examined. Three main kinds of esterases were distinquished by their spectra of hydrolytic activity toward alpha-naphthyl acetate, beta-naphthyl propionate and indoxyl acetate. The strains were grouped into four categories: three categories in which esterase patterns were observed and one category in which esterase bands were not observed. On the basis of the 20 carbon sources assimilated, the C. albicans strains were categorized into 11 biotypes with the major biotype accounting for 21 (45.7%) strains. The examination of proteolytic activity using casein and albumin enabled to divide the strains into four groups. All of the examined strains belonged to serotype A and all of them expressed lipolytic activity. Esterase electrophoretic patterns and biotypes based on proteolytic activities were compared with the ability to assimilate carbon from various sources.

Effects of gastrectomy or colectomy on liver metabolism and liver morphology in an experimental rat model.

BACKGROUND: It is sometimes necessary to translocate part of the gastrointestinal tract into the site of the organ earlier resected. This occurs after total esophageal, stomach or colon resection, which can lead to disturbances in the anatomical and functional balance of the gastrointestinal tract and the body as a whole. The aim of this study was to evaluate the effects of different resective surgical procedures performed on the proximal and distal GI tract in terms of total body weight, selected liver function tests, and ultrastructural changes in the liver in laboratory rats. MATERIAL/METHODS: The total of 57 resective procedures consisted of 31 total gastric and 26 total colon resections. The control group consisted of 12 animals on which only laparotomy was performed. Body weight, stool character, and the general condition of the animals were evaluated from the time of the operation. Twenty-six blood samples were collected for analysis of hemoglobin, total plasma protein, and liver aminotransferases levels. Thirty liver tissue samples from the 3 groups were taken and examined. RESULTS: We observed statistically significant decreases in total body weight, hemoglobin, and total plasma protein levels in the group where reconstructive procedures following gastrectomy were performed. In the group in which reconstructive procedures followed colectomy, a statistically significant decrease in total body weight was observed. Aminotransferase level changes were neither characteristic nor significantly different in any of the three groups involved in the investigation. CONCLUSIONS: Abnormalities of liver structure were observed at the ultrastructural level. Any correlation between these changes, general condition, and selected biochemical parameters requires further controlled clinical and experimental studies.

Diagnostic difficulties in pouchitis--experience from a single institution.

The aim of the study was to analyze the clinical, endoscopic and histological signs of pouchitis in patients operated on because of ulcerative colitis (UC) and familial adenomatous polyposis (FAP). Between 1984 and 2002, the Department of Surgery carried out 218 restorative proctocolectomies in patients with UC and 120 in FAP patients. The follow-up to assess the intensity of inflammatory changes included 110 patients: 72 UC and 38 FAP. During the endoscopic examination, samples were taken for routine histological examinations. The histological assessment was based on the so-called Moskwitz's Histological System. Acute pouchitis is diagnosed if the score is > or =4, and chronic condition--if the score is > or =4. We used a modified PDAI scale to analyze the symptoms, taking into account the microscopically detectable histological features of chronic pouchitis. The clinical symptoms of pouchitis were revealed in 28 patients (25.5%), endoscopic symptoms in 36 patients (32.7%), and histological parameters in 48 patients (28-58.3% with > or =4 score of acute pouchitis and 20-41.7% with > or =4 score of chronic pouchitis). The signs of acute and chronic pouchitis were evident in endoscopy and histology, with no clinical features observed in many cases. Patients with symptoms of chronic pouchitis require a continuous follow-up.

[Neuroendocrine tumors of the larynx].

The classification, diagnosis, treatment and prognosis of neuroendocrine tumors are presented. Rare case of typical carcinoid of the larynx is described. This was the second primary tumor after squamous lung cancer treatment. The diagnosis was based on routine histological investigation, and then confirmed by immunohistochemical tests with synaptophysin, chromogranin, S-100 protein and cytokeratin markers. The clinical presentation of the patient, age, gender, localization, macroscopic appearance and slow growth dynamic was in accordance with cases described in the literature.

Ionic liquids in embalming and tissue preservation. Can traditional formalin-fixation be replaced safely?

Ionic liquids (ILs) can be used for embalming and tissue preservation. ILs does not cause tissue damage and the tissue colour remains unaltered after treatment. Microscopical morphology of tissues fixed in ILs is of better quality than that of tissues fixed in formalin. Tissue preservation depends on the type of ILs. Best results were obtained with 1-methyl-3-octyloxymethylimidazolium tetrafluoroborate, the density of which resembles that of water. The salt is nonvaporous and when used as a formalin substitute, it eliminates health hazards in the pathological laboratory.

[Usefulness of isotope diagnosis, ultrasound neck investigation and thyroglobulin assays in quantification of patients operated on for differentiated thyroid cancer and prepared for radioiodine treatment].

UNLABELLED: The aim of the study was to assess the efficacy of surgical treatment of patients operated on differentiated thyroid carcinoma, using Tg serum assays and 131-I whole body scintigraphy. We investigated 208 patients aged from 15 to 78 yr. (mean 43.52, S.S. 16.37) including 182 females and 26 males. 183 cases of papillary cancer and 25 cases of follicular cancer were confirmed by pathologist. All the patients were investigated 6-8 weeks after thyreoidectomy. The following procedures were performed in all the patients: 1. ultrasound imaging. 2. 131 iodine uptake test, 24 hours after administration. 3. 131-I whole body scintigraphy (dose per patient 3 mCi, device Varicam). 4. TSH and Tg serum assay using FIA method. Moreover, all the tissue fragments obtained during surgery underwent histopathological assessment. RESULTS: The mean volume of the post-surgery thyroid tissue remains was 2.91 cc varying from 0 to 40. Volume was counted with Gutekunt's formula. Oncologically suspected lymph nodes were found in 8 cases. The mean TSH serum level was 79.31 (SD 59.59). The mean Tg serum level was 51.73 (SD 179). The mean value of an iodine uptake test was 6.96% (SD 6.69). Whole body scintigraphy discovered solitary thyroid site iodine uptake areas in 199 patients (95.7%). Additional uptake areas were observed in 4.3% 2 (0.9%) cases in cervical lymph nodes, 4 (1.9%) cases in lungs, 3 (1.5%) in bones. The correlation between investigations both laboratory and isotopic, and clinical state was observed in 207 cases (99.5%). The presence of high Tg serum level despite of absence of extracervical iodine uptake areas was observed in one case. We presume that the situation can be caused by the presence of multiple lung metastases not having possibility of iodine uptake. Ultrasound imaging, 131-I scintigraphy and Tg serum assays are essential methods in diagnosing and treatment process in the patients with differentiated thyroid carcinoma.

[Is hyperthyroidism often present in patients with thyroid differentiated carcinoma?].

The coexistence of thyroid cancer and hyperthyroidism is confirmed by many authors. It appears that the frequency of both disease can be greater as a result of lack of proper and penetrating biopsy diagnosis and of qualification to surgery treatment patients with hyperfunctional goiter and Graves disease, especially coexisting with nodes. The aim of our study was estimation of occurrence of hyperthyroidism in the patients with thyroid differentiated cancer. We examined group of 217 patients with diagnosed thyroid differentiated cancer, 20 patients (9.1%) of them were earlier hyperthyroidism diagnosed. 17 of them were hyperfunctional nodular goiter diagnosed and three as Graves disease, confirmed by presence of anti-TSH receptor antibodies (TRAK). Before thyroidectomy ultrasonography showed nodular goiter in 17 patients and hypoechogenic goiter with nodules in 3 patients. After thyroidectomy at the hyperthyroid patients in 16 papillary thyroid cancer and in 4 follicular thyroid cancers were diagnosed. The frequency of coexistence of cancer and hyperthyroidism in our material amounted for 9.1%. The results of our observation do not diverge from facts given in world literature and is point out the need for precise analysis of patients with hyperthyroidism and of proper qualification to surgery treatments changes suspected to be malignant process.

[Usefulness of serum calcitonin, CEA and AFP assays in the early detection of medullary thyroid carcinoma relapse].

The medullary thyroid carcinoma (MTC) occurs in the two major forms, the sporadic medullary carcinoma (SMTC) and hereditary medullary carcinoma, to which belong familial form (FMTC) and the element of Multiple Endocrine Neoplasia Syndrome MEN 2A and 2B. The method-of-choice of treatment is total thyroidectomy with following radiotherapy in selected cases. Among the follow-up methods, there are two used the most frequently: the pentagastrin and omeprazole stimulation test of calcitonin (CT), and CEA antigen assay. The aim of this study is to evaluate usefulness of plasma CT, CEA and AFP assay in the early detection of relapse or metastasis of MTC. 18 patients (14 females and 4 males) were investigated. The following procedures were performed in all the patients: plasma CT assays in the basal conditions and after stimulation tests, CEA and AFP. The results were analysed according to TNM staging and neck USG results. Our conclusion is that calcitonin stimulation tests, and CEA assays are useful methods to estimate the presence of relapse or metastases in the patients after surgical treatment due to MTC. Assays of plasma AFP concentration are not useful in a follow-up of patients operated on MTC.
