Multiple bladder calculi: a rare cause of irreducible uterine prolapse.

BACKGROUND: A vesical calculus in a prolapsed cystocele is rare. OBJECTIVE: To highlight bladder calculi as a cause of irreducible uterine prolapse. CASE REPORT: A case of irreducible total uterine prolapse caused by multiple vesical calculi is presented. Bladder stones were removed through vaginal cystolithotomy followed by vaginal hysterectomy. CONCLUSION: In cases of acute irreducible pelvic organ prolapse, the possibility of bladder stones should be kept in mind and X-ray pelvis including the prolapsed mass should be done to confirm the diagnosis.

Antenatal diagnosis of twin-reversed arterial perfusion sequence (acardiac amorphous) in a triplet pregnancy: case report.

Acardiac twinning is the most extreme form of twin-twin transfusion syndrome occurring in monzygotic twin pregnancies with monochorionic placentation. A case of acardiac amorphous foetus, occurring in association with spontaneously conceived triplet pregnancy, diagnosed on ultrasound in early second trimester is described and its antenatal management and brief review of literature discussed.

Recent onset neurofibromatosis complicating eclampsia with maternal death: a case report.

Neurofibromatosis in pregnancy has been associated with poor perinatal outcome. A 30-year-old multigravida developed peripheral lesions of neurofibromatosis during the third month of pregnancy. She had eclampsia complicated with fatal left-sided massive intracerebral haemorrhage detected on computed tomography at 8.5 months gestation. Investigations were suggestive of HELLP syndrome. A still-born male baby was delivered. In spite of all supportive measures the patient died on the third postpartum day. We conclude that recent onset neurofibromatosis in current pregnancy should be considered as a predictor of potentially adverse maternal and fetal outcome and such pregnancies be managed in tertiary level referral centre.

Long retained intravaginal foreign body: a case report.

Recurrent vaginal discharge in children may be due to variety of causes. Intravaginal foreign body retained for long duration can be a diagnostic dilemma. This report presents the case of a girl who had an intravaginal foreign body for over four years duration. Recurrent, unremitting, foul-smelling, bloody vaginal discharge in a child should alert the clinician to the possibility of a retained vaginal foreign body.

Scalp metastasis in carcinoma of the uterine cervix--a rare entity.

BACKGROUND: Distant metastasis in carcinoma of uterine cervix is a rare manifestation and bone metastasis is uncommon. CASE: A 60-year-old woman treated by radical radiotherapy for moderately differentiated cervical carcinoma FIGO stage IIIB presented 4 months later with swelling over right side of the scalp. Fine needle aspiration cytology from swelling showed poorly differentiated metastatic carcinoma. X-ray of the skull was suggestive of osteolytic metastasis of calvaria. CONCLUSION: Bone metastasis in cervical carcinoma is a late manifestation of advanced malignancy and heralds poor prognosis. Scalp metastasis is a very rarely reported phenomenon.

Neural-tube defect in dizygotic twins.

Despite the strong association between twinning and neural-tube defects (NTD), concordance of twins with NTDs is very rare. A case of concordant NTDs in dizygotic twins is reported in view of its extreme rarity.

Antigenic and allergenic properties of Amaranthus Spinosus pollen--a commonly growing weed in India.

Amaranthus spinosus (Fam. Amaranthaceae) is an important aeroallergen in India and grows commonly in different parts of the country. In spite of its clinical significance in Type I hypersensitivity disorders, the antigenic and the allergenic properties of the pollen have not been systematically resolved. We investigated antigenic and allergenic properties of 5 pollen samples of Amaranthus spinosus collected from the Delhi area at fortnightly intervals. The protein content did not exhibit statistically significant variability. However, samples collected during the peak flowering season showed higher protein content. Biochemical characterization of samples showed multiple protein fractions by IEF and SDS-PAGE analysis. Samples collected during peak season showed a slightly higher number of bands (22) in the mw range of 14-70 kD. Seven protein fractions of 70, 66, 60, 50, 40, 30 and 14 kD were observed to have IgE binding capabilities and 9 were treated as allergenic. The observations will be helpful in standardizing pollen antigens for diagnosis and immunotherapy in India.

Burden of chronic severe anemia in obstetric patients in rural north India.

There were 5124 deliveries during January to December 2000 and of these 386 patients (7.53%) were severely anemic (Hb < 6.0 g/dl). 1083 units of blood were transfused to these patients, a mean of 2.80 units to each patient. There were 2 maternal deaths due to severe anemia. Of the 386 patients, 246 could be interviewed in detail and their demographic details were compared with 100 normal pregnant women with haemoglobin levels of 10 g/dl or more. In the anemic group 81.7% were illiterate, 62.1% had no antenatal check, 78.8% had no access to safe drinking water 90.65% had no toilet facilities and used open fields for defaecation, 63.1% reported having diarrhoea and pain in abdomen off and on. 20.73% reported history of passage of worms in the stools. History of previous menorrhagia was present only in 1.2%. The comparable figures in the non-anaemic group were 34% illiterate, 24% no antenatal checkup, 55% no toilet facilities, 38% unsafe drinking water, 6% diarrhoea, 2% (worms in stools) and 5% menorrhagia. Hence it is concluded that along with prophylaxis with iron and folic acid sufficient consideration should be given to sanitation facilities, safe drinking water, recurrent gastrointestinal infection etc.
