IkappaBalpha and IkappaBbeta possess injury context-specific functions that uniquely influence hepatic NF-kappaB induction and inflammation.

IkappaB proteins play an important role in regulating NF-kappaB induction following a diverse range of environmental injuries. Studies evaluating IkappaBbeta knock-in mice (AKBI), in which the IkappaBalpha gene is replaced by the IkappaBbeta cDNA, have uncovered divergent properties of IkappaBalpha and IkappaBbeta that influence their ability to activate hepatic NF-kappaB and subsequent downstream proinflammatory processes in a stimulus-specific manner. While AKBI mice demonstrated identical levels of hepatic NF-kappaB activation in response to endotoxin, a significantly reduced level of hepatic NF-kappaB activation was observed in AKBI mice after liver ischemia/reperfusion (I/R) injury. This reduced level of NF-kappaB activation in AKBI mice after liver I/R also correlated with decreased induction of serum TNF-alpha, reduced hepatic inflammation, and increased survival. In contrast, no differences in any of these indicators were observed between AKBI mice and WT littermates after a lethal injection of LPS. Molecular studies suggest that the specificity of IkappaBalpha, but not IkappaBbeta, to properly regulate NF-kappaB induction during the acute phase of I/R injury is due to injury context-specific activation of c-Src and subsequent tyrosine phosphorylation of IkappaBalpha on Tyr42. These results demonstrate that IkappaBalpha and IkappaBbeta play unique injury context-specific roles in activating NF-kappaB-mediated proinflammatory responses and suggest that strategies aimed at inhibiting IkappaBalpha gene expression may be of potential therapeutic benefit in hepatic I/R injury.

Induction of tolerance in quadruple chimeric mice.

Human cord blood (CB) contains hematopoietic stem cells and progenitors. Because the major limitation to a widespread use of CB for transplantation lies in its limited volume, it is necessary to combine the CB from several donors. In this study, we show that lethally irradiated mice can be reconstituted with the injection of a mixture of T cell-depleted bone marrow cells (BMCs; total, 3 x 10(6)) obtained from three fully allogeneic mouse strains in two different mouse combinations. A higher survival rate was obtained in the triple injection group than in mice injected with BMCs (1 x10(6)) obtained from a single mouse strain. In the mixed chimeric mice, three kinds of donor-type and recipient-type cells were detected in all the hematopoietic organs 1 month after bone marrow transplantation (BMT). Mixed-lymphocyte reaction showed that the tolerance to both recipient-type and donor-type major histocompatibility complex determinants was induced in the chimeric mice. In the peripheral blood (PB) of these mice, only one type of cells from the three different donor strains became dominant in most chimeric mice and reached a stable level about 4 months after BMT. Polymerase chain reaction analyses, however, revealed that the skins from all the donors were accepted even when no cells with their phenotypes could be detected in the PB. These results suggest that both hemato-lymphoid reconstitution and stable tolerance to not only the recipient strain but also all the donor strains can be achieved in chimeric mice, indicating the possibility of mixed CB transplantation in humans.

Radiologists' performance for differentiating benign from malignant lung nodules on high-resolution CT using computer-estimated likelihood of malignancy.

OBJECTIVE: The purpose of our study was to evaluate whether a computer-aided diagnosis (CAD) scheme can assist radiologists in distinguishing small benign from malignant lung nodules on high-resolution CT (HRCT). MATERIALS AND METHODS: We developed an automated computerized scheme for determining the likelihood of malignancy of lung nodules on multiple HRCT slices; the likelihood estimate was obtained from various objective features of the nodules using linear discriminant analysis. The data set used in this observer study consisted of 28 primary lung cancers (6-20 mm) and 28 benign nodules. Cancer cases included nodules with pure ground-glass opacity, mixed ground-glass opacity, and solid opacity. Benign nodules were selected by matching their size and pattern to the malignant nodules. Consecutive region-of-interest images for each nodule on HRCT were displayed for interpretation in stacked mode on a cathode ray tube monitor. The images were presented to 16 radiologists-first without and then with the computer output-who were asked to indicate their confidence level regarding the malignancy of a nodule. Performance was evaluated by receiver operating characteristic (ROC) analysis. RESULTS: The area under the ROC curve (Az value) of the CAD scheme alone was 0.831 for distinguishing benign from malignant nodules. The average Az value for radiologists was improved with the aid of the CAD scheme from 0.785 to 0.853 by a statistically significant level (p = 0.016). The radiologists' diagnostic performance with the CAD scheme was more accurate than that of the CAD scheme alone (p < 0.05) and also that of radiologists alone. CONCLUSION: CAD has the potential to improve radiologists' diagnostic accuracy in distinguishing small benign nodules from malignant ones on HRCT.

[Neuroprotective effect of monoamine oxidase monoclonal antibody against vasogenic brain edema in rats].

OBJECTIVE: To observe the therapeutic effect of monoamine oxidase (MAO) monoclonal antibody (mAb) for vasogenic brain edema (VBE) in rats. METHODS: A total of 75 Wistar rats were randomized into non-edema, non-treated edema, saline-treated edema, mannitol-treated edema and MAO mAb-treated groups. Rat models of VBE were established by intraperitoneal injection of phenylephrine in the latter 4 groups. Brain water content in the gray and white matter was measured respectively with a moisture analyzer, and the permeability of the blood-brain barrier (BBB) determined by Evan's blue (EB) extravasation method. RESULTS: MAO mAb administration significantly reduced the brain water content in the gray and white matter as well as the permeability of BBB (P<0.01), Which was especially effective for the white matter, producing results comparable with those of the non-edema group (P>0.05). MAO mAb markedly alleviated brain edema, with better dehydrating effect on the white matter than mannitol (P<0.01), which reduced the water content of the brain gray and white matter undiscriminatingly and showed poor effect on the permeability of BBB. CONCLUSION: The pathogenesis of BBB permeability changes in VBE is related to the activity of monoamine oxidase, and MAO mAb has selective therapeutic effect on VBE, which is especially obvious on brain white matter.

[The experimental study on the inhibitory effect of tripterine on airway inflammation in asthmatic mice].

OBJECTIVE: To investigate the effect and mechanism of tripterine on airway inflammation in asthmatic mice. METHODS: 30 BALB/c mice were randomly divided into a control group, an asthmatic group (ovalbumin-sensitized) and a tripterine (1 mg/kg, introperitoneally)-treated group. Pathologic changes in lung tissues, number of eosinophils in bronchoalveolar lavage fluid (BALF) and expression of stem cell factor (SCF) protein in lung were observed. In vitro, we established coculture system of bone marrow derived-mast cells from C57B6 mice and fibroblast NIH3T3, which were then treated by 2 micro mol/L of tripterine, and compared with NIH3T3 and mast cells. Concentration of histamine and eotaxin in supernants of coculture was measured by fluorometry and ELISA respectively, and expression of SCF protein in fibroblasts from cocultures was analyzed with immunohistochemistry. RESULTS: Compared with the asthmatic group, less inflammatory cell infiltration in lung tissues was observed in the tripterine-treated group. There was significant difference in the number of eosinophils in BALF between the tripterine-treated group [(0.56 +/- 0.03) x 10(6)/L] and the asthmatic group [(1.25 +/- 0.40) x 10(6)/L, P < 0.05]. So was the expression of SCF protein in lung tissue [0.74 +/- 0.20, 2.50 +/- 0.19, P < 0.01]. In vitro, the concentration of histamine and eotaxin in coculture supernants and the expression of SCF protein in fibroblasts from coculture were (3.83 +/- 0.41) ng/ml, (5.79 +/- 0.40) ng/ml and (95 +/- 3)%, respectively; after tripterine intervention, the data changed to (2.88 +/- 0.35) ng/ml, (4.24 +/- 0.29) ng/ml, (17 +/- 5)% (all P < 0.01). CONCLUSIONS: Tripterine might suppress airway inflammation in asthmatic mice, probably by downregulating the expression of SCF in fibroblasts, then inhibiting the production of histamine and eotaxin in mast cells.

[Value of radiotherapy and chemotherapy after SEMS implantation operation in patients with malignant esophageal stricture].

BACKGROUND & OBJECTIVE: Owing to the lack of a systematic comparative study, it still remains a controversial issue whether a patient with malignant esophageal tumor causing dysphagia should receive radiotherapy or/and chemotherapy after the SEMS (self-expanding esophageal metal stents) implantation operation. The present paper aims at a careful study of this field. METHODS: A total of 283 specimens were divided into two groups, namely, the group merely receiving the SEMS implantation operation (Group 1, n=91) and the group receiving a combined treatment of the SEMS implantation operation followed by radiotherapy /and chemotherapy (Group 2, n=192). The survival time of the patients in the two different groups was compared and the factors associated with the survival time were analyzed by means of Cox regression method. RESULTS: The survival time of the patients in group 2 was apparently much longer than that of the patients in group 1 (499.27+/-239.7 days and 312.81+/-192.77 days, respectively, P< 0.0001). The factors associated with the survival time included the choice of treatment method, incidence of distant metastasis and lymph node metastasis, formation of esophageal fistula and the location of diseased area. Among them the relative risk of treatment method is the highest. CONCLUSION: A combined treatment of SEMS implantation operation followed by radiotherapy and chemotherapy is significantly associated with the effect of treatment and will undoubtedly prolong the life of the patients with malignant esophageal tumor causing dysphagia.

[Epidemiological study on traffic accident in Shanghai from 2000 to 2002].

OBJECTIVE: Data on traffic accident from year 2000 to 2002 were collected and testified. Epidemiological study was carried out to find the main risk factors of traffic accident. METHODS: The spatial distribution was conducted by means of Geographic Information System (GIS) and were marked on Shanghai digitalized map with different layers by different colors. RESULTS: Results showed that during 2000, 2001 and 2002, 69,669 , 68,894, and 47,088 traffic accident episodes occurred in Shanghai, resulting in 1747, 1724, 1557 deaths respectively with direct economic lost: 2.1, 2.4, 3.0 billion RMB. The main risk factors causing traffic accident deaths would include drinking alcohol before driving, fatigue and speeding. The leading causes of death were head injury and complex injury of body. GIS showed that there was a homocentric circle with more traffic accidents in downtown city, but more deaths in the urban city. Disease burden caused by traffic accident was very heavy. CONCLUSION: In Shanghai, traffic accident has great impact on social and economic issues thus should be controlled effectively. Focus should be layed on control of traffic accident in downtown area and to decrease the number of deaths related to traffic accident in the urban area.

[Relationship of plasma homocysteine and folic acid levels and 5,10-methylenetetrahydrofolate reductase gene mutation with venous thromboembolism].

OBJECTIVE: To investigate the association between concentrations of plasma homocysteine and folic acid, 5,10-methylenetetrahydrofolate reductase (MTHFR) C667T mutation and venous thromboembolism (VTE) and to analyze the effect of MTHFR C667T mutation on concentrations of plasma homocysteine and folic acid. METHODS: In 58 patients with VTE and 58 sex and age matched controls, epidemiological risk factors were surveyed. The concentration of plasma homocysteine was measured by high performance liquid chromatography, and the concentration of plasma folic acid was measured by radioimmunoassay. MTHFR C667T genotype was measured by PCR-RFLP. RESULTS: The concentrations of plasma homocysteine and folic acid showed significant difference between the case group and the control group (OR = 1.5, 95% CI: 1.216 - 2.213 and OR = 0.396, 95% CI: 0.149 - 0.709, respectively). There was no significant difference in the frequency of mutant alleles in site 667 of MTHFR gene between the cases and the controls (P > 0.05). The concentration of plasma folic acid was associated with the concentration of plasma homocysteine (multiple correlation coefficient = -2.061, P < 0.05). The MTHFR C667T polymorphism was associated with the concentration of plasma folic acid but not with the concentration of plasma homocysteine in both the case group and the control group. The multiple correlation coefficient between the MTHFR C667T polymorphism and the concentration of plasma folic acid is 0.5856 (P < 0.01). CONCLUSIONS: The results of our study demonstrate that hyperhomocysteinemia and folic acid deficiency are independent risk factors for VTE. Folic acid deficiency is a cause of hyperhomocystinemia while the MTHFR C667T mutation is one of the possible genetic factors causing folic acid deficiency in this Chinese population.

[Hemodynamic study of an over-midline axial flap].

OBJECTIVE: To investigate the hemodynamic changes of an over-midline axial flap. METHODS: An over-midline axial flap, based on the iliac artery and veins, was raised on the back of each selected Wistar rat (n = 30), vascularied by contralateral. The flap size is 7 cm x 3 cm. The over-midline part is 3 cm x 3 cm. A laser doppler flowmetry (LDF) was used to evaluate the hemodynamic changes preoperatively, and 6 hours, 24 hours, 48 hours, 72 hours, 7 days, and 14 days after the operation respectively. RESULTS: All of the flaps survived well. But, in the first three days after the operation, the blood perfusion of the flap in the areas of the midline and the distal part was significantly lower than the preoperation's level. Thereafter, the values of the blood perfusion started to increase till to the maximum 7 days after the operation. On the 14 days after the operation, the vascular structures was axially matched to the base of the flap. In the distal part of the flap, the choke vessels were gradually opened and resulted in the blood perfusion to the normal level. CONCLUSION: The hemodynamic changes of the over-midline axial flap transmidline flap could occur to match the need of the base of the flap.

[The induction and function of dendritic cells from human cord blood with IFN-alpha and GM-CSF in vitro].

OBJECTIVE: To probe into the possibility of obtaining dendritic cells (DCs) from human cord blood with rhIFN-alpha and rhGM-CSF. METHODS: The plastic adherent cells (monocyte-rich cells) from cord blood were cultured with rhGM-CSF and rhIFN-alpha for 7 days. The morphological properties of DCs were observed by microscopy, the expression of CD83 and CD86 on DCs were determined by flow cytometry. The function of DCs stimulating allogeneic T lymphocyte proliferation was detected by MTT colorimetric method. RESULTS: After 7 days culture, some of the cultured cells acquired typical DC morphology and showed increased expression of CD83 and CD86. The yield of DCs attained its peak when the concentrations of rhIFN-alpha and rhGM-CSF were 600 U/ml and 2000 U/ml, respectively. When cultured cells (containing DCs) as stimulator cells were co-cultured with allogeneic T lymphocytes (effector cells) in different ratio, they could stimulate T cell proliferation remarkably, especially when the ratio of effector cells to stimulator cells was 20 to 1. CONCLUSION: Our findings suggest that cord blood adherent cells, when cultured with rhGM-CSF and rhIFN-alpha, can be induced into mature DCs with the function of stimulating allogeneic T cell proliferation.

[Diffuse panbronchiolitis complicated by thymoma: a case report and a review of literature].

OBJECTIVE: To highlight the characteristics of diffuse panbronchiolitis (DPB). METHOD: One patient with DPB confirmed by thorocoscopic biopsy was described and relevant literatures were reviewed. RESULTS: DPB is a chronic lower respiratory tract disease common in Japanese, rare in China, characterized by infiltration of inflammatory cells around bronchioles. Although the etiology and precise mechanisms are under investigation, it is commonly hypothesized that heredity and immunity have a major role in DPB. Symptoms include cough, expectoration, and dyspnea after exercises. Pseudomonas aeruginosa is isolated from sputum in some cases. If left untreated, DPB progresses rapidly and has a poor prognosis if respiratory failure occurs. CONCLUSIONS: DPB should be included in the differential diagnosis of bilateral multiple pulmonary nodular shadows. Long-term, low-dose macrolide therapy may improve the prognosis through an anti-inflammatory effect.

[Effect of clozapine and risperidone on serum cytokine levels in patients with first-episode paranoid schizophrenia].

OBJECTIVE: To study the effects of clozapine and risperidone on serum cytokine levels in patients with first-episode paranoid schizophrenia, and explore the role of the cytokines in the psychopathological basis of the illness. METHOD: Fifty-eight patients with first-episode paranoid schizophrenia were treated with either clozapine or risperidone, and before and at the end of the 4th, 8th weeks and 6th months after the medication respectively, the serum levels of interleukin (IL)-6, 2, 18,and tumor necrosis factor (TNF)alpha were measured with enzyme-linked immunosorbent assay (ELISA). The Positive and Negative Syndrome Scale (PANSS) was used to evaluate the psychotic symptoms. RESULT: In patients treated with risperidone, the levels of serum IL-6 and IL-2 after 4 weeks, TNFalpha after 8 weeks, and IL-18 after 6 months were all significantly lowered in comparison with the pretreatment levels (P<0.01 or 0.05). In clozapine group, the levels of IL-2 after 4 weeks and IL-6 and IL-18 after 6 months were lowered significantly (P<0.01 or 0.05). Before the medication, serum IL-6 level was positively correlated with Positive Syndrome scores (r=0.386, P<0.01), IL-2 with the total score and Positive Syndrome scores (r=0.338, 0.305; P<0.01, 0.05), and TNFalpha with the total score (r=0.283, P<0.05). The changes of IL-2 and IL-6 after 8 weeks was positively correlated with the change of Positive Syndrome scores (r=0.268, 0.375; P<0.05, 0.01). Six months after the medication, the change in IL-6 and TNFalpha levels was positively correlated with the change of total score (r=0.365, 0.362; P<0.05). Before treatment, IL-6 was positively correlated with IL-2 levels (r=0.356, P<0.01), and TNFalpha with IL-18 levels (r=0.291, P<0.05). TNFalpha was positively correlated with IL-6 levels (r=0.332, P<0.01) 8 weeks after the medication. The changes in IL-6 was positively correlated with the those in IL-2 levels 6 months after the medication (r=0.391, P< 0.05). CONCLUSION: Clozapine and risperidone have similar immunosuppression actions and may affect serum IL-6 levels in patients with paranoid schizophrenia, in the psychopathology of which the cytokines play their roles of various importance.

[Microvascular study of the transmidline scapular flap vascularized by the contralateral circumflex scapular artery].

OBJECTIVE: To observe the microvascular distribution of the transmidline scapular flap supplied by the contralateral circumflex scapular artery. METHODS: The integument and deep tissues of 6 fresh cadavers were dissected and radiographed after vermilion mixture was injected to the unilateral circumflex scapular artery. RESULTS: The vascular tree passed the midline and reached to the contralateral acromion. The vessel density was the highest in the irrigating side of the back, which was higher in the middle area. In the contralateral side, the high vessel density concentrated in the upper part of the back. CONCLUSIONS: The result revealed the direct evidence for the clinical application of the transmidline scapular flap. In design and elevating of the transmidline scapular flap, it should be slanting to the upper part of the contralateral back.
