Investigation of new psychophysical measures for evaluation of similar images on thoracic computed tomography for distinction between benign and malignant nodules.

We have been developing a computerized scheme to assist radiologists in improving the diagnostic accuracy for lung cancers on low-dose computed tomography (LDCT) scans by use of similar images for malignant nodules and benign nodules. A database of 415 LDCT scans including 73 cases with 76 confirmed cancers and 342 cases with 413 confirmed benign nodules was first collected in an LDCT screening program for early detection of lung cancers in Nagano, Japan. An observer study by use of receiver operating characteristics analysis was first conducted with five radiologists to demonstrate that presenting similar images for malignant nodules and benign nodules can significantly improve radiologists' performance in the diagnosis of unknown nodules. Another observer study was then conducted for obtaining reliable data on subjective similarity ratings by 10 radiologists. Based on the subjective similarity ratings, three important features were selected from a number of nodule features, and four different techniques for the determination of similarity measures, namely, a feature-based technique, a pixel-value-difference based technique, a cross-correlation-based technique, and a neural-network-based technique, were investigated and evaluated in terms of the correlation coefficient with the subjective similarity ratings. The experimental results in this study indicated that the neural-network-based technique can provide a reliable psychophysical similarity measure which is comparable to the subjective similarity ratings for a single radiologist when evaluated by use of correlation with the average similarity ratings for the other nine radiologists.

Evaluation of liver functional reserve by combining D-sorbitol clearance rate and CT measured liver volume.

AIM: Our research attempted to evaluate the overall functional reserve of cirrhotic liver by combination of hepatic functional blood flow, liver volume, and Child-Pugh's classification, and to discuss its value of clinical application. METHODS: Ninety two patients with portal hypertension due to hepatic cirrhosis were investigated. All had a history of haematemesis and hematochezia, esophageal and gastric fundus varices, splenomegaly and hypersplenia. A 2-year follow-up was routinely performed and no one was lost. Twenty two healthy volunteers were used as control group. Blood and urine samples were collected 4 times before and after intravenous D-sorbitol infusion. The hepatic clearance (CL(H)) of D-sorbitol was then calculated according to enzymatic spectrophotometric method while the total blood flow (Q(TOTAL)) and intrahepatic shunt (R(INS)) were detected by multicolor Doppler ultrasound, and the liver volume was measured by spiral CT. Data were estimated by t-test, variance calculation and chi-squared test. The relationships between all these parameters and different groups were investigated according to Child-Pugh classification and postoperative complications respectively. RESULTS: Steady blood concentration was achieved 120 mins after D-sorbitol intravenous infusion, which was (0.358+/-0.064) mmol x L(-1) in cirrhotic group and (0.189+/-0.05) mmol x L(-1) in control group (P<0.01). CL(H) =(812.7+/-112.4) ml x min(-1), Q(TOTAL) =(1280.6+/-131.4) ml x min(-1), and R(INS) =(36.54+/-10.65) % in cirrhotic group and CL(H)=(1248.3+/-210.5) ml x min(-1), Q(TOTAL)=(1362.4+/-126.9) ml x min(-1), and R(INS)=(8.37+/-3.32) % in control group (P<0.01). The liver volume of cirrhotic group was 1057+/-249 cm(3), 851+/-148 cm(3) and 663+/-77 cm(3) in Child A, B and C group respectively with significant difference (P<0.001). The average volume of cirrhotic liver in Child B, C group was significantly reduced in comparison with that in control group (P<0.001). The patient, whose liver volume decreased by 40 % with the CLH below 600 ml x min(-1), would have a higher incidence of postoperative complications. There was no strict correspondent relationship between CL(H), liver volume and Child-Pugh's classification. CONCLUSION: The hepatic clearance of D-sorbitol, CT measured liver volume can be reliably used for the evaluation of hepatic functional blood flow and liver metabolic volume. Combined with the Child-Pugh's classification, it could be very useful for further understanding the liver functional reserve, therefore help determine reasonable therapeutic plan, choose surgical procedures and operating time.

[Establishment of Helicobacter pylori infection model in Mongolian gerbil].

OBJECTIVE: To establish a stable and reliable model of Helicobacter pylori infection in Mongolian gerbil and to observe pathological changes in gastric mucosa from the infected animals. METHODS: Mongolian gerbils were randomly divided into six groups infected with H.pylori strain NCTC11637 (n=6, group N), six groups infected with H.pylori clinical strain Y06 (n=6, group Y) and six groups as negative control (n=4, group C). H.pylori suspensions at the concentrations of 2 X 10(8)CFU/ml and 2 X 10(9) CFU/ml of strain NCTC11637 and strain Y06 were prepared with Brucella broth from Columbia agar containing sheet blood. The animals in one group N and in one group Y were orally challenged once with 0.5 ml of 2 X 10(8) CFU/ml H.pylori suspension. The animals in another group N and in another group Y were orally challenged with 0.5 ml of 2 X 10(9) CFU/ml H.pylori suspension for three times at the intervals of 24 hours, respectively. The animals were killed after 2nd, 4th and 6th week of the last infection and the gastric mucosal samples were taken for urease test, bacterial isolation, routine pathological and H.pylori histochemical examinations. RESULTS: Infection rates of the animals in group N and group Y at the 2nd, 4th and 6th week after one challenge were 0%, 0%, 66.7% and 0%, 16.7%, 16.7%, respectively. Infection rates of the animals in groups N and Y at the 2nd, 4th and 6th week after three challenges were 66.7%, 100%, 100% and 66.7%, 66.7%, 100%, respectively. In animals with positive bacterial isolation H.pylori was found to colonized on the surface of gastric mucosal cells and in the gastric pits, and the lamina propria of gastric mucosal was infiltrated with chronic inflammatory cells. CONCLUSION: By using H.pylori suspension at high concentration of 1 X 10(9) CFU for multiple times, the orally challenged Mongolian gerbils can be prepared as a stable and reliable H.pylori infection model. H.pylori can colonize in gastric mucosa of the infected animals, and mild inflammation reactions can be seen.

[Measurement of transient evoked otoacoustic emissions and follow-up of 215 healthy newborns].

OBJECTIVE: To evaluate the characteristics of transient evoked otoacoustic emissions (TEOAE) of healthy newborns and improve hearing screening of newborns. METHOD: To investigate the history of pregnancy and parturition, TEOAE was measured in 425 ears of 215 healthy newborns from 0 to 8 days postpartum and follow-up study was undertaken. RESULT: The general prevalence was 89%. There was some relationship between prevalence and testing time. The average prevalence was 74% in newborns of 0-3 days. The prevalence was 97% in newborns of 4-8 days. The way of parturition, laterality and time of pregnancy over 36 weeks hadn't any significant effect on the prevalence rate. The prevalence of females was higher than males. Quick screen was more sensitive than TEOAE. One ear of hearing loss was diagnosed after follow-up. CONCLUSION: The hearing screening test of newborns should be taken in hospitals after 4 days of birth, the way of parturition had no effect on the cochlear function. The mature time of cochlear hair cells was by the 36th week of pregnancy. Quick screen couldn't take place of TEOAE. Someone who had no TEOAE in one ear or two ears must be followed up and reexamined and exact diagnosis could be made as soon as possible combining with ABR.

[Effect of T3 on the expression of transferrin receptor and ferritin in K562 cells and its possible mechanism].

OBJECTIVE: To explore the effect of T(3) on the expression of transferrin receptor (TfR) and ferritin (Fn) in K562 cells and its possible mechanism. METHODS: Flow cytometry was used for the detection of TfR expression, radioimmunoassay for Fn expression, RNA/protein band shift assay for the binding activity of iron regulatory protein (IRP) and iron responsive elements (IRE), and RT-PCR for TfR and Fn mRNA levels. RESULTS: Different concentration of T(3) significantly increased Fn expression of K562 cells, especially at 100 nmol/L and 200 nmol/L (p < 0.05). However, T(3) had no effect on TfR expression. T(3) decreased the binding activity between IRP and IRE, particularly at concentration of 50 nmol/L. Different concentration of T(3) increased Fn-H mRNA level at different time point while it had no effect on TfR mRNA level. CONCLUSION: T(3) increased Fn expression of K562 cells through the possible mechanisms of either the post-transcriptional regulation or transcriptional modulation.

[The changes of angiotensin II receptor expression in cardiomyocytes of ventricles in pigs with experimental ventricular septal defect].

OBJECTIVE: To investigate the changes of angiotensin II receptors expression in cardiomyocytes of both ventricles in pigs with experimental ventricular septal defect (VSD). METHODS: Nineteen young pigs were divided into 3 groups: the pigs that received operation to produce experimental VSD, the pigs that underwent sham-operation, and the pigs as normal control. One month after the operation, the pigs were catheterized and then put to death; the cardiomyocytes were taken from their ventricles and were isolated. Radioligand binding assay was done to measure the Bmax and KD of angiotensin II receptor per 10(6) cells. RESULTS: For both Bmax and KD of angiotensin II receptors, there were no significant differences between the sham-operation group and the control. The Bmax of left ventricle AT1R (188.42 +/- 133.97 fmol/10(6)) and Bmax of right ventricle AT1R (272.14 +/- 232.74 fmol/10(6)) and AT2R (40.42 +/- 34.76 fmol/10(6)) in the operation group were significantly elevated in comparison with the control (Bmax of left ventricle AT1R 29.20 +/- 19.50 fmol/L, and Bmax of right ventricle AT1R 76.72 +/- 51.21 fmol/L and AT2R 9.63 +/- 1.27 fmol/L) respectively, P < 0.05. CONCLUSION: Intracardiac left-to-right shunt related volume overload leads to the elevation of AT1R expression in cardiomyocytes, which serves as an important mechanism of ventriclular hypertrophy and remodling in congenital heart disease. The increased expression of AT2R in right ventricle is a possible explanation of the phenomenon that moderately sized VSDs usually show left other than right ventriclular hypertrophy, taking into account that AT2R has been suggested to mediate antitrophic reaction.

[Repair of palm defects with improved flaps pedicled with dorsal carpal branch of ulnar artery].

OBJECTIVE: To discuss the method to repair the defects of palm with the improved flaps pedicled with the dorsal carpal branch of ulnar artery. METHODS: The improved flaps were designed on the basis of anatomical distribution of the dorsal carpal branch of ulnar artery and the medial antebrechial cutaneous nerve, the ulnar artery was ligated and cut at the beginning of its dorsal carpal branch. The flap pedicled with dorsal carpal branch including the distal ulnar artery was achieved and applied clinically to repair 15 cases of the skin and soft tissue defects of palm from August 1997 to November 2001. The size of flaps ranged from 7 cm x 5 cm to 12 cm x 8 cm. RESULTS: All of the cases were followed up 3 weeks to 6 months, and the flaps completely survived. There was no ischemia and necrosis at the distal part of flaps and the appearance and function was satisfactory. CONCLUSION: The improved flap has long vascular pedicle, abundant blood supply and sensitive sensation, so it can be used to repair defect of palm.

[Analysis of cell components of CD59+ normal cells and abnormal clons in bone marrow of patients with paroxysmal nocturnal hemoglobinuria].

OBJECTIVE: To explore the cell components of normal cells and abnormal clons in bone marrow of patients with paroxysmal nocturnal hemoglobinuria (PNH). METHODS: Bone marrow nuclear cells (BMNCs) were labeled by immunofluorescence and analyzed by flow cytometry. RESULTS: 1. There was significant difference in BMNCs between normal CD59+ cells and abnormal clons (CD59-) cells, the normal CD59+ BMNCs were mainly composed of the cells in lymphocyte-window, while the abnormal CD59- BMNCs were mainly in blast window and in granulocyte-window. 2. The CD34+ cells in CD59+ BMNCs were also markedly decreased when compared with their CD59- counterparts, and the decrease of CD34+ cells in CD59+ BMNCs was more obvious in lymphocyte window than that in blast window. CONCLUSION: The constituents of BMNCs and the amount of CD34+ cells in CD59+ and CD59- cell populations of PNH patients are significantly different, the normal hematopoiesis is exhausted. On account of the defects in normal hematopoietic cells in patients with PNH, the feasibility of using autologous bone marrow transplantation following purging of abnormal clons as a treatment of PNH should be carefully judged, the normal hematopoietic cells alone might not be able to sustain adequate hematopoiesis without further manipulations.

[The effect of erythropoitin and thyroid hormone on K562 iron metabolism].

OBJECTIVE: To explore the effect of recombinant human erythropoitin (rhEPO) and T3 on the iron metabolism of K562 cells and its possible mechanism. METHODS: With the use of the bone marrow iron stain method, the positive stained cells rate was calculated. Flow cytometry was performed to detect the transferrin receptor (TfR) expression of K562 cells. RESULTS: rhEPO increased the positive cell rate of iron stain, which was especially noticeable in the rhEPO 5 U/ml group (P < 0.01), and the effect was time-dependant, i.e., the longer the incubating time, the higher the positive rate. T3 also elevated the positive cell rate of iron stain significantly at different concentrations, and the highest rate was seen when incubating K562 cells for 48 hours. The expression of TfR was increased significantly when incubating with rhEPO for 72 hours (P < 0.05), but there was no marked change when incubating with T3. CONCLUSION: The possible mechanism by which rhEPO increased the positive cell rate of iron stain might be due to the enhancement of TfR expression and hence the increase in iron intake of K562 cells.

[Pulmonary epithelioid hemangioendothelioma. a case report and review of the literature].

OBJECTIVE: To describe the clinical features of pulmonary epithelioid hemangioendothelioma. METHODS: With a case report and review of the related literatures, the etiology, clinical manifestations, diagnosis, differential diagnosis, management and prognosis of pulmonary epithelioid hemangioendothelioma were described. RESULTS: The etiology of this rare disease remains unknown. Symptoms are scanty and usually mild; chest radiograph or computed tomography usually reveals multiple bilateral pulmonary nodules. Primitive lumen formed by a single cell is the pathologic feature. Immunohistochemical stains show that the malignant cells are of the endothelial type. There is no effective treatment for this disease and its prognosis is unpredictable. CONCLUSION: Pulmonary epithelioid hemangioendothelioma is rare and often misdiagnosed as other pulmonary diseases.

[Effects of open heart surgery under normothermic and hypothermic cardiopulmonary bypass on cytokines and complements].

OBJECTIVE: To explore the detrimental influence of normothermic and hypothermic cardiopulmonary bypass during open heart surgery on cytokines and complements. METHOD: Forty patients with congenital or rheumatic heart disease were randomized into 2 groups to receive normothermic cardiopulmonary bypass (CPB, study group, n=20) or hypothermic CPB (control group, n=20). Venous blood samples were respectively collected at scheduled time points preoperatively, at the end of CPB, and 1,4,7,14 d postoperatively to examine the level of interleukin (IL)-2, tumor necrosis factor (TNF)-alpha, C3, and C4. RESULTS: IL-2 in both groups decreased significantly at the end of CPB, postoperative day 1 and 4, but recovered the normal level at day 7 postoperatively. IL-2 in control group was significantly lower than that in the study group at each time points at the end of CPB and day 1 and 4 postoperatively. TNF-alpha in two groups was both elevated at the time points cited above, and in the study group, recovery of normal TNF-alpha level occurred at day 7 postoperatively, whereas in the control group, the recovery was not achieved until postoperative day 14. C3 in the study group was significantly lower at the time points of the end of CPB, day 1, 7 postoperatively than that in control group, but both elevated above normal at the end of CPB, day 1, and 4 postoperatively; in the study group, C3 became normal at day 7 postoperatively, which occurred in the control group only till day 14 postoperatively. At the end of CPB and day 1 postoperatively, C4 was significantly lower in the study group than in the control group, both below the level measured preoperatively at the time points of the end of CPB, day 1 and 4 postoperatively. CONCLUSION: Open-heart surgery under normothermic CPB has less detrimental influence on cytokines and complements than the operation under hypothermic CPB for better recovery of the patient.

[Study on inhibitory effect of antisense VEGF RNA on the growth of hepatocellular in vitro in vivo].

OBJECTIVE: To determine the effects of PCMV-FGEV transfection on the profile of SMMC-7721 hepatocellular in vitro in vivo. METHODS: SMMC-7721 hepatocellular was transfected with PCMV-FGEV antisense, PCMV-VEGF sense and empty vector plasmid encapsulated by lipofectamine. The positive cell clones were selected with G418. The stable transfection and expression of VEGF in the SMMC-7721 hepatocellular were determined by in situ hybridization and immunochemical analysis. The effect of PCMV-FGEV transfection on SMMC-7721 hepatocellular proliferation was observed by MTT colorimetric assay. Flow cytometry was used to determine the effects of PCMV-FGEV transfection on cell apoptosis of SMMC-7721. The growth of transfected cells was also observed in nude mice. RESULTS: There was reduced VEGF expression in SMMC-7721 transfected with PCMV-FGEV confirmed by in situ hybridization and immunohistochemical analysis. There was no effect of PCMV-FGEV transfection on cell proliferation and cell apoptosis of SMMC-7721 in vitro. The growth of cell with PCMV-FGEV transfected was slow in nude mice (vivo) and accompanied with obvious apoptosis. The latent time of tumor in the antisense mice group was 25.0+/-1.8 days, which was longer than that in sense and control group significantly (F=19.455, P<0.01). On the other hand, the average tumor weight in antisense group (0.96 g+/-0.28 g) was the smallest among the three groups (F=21.501, P<0.01). CONCLUSIONS: The expression of VEGF was inhibited by PCMV-FGEV. There was no effect on cell proliferation and cell apoptosis of SMMC-7721 by transferring PCMV-FGEV gene into SMMC-7721 cells in vitro. But in vivo it can inhibit tumor growth and induce cell apoptosis.

[Study of effect of mifepristone on apoptosis of human ovarian cancer cell line 3AO].

OBJECTIVE: To investigate the effect of mifepristone on the activity of proliferation and the apoptosis, the expression of estrogen receptor (ER), progesterone receptor (PR) protein and morphology changes of human ovarian carcinoma cell line 3AO and SKOV3 in vitro. METHODS: The proliferative activity of 3AO and SKOV3, which were cultured in vitro, was measured by tetrazolium-based colorimetric assay (MTT assay). Flow cytormetry (FCM) was used to measure the expressive rate of ER, PR, p53 protein, bcl-2 protein, cell apoptotic rate and cell proliferative cycle of 3AO cells, which were cultured with different concentration and duration of mifepristone. The morphologic and ultrastructure changes of apoptotic 3AO cells was observed by the light and electron microscopy. RESULTS: Mifepristone inhibited significantly the proliferation of 3AO cells in dose-time dependent manner in vitro. The inhibitory rate of 3AO cells growth, which were cultured with different concentration of mifepristone (5, 10, 20, 40, 80 micro mol/L) and duration (24, 48, 72 h) was from 1.7% to 75.0% (P < 0.01), but the proliferation activity of SKOV3 cells in vitro had not significant changes (P > 0.05). 3AO cells apoptosis activity appeared the positive correlation with the dose of mifepristone and cultured duration (P < 0.01). Mifepristone blocked 3AO cells proliferative cycle at the G(0)-G(1) stage, decreased the cell radio of S stage. Mifepristone induced the apoptosis of 3AO cells in vitro. The light and electron microscopy demonstrated that 3AO cells cultured with mifepristone appeared typical morphological characteristics of apoptosis including the compaction and margination of the chromosomes, nuclear fragments and formation of apoptotic bodies. Mifepristone up-regulated significantly the expression of p53 protein, but down-regulated the expression of bcl-2 protein (P < 0.01). The expressive rates of p53 and bcl-2 proteins were (54.8 +/- 4.0)% and (10.1 +/- 1.2)%, respectively, when 3AO cells was cultured with mifepristone of 10 micro mol/L for 24 h. Compared with the expression rates of control group (27.1 +/- 3.3)% and (17.6 +/- 1.0)%, the difference was significant (P < 0.01). The expressive rate of PR protein of 3AO cells cultured with mifepristone of 10 micro mol/L for 48 h was (12.7 +/- 1.4)%, which was decreased compared with the expressive rate of control group (44.9 +/- 1.4)% (P < 0.01). CONCLUSIONS: Mifepristone inhibited significantly the proliferation and induced the cell apoptosis of cell line 3AO in dose-time dependent manner in vitro. The anti-tumor effect was related to down-regulation the expression of PR protein and bcl-2 protein, and to up-regulation the expression of p53 protein of 3AO cells.
