[The inherent liability to infection of various tissue for urethra reconstruction].

OBJECTIVE: Observing the long-term results of reconstructed urethra to reduce the postop infection rate of hypospadias. To explore the standard of selecting tissue for urethra reconstruction by comparing the different inherent liability to infection in use of different kinds of tissue for urethra reconstruction. METHODS: Neo-urethra inner wall and normal urethra were observed through urography and urethroscope. The tissue section of neo-urethra, normal urethra and tissue for urethra reconstruction were observed through light microscope, scanning electron microscope and transmission electron microscope. RESULTS: The structure and liability to infection of neo-urethra made of bladder mucosa, buccal mucosa or skin never changed after long time. Metaplasia never took place in neo-urethra. CONCLUSIONS: The structure and liability to infection of neo-urethra were the same as those of the congeneric tissue. Bladder or buccal mucosa is the best tissue for urethra reconstruction considering urethra microenvironment. Neo-urethra made of skin is more liable to infection by nature. The best opportunity of hypospadias operation should be before puberty.

Study on the relationship between G1057D variants of IRS2 gene and obese T2DM in Chinese Han subjects.

OBJECTIVE: To investigate the relationship between the G1057D variants of insulin receptor substrate-2(IRS2) gene and type 2 diabetes mellitus (T2DM) in subjects. METHODS: Four hundred and thirty-nine Chinese Han subjects, including 218 patients with T2DM and 221 normal controls, were selected from the Hans in the Liaoning area, and each group was divided into two subgroups according to body mass index. The G1057D variants of IRS2 were detected by polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP) and their relationships with T2DM were analyzed. RESULTS: (1) The frequency of G1057D variant was 29% in all subjects. The frequency of DD genotype was significantly lower in non-obese DM group than in non-obese control group. The Logistic regression analysis showed that the odds ratio of DD genotype was 0.265. The frequency of DD genotype was significantly higher in obese DM group than in obese control group. The Logistic regression analysis showed that the odds ratio of DD genotype was 3.991. (2) In the non-obese control group, the FPG and 2hCP of DD genotypes were lower than those of GG genotypes (P< 0.05, P< 0.01), the HOMA-B of DD genotypes was higher than that of GG genotype (P< 0.01). In the non-obese DM group, the waistline/hip ratio (WHR) of DD genotypes was higher than that of GG genotypes(P< 0.01). In the obese DM group, the WHR, HOMA-IR, 2hPG, 2hINS and 2hCP levels of DD genotypes were higher than those of GG genotypes, while the level of HOMA-B of DD genotypes was lower than that of GG genotypes. In the obese control group, the WHR, HOMA-IR, 2hPG, 2hINS and 2hCP levels of DD genotype were higher than those of GG genotype, and the HOMA-B level of DD genotype was lower than that of GG genotypes (P< 0.05). CONCLUSION: The relationships between G1057D variants of IRS2 and T2DM are mediated by obesity.

[The efficacy of half of the Global Initiative for Asthma recommended dose of inhaled corticosteroids in the management of Chinese asthmatics].

OBJECTIVE: To explore the optimal doses of inhaled corticosteroids (ICS) for the management of Chinese asthmatics in order to enhance cost-effectiveness. METHODS: A randomized, paralleled and controlled multi-center study was carried out in 11 provincial hospitals. After one week run-in period when only inhaled salbutamol as needed was given, the asthmatic patients (n = 86) were divided into two groups: patients in group G (n = 42) received inhaled ICS with the doses recommended by Global Initiative for Asthma (GINA), ie, fluticasone propionate (FP) 250 microg bid for moderate asthma and 375 microg bid for severe asthma. Patients in group H (n = 44) received half of the above doses of FP (125 microg bid for moderate asthma and 125 microg in the morning, 250 microg in the evening for severe asthma). RESULTS: There were no significant differences between group G and group H in the 32 measurements after management, including day symptom score (severe asthma in group G 0.7 +/- 0.8, group H 0.4 +/- 0.6 and moderate asthma in group G 0.4 +/- 0.5, group H 0.3 +/- 0.5), FEV(1) [severe asthma in group G (1.5 +/- 0.5) L, group H (1.8 +/- 0.6) L/min and moderate asthma in group G (2.3 +/- 0.6) L, group H (2.3 +/- 0.8) L/min, FEV(1)% of predicted [severe asthma in group G (54 +/- 17)%, group H (59 +/- 19)%, and moderate asthma in group G (79 +/- 14)%, group H (79 +/- 15)%], the increased morning PEF value (45, 67 L/min in moderate and severe asthma in group G and 56, 65 L/min in moderate and severe asthma in group H respectively), frequency of nights awakened (81, 69 in severe asthma in groups G and H respectively), numbers of well controlled cases (88 and 98 d in severe asthma in groups G and H respectively) and the use of extra bronchodilators (salbutamol, puff/day, 5.0 and 3.4 d in severe asthma in groups G and H respectively, P > 0.05). On the other hand, in the subgroups of moderate asthma, night symptom score (0.30 +/- 0.22 in group G, 0.13 +/- 0.33, t = -2.06, P < 0.05), number of exacerbations (3/22 in group G, 11/24 in group H, chi(2) = 4.74, P < 0.05), number of total controlled cases (18/22 cases in group G, 12/24 cases in group H, chi(2) = 4.97, P < 0.05) were significantly different between group G and group H. In the subgroups of severe asthma, the total days of exacerbations were different between group G and group H (11 days in group G, 6 days in group H, U = 31, P < 0.05). CONCLUSION: As a whole, similar therapeutic effect was achieved using half of the GINA recommended dose of ICS with/without salbutamol inhalation as needed compared with those receiving the GINA recommended full dose of ICS in most Chinese asthmatics.

[Infrared spectra analysis of chromium cation biosorbed by biosorbent ZL5-2].

Biosorption of Cr( VI) by the novel biosorbent ZL5-2 produced by Agrobacterium. sp was studied, and infrared spectra of native, absorbing and desorbing ZL5-2 were compared. The initial pH was important for biosorption. The optimum effect on Cr(VI) biosorption was acquired within initial pH 0.5-1.5, then with the pH increasing, the effect on Cr6+ biosorption was reduced. The biosorption for Cr(VI) was a quick process, only within 10 min 65.4% Cr(VI) was biosorbed, and the biosorption was in equilibrium after 60 min, then all the Cr(VI) was biosorbed after 80 min. The absorbed Cr(VI) could be recovered by desorption, and the desorption rate reached 13.6%-67.9%. The absorbance peak around 3 400 cm(-1), which reflected OH stretching vibration, moved about 8 cm(-1), and its absorbance decreased; the absorbance peak around 2 900 cm(-1) decreased, which reflected C-H stretching vibration; and the absorbance peak around 1 600 cm(-1), which reflected amide I group stretching vibration, moved about 13 cm(-1), and its absorbance decreased. After desorption, the absorbance peaks which reflected OH and C-H were increased to the initial degree, and the absorbance peak which reflected NH and amide I group did not increase. Therefore, authors presumed that the process of biosorption was reversible adsorption and irreversible adsorption simultaneously, however the reversible adsorption was more important in the biosorption.

[The UV-Vis spectra and substituent effect of organoimido derivatives of polyoxometalates].

In the presence of a carbodiimine, i.e. DCC, a series of organoimido derivatives of polyoxometalates have been synthesized via the reaction of [alpha-Mo8O26]4- with aromatic amines and its hydrochloride salt. Elemental analysis, IR, 1H-NMR and UV-Vis spectra were used to characterize those hybrids, in particular their UV-Vis spectra have been studied. The results show that typical metal-ligand charge transfer (MLCT) transitions occur in the organic-inorganic hybrid molecules. There is a good linear relationship between the shift of UV-Vis absorptions (delta lamda max) and conjugation effect of the p-substituted group (sigmaR).

[Arthroscopic transtibial double-bundle posterior cruciate ligament reconstruction using calcaneal-tendon allograft].

OBJECTIVE: To evaluate the surgical approach and the clinical outcome of arthroscopic transtibial double-bundle and double-tunnel posterior cruciate ligament(PCL) reconstruction using calcaneal-tendon allograft. METHODS: Ten patients with knee instability caused by PCL rupture were treated by arthroscopic PCL reconstructions using calcaneal-tendon allograft only by anterior approach between March, 2002 and April, 2003. First, the square bone block 10 mm in diamter, which is connected with a band of calcaneal tendon 12 mm in diameter, was fixed into the tibial tunnel. Then, the tendon was divided into two parts with a proportion of 3 : 2 (7 mm : 5 mm), which were fixed into the anterolateral (AL) and posteromedial (PM) tunnels in medial femoral condyle respectively. The knees were fixed with plaster slabs or braces for 4 weeks after operation. The periods of follow-up were 12 to 48 months (average, 32 months),and the outcome was evaluated by Lachman's test, posterior drawer test (PDT), and Lysholm scorses. RESULTS: Knee instability was seen no more in the 10 cases after operation. Recovery of stability in coronary axis was apparent. The results of Lachman's test and PDT were both positive in every case before operation, but became negative after operation. Extension and flexion of knee joints became normal. Lysholm scores rose from 58 (pre) to 93 (post). Joint function of nine cases reached excellent level, one case reached good level, and the total rate of excellence and good was 100%. Clinical observations suggested that immunological rejection was mild (only one patient had a low fever); meanwhile, no surgical complication happened. During 48 months, no ligament looseness was observed. Satisfactory clinical outcome was achieved in the whole group. CONCLUSION: Double-bundle and double-tunnel PCL reconstruction using calcaneal-tendon allograft is a good surgical approach, which not only avoids injuring the donor area, but also accords with the anatomical structure of PCL. The short-term outcome has been proved, but long-term outcome needs more observations.

[SSRHunter: development of a local searching software for SSR sites].

Progress in genome research has made it possible to develop new SSR markers by bioinformatics in a relatively narrow region of genome. To realize it, the first thing is to search for potential SSR sites. Any known methods have more or less defects. Efforts were made to develop a local SSR sites searching software in this study. The resultant software, SSRHunter, could accomplish this task perfectly. Furthermore, SSRHunter could provide automatic pretreatment of sequences, routine arrangement of sequences, sequence transformation and convenient report output.

[Restenosis and its management after metallic stents implantation in benign tracheal and main bronchial stenosis].

OBJECTIVE: To observe the occurrence of restenosis after metallic stents implantation in benign tracheal and main bronchial stenosis, and to evaluate the therapeutic effects of balloon dilatation, cryotherapy and electrocautery. METHODS: Thirty patients with benign tracheal stenosis and 35 patients with main bronchial stenosis caused by tuberculosis were treated with metallic stents implantation and a follow-up was carried out to observe the occurrence of restenosis. Combined balloon dilatation, cryotherapy and electrocautery were used to manage the restenosis. The lumen of stenotic segment, dyspnea index and pulmonary function were measured before management and after the patient's condition became stable. RESULTS: Restenosis occurred in 6 of the 30 patients receiving tracheal stenting and in 8 of the 35 patients receiving bronchial stenting, the restenosis rates were 20% and 22.86%, respectively. In total, 30 Chinese-made stents and 36 Ultraflex stents were implanted, and the restenosis rate was 20% and 22.2%, respectively (P > 0.05). The restenosis rate was 4/9 in the upper segmental tracheal stents, and was 9.09% in the middle-lower segmental stents (chi(2) = 5.114, P < 0.05, but chi(c)(2) = 3.100, P > 0.05). The restenosis rate was 16.67% in the fibrotic stage of endobronchial tuberculosis (EBTB), and was 60% in the inflammatory reaction stage of EBTB (chi(2) = 4.564, P < 0.05, but chi(c)(2) = 2.437, P > 0.05). The effective rate was 4/6 in the tracheal stenting restenosis patients, and was 2/4 and 2/2 in the upper and middle-lower segment groups, respectively. The effective rate was 7/8 in the bronchial stenting restenosis patients. After management, the lumen diameter of stenotic segment in the tracheal stenting restenosis patients increased from (3.33 +/- 1.63) mm to (9.33 +/- 3.98) mm (P < 0.02), the dyspnea index decreased from (3.67 +/- 0.52) to (1.50 +/- 1.64) (P < 0.02), the vital capacity (VC) increased from (1.39 +/- 0.17) L to (2.43 +/- 0.70) L (P < 0.01), the forced expiratory volume in one second (FEV(1)) increased from (1.02 +/- 0.15) L to (2.00 +/- 0.72) L (P < 0.02). After management, the lumen diameter of stenosis segment in the bronchial stenting restenosis patients increased from (3.00 +/- 0.76) mm to (7.38 +/- 2.00) mm (P < 0.001), the dyspnea index decreased from (1.63 +/- 0.52) to (0.50 +/- 0.76) (P < 0.005), VC increased from (1.74 +/- 0.16) L to (2.74 +/- 0.41) L (P < 0.001), FEV(1) increased from (1.41 +/- 0.19) L to (2.36 +/- 0.37) L (P < 0.001). CONCLUSION: Restenosis occurred in some patients with benign tracheal and tuberculous bronchial stenosis after metallic stent implantation. The restenosis rate was higher in the upper tracheal stenosis than that in the middle-lower tracheal stenosis. Caution should be taken when metallic stents are placed in this part of the trachea. The restenosis rate was higher in the inflammatory reaction stage of EBTB than in the fibrotic stage. Effort should be made to avoid placing metallic stents at this stage of the disease. Balloon dilatation, cryotherapy and eletrocautery are effective methods in managing restenosis after stent implantation.

[Determinants of long-term survival in 38 patients with carcinoma of ampulla of Vater treated by local resection].

OBJECTIVE: To investigate determinants of long-term survival for carcinoma of ampulla of Vater treated by local resection. METHODS: The clinical and pathological data of 38 such patients treated by local resection from 1983 to 2003 were retrospectively analyzed. According to UICC staging system, there were T1 30, T2 7 and T3 1. Lymph nodes were involved in 4 during operation which was present in primary lesions larger than 2 cm across. All patients were treated by local resection. At first, external palpation was carried out to ascertain accessibility. Then with the duodenum opened, direct exploration was carried out. On deciding for resection, the common bile duct was probe explored which guided the circumferential ring resection 1 cm, away from the tumor, including all layers of duodenum, ampula and partial bile and terminal pancreatic ducts and the posterial wall of duodenum was completed in steps. Meticulous care was taken not to suture the pancreatic duct and endotheliation was ensured at the mouth of common bile duct and duodenum. The basal tissue was frozen sectioned to ensure negative stumps. The gall bladder of 6 patients was also resected. SPSS 10.0 software was used in data processing, log-rank test used in univariate analysis and Cox equation for multivariate analysis and Kaplan-Meirer method for the survival rates. RESULTS: Thirty-eight patients received local resection giving an operative mortality of 0% and morbidity of 13.2%. The 1-, 5- and 10-year survival rate was 83.5%, 51.4%, and 38.9%, respectively, with a median survival of 3.35 years. Up to now, 13 patients have survived for more than five years and 2 patients beyond ten years. The tumour size, tumour grading, lymph node status and UICC stage were significant prognostic factors in univariate analysis. However, only lymph node status was a statistically independent predictor of prognosis in multivariate analysis. CONCLUSION: Local excision is safe giving low morbidity and good survival in carefully selected cases. Preferably it is indicated only in high risk patients with a pT1 and well differentiated ampullary cancer smaller than 1 cm in diameter.

[Study on the preparation and bio-distribution of silybin lipid nanospheres].

OBJECTIVE: To prepare silybin lipid nanospheres (SLN) and to evaluate the properties of morphology, particle size and the silybin distribution in mice. METHOD: Silybin lipid nanospheres were prepared by thin film emulsion-high pressure homogenization technique. Concentrations of silybin in mice blood, liver, spleen, lung, kidney, brain, heart, stomach after oral administration were determined by high performance liquid chromatography. RESULT: Scanning electron micrograph showed that most of the SLN were spherical. The average diameter from photon correlation spectrometer was 148.9 nm with the polydispersity 0.17. Body distribution data indicated that SLN could increase the distribution of silybin in blood and liver, decrease the amount of silybin in stomach as compared with the preparation on market, and the drug targeting index (DTI) in liver was 1.81. CONCLUSION: SLN can increase the uptake of silybin in liver after oral administration, which must benefit the hepatitis treatment.

[Changes of discharge rate and pattern of 5-hydroxytrypamine neurons of dorsal raphe nucleus in a rat model of Parkinson's disease].

In the present study, changes of the neuronal activity of 5-hydroxytrypamine (5-HT) neurons of dorsal raphe nucleus(DRN) in a rat model of Parkinson's disease (PD) were investigated with glass microelectrode recording. The results showed that the discharge rates of 5-HT neurons in control and PD rats were (1.61+/-0.56) Hz and (2.61+/-1.97) Hz, respectively. The discharge rate of PD rats was significantly increased when compared to that of the control rats. In control rats, 79% of 5-HT neurons discharged regularly and 21% in bursts. In PD rats, however, 36% of 5-HT neurons discharged regularly, 16% irregularly and 47% in bursts. The percentage of 5-HT neurons discharging in bursts was obviously higher than that of the control rats (P<0.05). The data suggest that the discharge rate and bursting pattern of 5-HT neurons in DRN are increased in a rat model of Parkinson's disease.

A modified mammalian tandem affinity purification procedure to prepare functional polycystin-2 channel.

The tandem affinity purification (TAP) procedure was initially developed as a tool for rapid purification of native protein complexes expressed at their natural levels in yeast cells. This purification procedure was also applied to study interactions between soluble proteins in mammalian cells. In order to apply this procedure to mammalian membrane proteins, we created a modified TAP tag expression vector and fused with the PKD2 gene, encoding a membrane cation channel protein, polycystin-2, mutated in 15% of autosomal dominant polycystic kidney disease. We generated epithelial Madin-Darby canine kidney cell line stably expressing TAP-tagged polycystin-2, improved the subsequent steps for membrane protein release and stability, and succeeded in purifying this protein. Using patch clamp electrophysiology, we detected specific polycystin-2 channel activities when the purified protein was reconstituted into a lipid bilayer system. Thus, this modified TAP procedure provides a powerful alternative to functionally characterize membrane proteins, such as ion channels, transporters and receptors, using cell-free system derived from mammalian cells.
