[A multicenter study of efficacy and safety of oseltamivir in the treatment of suspected influenza patients].

OBJECTIVE: To evaluate the efficacy and safety of oseltamivir in the treatment of suspected influenza patients. METHODS: A randomized, open, control and multiple center trial was conducted among 1176 individuals with suspected influenza. They were randomized either to oseltamivir group (oseltamivir 75 mg, twice daily for 5 days) or control group who were given symptom relief medicine. RESULTS: No difference was found between two groups in influenza symptoms of the patients (P > 0.05) before the treatment. (1) Oseltamivir treatment significantly reduced the duration of fever about 25 hours post treatment. (2) The AUC score of other symptoms was decreased by 160.21 (about 30.21%), and the duration was shorten by 20 hours. (3) The ratio and duration of antibiotic use in oseltamivir group were less than that of control group (P < 0.0001 and P < 0.05). (4) The incident rate of second generation influenza in oseltamivir group was also less than that of control group (P < 0.0001). (5) Secondary complications such as bronchitis, sinusitis and pneumonia occurred 3.23% in Oseltamivir group and 4.16% in the control group (chi(2) = 1.209, P = 0.272). (6) Digestive symptom such as light nausea were occur in patients who took oseltamivir, there was no difference of side effects rate between two groups (5.18% and 4.16%, chi(2) = 0.680, P = 0.410). (7) 75.68% (28) were confirmed with positive result of virus test in 37 suspected influenza patients. CONCLUSION: Our data suggests that Oseltamivir is effective and well tolerated in suspected influenza patients. It can reduces the duration and severity of influenza symptom and fever, decrease the incidence of suspected influenza in the contacted population, antibiotic using, and with light side-effect.

[Prevalence of obstructive sleep apnea-hypopnea syndrome in Chinese adults aged over 30 yr in Shanghai].

OBJECTIVE: To investigate the prevalence of obstructive sleep apnea-hypopnea syndrome (OSAHS) in adults aged over 30 yr in Shanghai, to provide data for estimating the burden of this disease and making strategies for treatment and prevention of this disorder. METHODS: 9,600 subjects (aged >or= 30 yr) were derived from a random sample of the population living in six streets, four districts in Shanghai. They were asked to answer the questions from a questionnaire concerning snoring, daytime sleepiness, and habits such as smoking and drinking, etc. According to the degree of snoring, 150 moderate and severe snorers were selected to be tested by portable PSG for whole night data. The prevalence of OSAHS was conservatively estimated from the result. RESULTS: 8,081 subjects (84.18%) had answered the questions, among them 6,826 (84.47%) were available for evaluation. The prevalence of ever snoring was 57.11%, moderate and severe ones 23.73%. Before the age of 70, the prevalence of snoring increased with age, and higher in males than females, particularly for moderate and severe snoring. Smoking and drinking were associated with the severity of snoring. The prevalence was higher in women post-menopause and with irregular menstruation. ESS was significantly different among snorers with various severities. When AHI >or= 5 was taken as the cut-point, the prevalence of OSAHS was 20.39%. After excessive daytime sleepiness (EDS) definited as ESS >or= 9 was added as a criterian, the conservatively estimated prevalence of OSAHS was 3.62%. CONCLUSIONS: The estimated prevalence of OSAHS in adults over 30 yr old was 3.62%. This can be a great burden to public health and implies that the disease needs better understanding and study.

[Treatment of distraction osteogenesis in the patients of obstructive sleep apnea-hypopnea syndrome with micrognathia].

OBJECTIVE: To apply the treatment of distraction osteogenesis(DO) to obstructive sleep apnea-hypopnea syndrome(OSAHS) patients with croniomaxillofacial deformities. METHODS: All 46 OSAHS patients with micrognathia are had polysomnography(PSG) study and cephalometric analysis. Their age from 4 to 18 years old, the mean age is 11.4. The number of temporal mandibular joint (TMJ) ankylosis with micrognathia, micrognathia; 1st & 2nd bronchial arch syndrome and crouzon syndrome patients were 32, 9, 2 and 3 respectively. All were treated with DO. Maxilla or mandible was advanced from 5 to 35 micrometers; the mean advanced distance is 18.34 mm. They were all revaluated by PSG and cephalometric analysis postoperatively. RESULTS: All patients have good respond to the treatment. They have a better appearance and the narrow upper airway was enlarged remarkably, their AHI drop from 66.31 +/- 14.74 pre-operately to 3.16 +/- 1.70 pro-operately, and minimal posterior airway space(PAS) from (5.48 +/- 2.76) mm to (9.97 +/- 2.05) mm. There is remarkable difference (P < 0.001). CONCLUSION: DO is a good method for the patients of OSAHS with micrognathia.

[A preliminary study on speech therapy for adult cleft palate patients].

OBJECTIVE: To study speech feature and treatment of adult with misarticulation by review 20 patients' clinical information. METHODS: 20 adult patients (10 male, 10 female, age ranging from 20 to 38 years, mean age 20.35 years) with misarticulation were available for this study. All received specialized examination, Chinese speech intelligibility test and blowing test before speech treatment, 17 cases underwent posterior pharyngeal flap operation before speech treatment. Speech treatment included audio discrimination, behaviour training, lip phone training, aspirates and fricatives training, tongue lolling out, comprehensive training, et al. RESULTS: All cases were restored normal speech approximately after speech treatment, consonants loss and hypernasality of some patients totally disappeared. But there were some complications in speech treatment of 4 cases. CONCLUSION: The outcome of appropriate adult speech treatment was satisfactory, though adult speech treatment was more difficult than child's.

[Stimulation and evaluation on maxillary distraction osteogenesis using CASSOS 2001].

OBJECTIVE: To simulate maxillary distraction osteogenesis and evaluate the change of soft and hard tissue before and after treatment, using Computer-Assisted Simulation System for Orthognathic Surgery( CASSOS 2001). METHODS: A fourteen-year-old boy with severe maxillary hypoplasia, due to unilateral cleft lip and palate, was analysed by cephalometric analysis. The simulations of maxillary distraction osteogenesis (Le Fort I osteotomy and Le Fort II osteotomy) were re-analysed. After the treatment, cephalometric analysis was preformed again. The data were compared. RESULTS: The maxillary hypoplasia was well treated using maxillary distraction osteogenesis; Compared with Le fort I osteotomy, more satisfactory results can be obtained by Le fort I distraction osteogenesis. CONCLUSION: Maxillary distraction osteogenesis is a better way to treat severe maxillary hypoplasia with operated CLP than maxillary osteotomy. CASSOS 2001 can help surgeons and patients on simulation and evaluation of maxillary distraction osteogenesis, and on decision of treatment plan.

[Clinical practice of behavior therapy in speech treatment].

OBJECTIVE: To apply behavior therapy to treat patients with velopharyngeal insufficiency(VPI),with the aim of improving speech treatment. METHODS: 20 patients(10 male, 10 female,ranged from 4.0 to 38.4 years, mean age 17.8 years)with VPI were available for this study. Of 20 patients, 5 with congenital velopharyngeal insufficiency,10 with posterior pharyngeal flap,5 with palatoplasty. Behavior therapy was adopted in speech treatment. The quantitative measurement of Chinese speech intelligibility and blowing test was carried out before speech therapy and after speech therapy. RESULTS: The effect of behavior therapy in speech treatment was satisfied. Their Chinese speech intelligibility and blowing test after speech therapy were obviously better than before. CONCLUSION: The behavior therapy is a very effective method in speech in treatment, but it was important to select indications and treat individually.
