[Clinical application of the anterolateral thigh flap in 112 patients].

OBJECTIVE: To investigate the anatomic variations of the perforator vessels of anterolateral thigh (ALT) flap and the clinical indications. METHODS: From March 1985 to August 2004, the anterolateral thigh flap graft was performed in 112 patients. The clinical data were analyzed. There were 67 males and 45 females, aging from 5 to 65 years with an average of 38. 5 years. According to recipient site condition, four methods of flap harvesting were as follows: (1) 78 received free fasciocutaneous flaps; (2) 22 received free adipofascial flaps; (3) 5 received pedicled island fasciocutaneous flaps; (4) 7 received pedicled reverse-flow island fasciocutaneous flaps. Facial, neck, breast, extremity joint, plantar, and perineum defects were repaired and the effectiveness and donor site morbidity were evaluated. RESULTS: The blood supply of ALT flap came from the descending branch or transverse branch of the lateral circumflex femoral artery. The skin vessels were found to be septocutaneous perforators in 33% of flaps and to be musculocutaneous perforators in 77% of flaps. Of 112 flaps, 107 survived completely, the survival rate was 95.6% with little donor site morbidity. CONCLUSION: ALT flap is a versatile soft-tissue flap. If refined to perforator flap, it can achieve better results in reconstructing defect and minimizing donor-site morbidity.

[Construction and expression of basic fibroblast growth factor mammalian expression vector].

OBJECTIVE: To construct a mammalian expression vector of basic fibroblast growth factor (bFGF) and to investigate the expression of bFGF in vitro and in vivo. METHODS: A mammalian expression vector pcDNA3. 1/ myc-His(-)C-bFGF was constructed with gene cloning technique. The mammalian expression system was prepared and purified. The expression of bFGF cDNA in cultured transfected cells was examined by RT-PCR and cell immunohistochemistry. The recombinant plasmids, pcDNA3. 1/myc-His(-)C-bFGF and pCD2-VEGF121, were transferred into rabbit cervical muscle by direct injection of plasmid following electric pulses in vivo. The transferred gene expression and the biological effect were measured by use of histochemistry and immunohistochemistry analysis. RESULTS: The eukaryon expression system pcDNA3. 1/myc-His (-)C-bFGF could express the target protein bFGF in vitro. The recombinant plasmids, pcDNA3. 1/myc-His(-)C-bFGF and pCD2-VEGF121 were transferred into muscles flap in vivo successfully. The active proteins bFGF and VEGF121 were expressed at high levels. Blood vessels increased significantly in the muscles, and blood circulation was improved by local angiogenesis. CONCLUSION: The eukaryon expression vector of bFGF is constructed and can be expressed successfully in vitro and in vivo. That is a primary preparation for the research on tissue transplantation and tissue engineering with bFGF gene therapy.

Alternative method to improve the repair of the donor site of the anterolateral thigh flap.

The groin flap was designed to improve the conventional repair of the donor site of the anterolateral thigh flap. The modification method was applied successfully to cover the defects of donor sites of anterolateral thigh flaps in five patients. An ideal color match and a stable and completed composition for the donor sites were obtained. Little morbidity of the donor sites occurred. Cosmetic and functional rehabilitations of the donor sites were achieved. These results suggest the potential and novel method to improve the repair of the donor sites of anterolateral thigh flap.

[The applications of the dermal vault technique in mammaplasty].

OBJECTIVE: To search for the applications of the dermal vault in mammaplasty. METHODS: The dermal vault was planned as large as possible according to the degree of the macromastia and mammaptosis. The residual gland was sculptured and fixed with the dermal vault in order to reconstruct the protrude, pliable and living breasts. RESULTS: The 27 cases (53 breast) were performed with this technique. The result is satisfactory and there are no complications. CONCLUSION: It is a good method to use the dermal vault as a sculpturing and fixing material in mammaplasty. The breasts formed are projecting, pliable and moveable. The scar is not apparent and the result is persistent.

[Three-dimensional observation of the vasculature in the anterolateral thigh adipofascial flap and its clinical applications].

OBJECTIVE: To investigate the blood supply patterns of the anteriorlateral thigh adipofascial flap for clinical applications. METHODS: To investigate the blood supply patterns of the anteriorlateral thigh (ALT) adipofascial flap, 16 fresh cadavers were anatomically examined by intraarterial injection of colored latex. Three-dimensional analysis of the vasculature of the subcutaneous adipofascial tissue was performed. Sixteen patients underwent microsurgical correction with the ALT adipofascial flap. Among them, there were fifteen with hemifacial atrophy, one with micromastia. RESULTS: The three-dimensional arterial structures of the ALT adipofascial flap were the same in all the cadaver specimens. Each layer of the adipofascial tissue was supplied by several blood vessels of the axial pattern, especially in the deep layer. Sixteen patients were successfully treated with this flap. The postoperative follow-up ranged from six months to eleven years. There was not postoperative flap necrosis or absorption of the fatty tissue. Stable restoration of the facial contour or the breast was achieved. The donor-site morbidity was minimal. CONCLUSION: The blood supply of the ALT adipofascial flap is reliable. A considerable amount of the fatty layer of the flap can be removed primarily. It is a preferable procedure for reconstructing soft tissue defects.

[Repair of large facial defect with an expanded clavicular-pectoral skin flap].

OBJECTIVE: To search for an ideal method to repair large facial defects. METHODS: One or two soft tissue expanders were implanted at the clavicular-pectoral region. After fully expansion, a clavicular-pectoral skin flap was designed and created with similar color, texture, and size to the facial defect. The flap would pass over the neck and transfer to the face to repair a large defect. RESULTS: 10 cases were treated with this method. All have been followed up for 12-18 months with satisfactory results. CONCLUSION: The large facial defects could be repaired by the expanded clavicular-pectoral skin flap without damaging the natural neck contour and donor site abnormality was avoided. The patients get an aesthetic look and good function.
