A pocket computer program for obstetric ultrasound calculations.

A pocket computer program is currently being used to perform complicated obstetric ultrasound calculations at the Medical Center Hospital of Vermont. The program calculates decimal weeks between dates, gestational age from biparietal diameter, crown-rump length or femur length, total intrauterine volume, head-abdominal circumference ratios, projected gestational age from previous measurements, and estimated fetal weight. Calculations can be printed for a permanent record. The pocket computer program provides a convenient, accurate, and affordable method of obstetric ultrasound analysis.

'Leopard spot' retinopathy in Warburg syndrome.

A female infant with brain and muscle abnormalities characteristic of Warburg syndrome had an atypical retinal dysplasia which appeared clinically as a 'leopard spot' retinopathy. The retinal histology is described in detail and is remarkable for periodic thinning of the inner nuclear layers and for isolated rosettes in the periphery. We believe 'leopard spot' retinopathy is yet another ocular manifestation of the Warburg syndrome.

Asymptomatic scrotal masses in children.

The ultrasound scans of 16 boys with asymptomatic scrotal masses were retrospectively studied. Ultrasound localized seven of the masses as primary testicular lesions and nine as extratesticular lesions, six of which were hydroceles. Ultrasonography also characterized the scrotal masses as predominantly cystic or solid. Ultrasound could not differentiate benign from malignant disease when a solid mass was found. When a hydrocele was identified, ultrasound was able to image the underlying testis and accurately evaluate it for primary disease. These ultrasound observations can help the surgeon decide when to explore the scrotum and whether to approach the testes via an inguinal or scrotal incision. Conversely, ultrasonographic confirmation of a normal testis may prevent needless surgery and the removal of a normal testis.

Ultrasonic evaluation of abdominal aortic thrombus.

Ultrasonograms of 38 patients and computed tomograms of 21 patients with abdominal aortic aneurysms (AAA) were retrospectively reviewed. In 69 per cent of cases in which intraluminal thrombus was demonstrated, sonography revealed a high-amplitude linear echo along the luminal surface of the thrombus. Although calcification could be easily visualized within the aortic wall by computed tomography, it was seen at the lumen--thrombus interface in only one patient. In addition, a resected aortic aneurysm with thrombus, when scanned in a water bath in vitro, demonstrated this echo pattern at the luminal-thrombus interface on sonography but revealed no calcification on pathologic examination. An explanation of the source of this echo pattern and a caution against misinterpretation of this echo pattern as the aortic wall in a dissection or leaking aneurysm with hematoma are presented.

Abdominal pseudotumor in childhood: distended gallbladder with parenteral hyperalimentation.

Two children receiving total parenteral hyperalimentation had palpable upper abdominal masses. Sonography showed these masses to be distended gallbladders. Sonograms of four other children receiving parenteral hyperalimentation were examined to determine if gallbladder distension in these cases was coincidental or common; three of four also had gallbladder distension. Follow-up sonograms after initiation of oral feedings in two children showed reduced gallbladder dimensions. Children can have palpable, distended gallbladders while fasting and receiving parenteral hyperalimentation. Sonography can differentiate distended gallbladders from other abdominal masses.

Studies with timed-pregnant squirrel monkeys (Saimiri sciureus).

Timed-pregnant monkeys were produced in a large nonhabituated colony of Saimiri sciureus of Bolivian origin. In a colony of 373 females and 40 males, 277 females (74%) were considered to be inseminated, based on microscopic observation of sperm and/or detection of a coagulum (plug) in the vagina. Forty-six full-term progeny were delivered. The mean gestational period was 152.5 days (SD = 3.9 days). For continuous cohabitation, the median time to insemination was five days, with 75% of inseminations occurring within eight days. Pregnancy evaluation (mouse bioassay) indicated a high level of resorptions within the first 50 days of gestation. This may help explain the low birth rates reported for other nonhaibuated colonies.
