[Computed tomographic imaging of the dentoalveolar-maxillary sinus lacuna of the dentate jaw].

A human bone segment of the lateral maxilla was scanned using high-resolution computed tomography (CT) in contiguous parallel axis slices in order to image the topographical relationship between upper molars and the maxillary sinus. The bone segment was dissected analogously in 1 mm thick sections. After exposing contact films from these jaw cuttings, microsections were made for histological interpretation. The radiological-histological comparison between the CT-scans, contact films and the histologic specimens revealed absolute differences of 0.2-0.6 mm. The high resolution CT allowed an interpretation of details located between teeth and bone when the minimum bone thickness was 0.5 mm or more. First experiences with the CT-technique in orthodontic patients were promising, especially for the differential diagnosis of the dentoalveolar maxillary sinus relation.

[Early results of implantation of an unconstrained metatarsophalangeal joint prosthesis of the first toe].

PURPOSE: Does the ReFlexion Implant System provide an alternative in operative treatment of hallux rigidus? METHODS: We have been using the non-constrained prosthesis since April 1997. 26 implants were reviewed 13-41 months after insertion, analysing radiological and clinical data. A questionnaire was answered by the patients. RESULTS: Surgery was either performed cementless (8 cases), as a hybrid, i.e., only phalangeal part cemented (14 cases), or cemented in both components (4 cases). So far, 3 joint replacements had to be removed for pain. Radiological signs of loosening around the cemented phalangeal components were evident in 11 cases. There was no correlation between radiological loosening and discomfort. CONCLUSIONS: The implant can reduce pain distinctly. Patients satisfaction with the treatment is relatively high. The range of movement in the first MTP is still limited. In our opinion the ReFlexion Prosthesis can be quoted as an acceptable alternative for surgical treatment in advanced cases of hallux rigidus. In case of failure a conversion to the distraction-arthrodesis is possible.

[Measuring plantar weight distribution of shoes with forefoot support].

PURPOSE: Differences in weight bearing using forefoot relief ortheses available on the German market. METHODS: Six different forefoot relief shoes were tested. All ten probands wearing the same shoe-size, had a physiological foot anatomy and a normal gait. Contact time, contact area, maximum pressure and the pressure-time integral were assessed in six different areas of the sole. We used the dynamic device Fast SCAN by MEGASCAN placing the sensor inside the orthesis. RESULTS: A reduction of the pressure underneath the forefoot was seen with all sample shoes. In particular 3 types showed favourable parameters. The Metasan walking aid keeping the forefoot hanging free, reduced the pressure to almost zero compared to barefoot walking. The Forefoot relief orthesis with protective shield--also not supporting the forefoot--permitted a reduction down to 20% of the barefoot pressure. The Forefoot relief orthesis Dr. L. S. Barouk is reducing the pressure to 20% as well, allows a safer gait by lifting the front parts of the foot. Concerning the gait line remarkable differences between the shoe-types are seen. In all samples the front tipping edge of the shoe was free of peak loading. CONCLUSIONS: The shoes tested do reduce the pressure under the toes and under the metatarsals. They are not suitable for the postoperative care of proximal metatarsal or tarsometatarsal osteotomies.
