Small retinal hemorrhages as the only sign of an intracranial aneurysm.

PURPOSE: To report a woman with two small circumpapillary and nerve fiber layer hemorrhages found on routine examination. METHODS: Case report. The patient had a history of severe headache 2 weeks previously but no other symptoms or signs. RESULT: Magnetic resonance imaging showed an intracranial aneurysm. CONCLUSIONS: This potentially lethal abnormality should be considered with small unexplained retinal hemorrhages, even when the neurologic examination is normal. A history of recent headache should be sought.

Clinical and histopathologic studies of two families with lattice corneal dystrophy and familial systemic amyloidosis (Meretoja syndrome).

Lattice corneal dystrophy associated with familial systemic amyloidosis (Meretoja syndrome) has rarely been described other than in patients of Finnish origin. The authors report two North American patients with this disease who manifest blepharochalasis, lattice corneal dystrophy, open-angle glaucoma, and cranial neuropathy. In one patient, a corneal intraepithelial and subepithelial pseudodendrite was managed by superficial keratectomy, and this same patient benefited from surgical brow suspension for facial muscular weakness. In the second patient, penetrating keratoplasty was complicated by a neurotrophic persistent epithelial defect. Corneal tissue from both superficial keratectomy and penetrating keratoplasty exhibited ultrastructurally characteristic amyloid filaments and associated elastoid material. Transmission electron microscopy of conjunctiva and skin biopsies similarly revealed amyloid deposits associated with most basement membranes, the perineurium and endoneurium of most peripheral nerves, and the intima and media of arteries. By immunoperoxidase staining, the corneal amyloid deposits were positive for the amyloid P-component protein but negative for the nonimmunoglobulin amyloid A protein and prealbumin. Serum prealbumin and amyloid A related protein were normal.

The breakdown of polypropylene in the human eye: is it clinically significant?

Polypropylene intracameral sutures and intraocular lens haptics have been reported to develop cracks after varying lengths of time within the eye. A study of the degradation of a 10-0 polypropylene suture and the absence of noticeable degradation on a 4-0 polypropylene intraocular lens haptic removed five years and three months after implantation is presented. The relationship of the molecular orientation of the polypropylene suture to the clinical significance of its cracks is discussed.

Neodymium:YAG laser cutting of intraocular lens haptics.

Neodymium:YAG laser cutting of polymethylmethacrylate and polypropylene anterior chamber and posterior chamber intraocular lens haptics was studied in terms of ease of transection and physical structure of the cut areas as seen by scanning electron microscopy. A marked difference was discovered, with the polymethylmethacrylate cutting easily along transverse planes, whereas the polypropylene resisted cutting along longitudinal fibers. Clinical guidelines are presented.

Surgically induced corneal astigmatism and its spontaneous regression.

Serial keratometry readings were obtained pre- and postoperative cataract extraction utilizing all silk closure. The induction of with-the-rule cylinder and its spontaneous regression was plotted over 24-weeks. The data are compared to others utilizing all nylon closure.

The detrimental effect of aspirin on hyphema rebleed.

One hundred ninety-five cases of traumatic hyphema admitted to 2 medical centers over an 8 year period were studied in terms of factors effecting the rebleeding rate. It was found that aspirin given for analgesia had a detrimental effect on the rebleeding rate, and that dilating the iris offered some protection against this complication. This report is the second one to show association of higher rebleeding rate and aspirin.
