THE FUNCTIONING OF A PRIVATE PSYCHIATRIC CLINIC.

SOME OF THE CLINICAL ADVANTAGES OF A PRIVATE PSYCHIATRIC CLINIC ORGANIZED FOR GROUP PRACTICE ARE: Readily available consultations with colleagues; cross-referral with better communication; more evenly filled hours; larger pools of patients for establishing and maintaining group therapy; better off-duty coverage; ready availability of the three disciplines, psychiatry, psychology and social work; satisfaction to the private psychiatrist of being able to arrange competent and prompt treatment for patients unable to meet usual fees; and cooperative research.Financial advantages include economies of time and money, a profit sharing plan, and availability of group life insurance, health benefits and social security.

Oxytocic activity of basic (aminomethyl) derivatives of phenols and related compounds.

A series of piperidinomethyl and related derivatives of naphthols, substituted phenols and indoles has been tested for oxytocic activity, using in vitro and in vivo methods of assay. Some of the compounds possessed very high activity, exceeding that of ergometrine. Activity was not associated with any structural resemblance to the ergot alkaloids.Highest activity occurred with 2-piperidinomethyl derivatives of phenols, among which maximum potency was conferred by substitution, at both the 4- and 5- positions, by methyl or ethyl or by linkage of these positions to form an indane derivative. In all series, piperidinomethyl derivatives were more active than those formed with other bases and methylation in the position alpha- to the nitrogen atom augmented the activity of both piperidine and morpholine derivatives. Among 2'-methyl piperidinomethyl phenols, the (-) form was more active than the (+). Acylation or alkylation of the phenolic hydroxyl group did not affect activity. The oxytocic activity was specific, the compounds being less effective upon other forms of smooth muscle. Effects upon blood pressure and respiration of a central nature were observed.

Methodology in medical research; the need for controlled clinical studies.

Too many medical researchers vitiate their work by ignoring the problem of uncontrolled variables. They therefore publish clinical impressions "dressed up" in scientifically meaningless numbers. A prototypical example of this practice is contrasted with a controlled study, each employing the same (small) number of patients. It is shown how the use of controls can convert a meaningless experiment into one that has assessable scientific significance.A survey of current literature revealed that in only 21 of 100 articles studied were adequately controlled experimental conditions employed. Since they usually deal with very complex systems, it is urged that medical researchers exercise more scientific rigor with regard to control problems.
