Anti-IgA antibodies in IgA-deficient children.

IgG and IgM isotype antibodies to polyclonal human IgA, myeloma IgA1, and myeloma IgA2 were estimated in 38 IgA-deficient children aged between 0.9 and 15 years. All children had IgM anti-IgA antibodies. IgG antibodies against either polyclonal IgA, IgA1, or IgA2 were present in 63% of the IgA-deficient children. IgG anti-IgA antibodies were detected against all three antigens in 8 of 11 severely IgA-deficient children and in 7 of 27 partially IgA-deficient children, but in only 1 of 23 healthy adult controls. The proportion of children with IgG anti-IgA antibodies was significantly greater in the severely IgA-deficient group in comparison with the partially IgA-deficient group and the adult controls (chi-square test, P less than 0.01 and P less than 0.005, respectively). There was a strong correlation within each IgG subclass between antibody responses toward each of the three IgA antigens. Twenty-four children were followed over a period ranging from 0.9 to 11 years (mean, 2.3 years). Three children who were initially IgG anti-IgA antibody negative became antibody positive and three who were antibody positive became antibody negative. Five children with severe IgA deficiency remained severely IgA deficient and IgG antibodies to IgA persisted in all five at follow-up. The presence of IgG anti-IgA antibodies did not influence the normalization of serum IgA at follow-up in 14 of 19 children who were initially partially IgA deficient.

Alcoholism in residency program candidates.

Recognition of physician impairment due to alcoholism and drug dependency is increasing. Medical students who are in the early phases of the disease are being recognized and treated more frequently today. Yet students who achieve sobriety are confronted with a generally hostile reaction from directors of residency training programs. In this article, the authors report on results of a survey of policies and attitudes of a sample group of program directors and suggest that there is a need to adopt policies for accepting residency applicants that take into consideration the fact that favourable outcomes are provided by treatment of alcohol- and drug-dependent medical students and physicians.

Linking personal computers to a hospital laboratory computer.

In the Department of Pathology, Norwood Hospital, a number of individual microcomputers in the home and throughout the laboratory have been connected to a central hospital laboratory computer. The microcomputers are connected to the central computer over standard telephone lines via the autoanswer modem for any telephone. Here, the author describes the system and its benefits, and explains why this type of application is a major potential use of computers in medicine in the future.

Surgical pathology reports with a portable microcomputer.

A microcomputer reporting system for gross and microscopic surgical pathology is simple and inexpensive, reduces the need for clerical personnel, and saves the pathologist's time in the preparation of reports. Gross descriptions are performatted. A dictionary of microscopic diagnoses is constructed and stored on disks. The diagnosis to be used for a case is selected by a pathologist by number. It is called to a video screen from disk storage by entering the number on the microcomputer keyboard. The diagnosis is edited or expanded for an individual patient while it is on the screen and is printed after changes are made. Each diagnosis contains preentered classification, coding, and filing information, as well as format, medical terminology, and spelling. The system has rapid retrieval and search capability and may be used for patients awaiting frozen-section diagnosis. The system requires fewer clerical personnel in surgical pathology than with free-form reporting.
