Sixteen months' experience with the Illouz technique of lipolysis.

The Illouz technique of lipolysis was used in 167 patients over a 16-month period. Five hundred procedures were done in a variety of areas. The operation has been performed almost entirely as an outpatient procedure with excellent to good results in almost all areas and minimal complications. The technique is detailed, along with considerations of fluid replacement and postoperative care.

Correction of hammer toe surgery deformity by Z-plasty and bone graft.

Some of the older orthopedic techniques for correcting hammer toes and corns called for major resection or even total resection of a phalanx of the toe. The late result of some of these procedures was a deformed, flail, or contracted toe on the dorsum of the foot. The patient described had such deformities on both little toes. Surgical correction included Z-plasties for the skin, extensor tendon resection, and bone grafts. Three-year follow-up has shown the technique to be a safe and satisfactory long-term method of reconstruction.

The muscle-suspension lower blepharoplasty.

The muscle-suspension (or muscle sling) lower blepharoplasty is a technique that can be used to some extent for all lower lids in which tightening and smoothing is desired. It seems to provide an extra degree of support by counteracting the natural tendency of gravity to produce scleral show or ectropion when the lid skin is tightened. It consists of anchoring a sling of orbicularis muscle to the periosteum of the lateral orbital rim, with an upward and lateral pull--while the skin is pulled in a more medial or upward direction.

Salvage of the ear in acute trauma.

Salvage of the ear in major acute trauma has been refined over the years. When severed parts were merely discarded, patients had to accept the defect or seek correction by secondary reconstruction. As primary reconstruction improved, the severed parts were reattached minimizing late deformities. Even when severed parts were not available, primary reconstruction advanced to meet the need. In the future it appears that immediate microvascular reconstruction will surpass our present achievements.

Clinical applications of free flap transfer in the burn patient.

Free microvascular flap transfer is indicated in situations in which flap coverage is required and no local flap is available. The technique of transfer of a groin flap based on the superficial circumflex iliac vessels was described. The procedure has been applied to two patients with acute electrical burns with extensive soft-tissue loss, three patients with scar contractures, and one with osteoradionecrosis. Satisfactory results were achieved in all patients. The procedure is contraindicated if recipient or donor vessels are abnormal or inadequate, and in the face of fulminating infection, general debility, and obesity. With scrupulous attention to indications and contraindications, the procedure is safe, reliable, and a valuable addition to the surgeon's armamentarium.
