Informants' knowledge of reproductive history and estrogen replacement.

BACKGROUND: There has been much interest in assessing estrogen use in healthy older women and those with Alzheimer's disease. However, data for the women with Alzheimer's disease must be obtained from an informant. The aim of this study was to better understand what informants are likely to know about reproductive history and estrogen use. METHODS: Reproductive history data from informants of Alzheimer's patients were modeled by comparing responses from 40 cognitively healthy older women with that of a designated informant. The designated informants were similar in demographics to informants for patients with Alzheimer's disease. RESULTS: Informant data regarding reproductive history was likely to be accurate, when known. However, 30% of the subjects did not identify an informant who had personal knowledge of them. Of those informants who had personal knowledge of the subject, accuracy for those who reported that they knew the information varied depending on the aspect of reproductive history assessed (age of menarche, 29%; age of menopause, 20%; pregnancies, 63%; live births, 92%; hysterectomy, 92%; and postmenopausal estrogen use, 82%). Daughters served as the most likely and most accurate informants in this study. CONCLUSION: This study demonstrates that information obtained from informants for patients with Alzheimer's disease is likely to be accurate for some but not all aspects of reproductive history. Of concern for such studies will be the 30% of patients who do not have an informant with personal knowledge about them.

Spontaneous pregnancies and pregnancies as a result of treatment on an in vitro fertilization program terminating in ectopic pregnancies or spontaneous abortions.

Five hundred three patients were accepted for the in vitro fertilization (IVF) program from 1983 to 1986. Two hundred ninety-nine patients had a total of 678 IVF treatment cycles during this period. Eighty-one clinical pregnancies resulted, of which 8 were ectopic pregnancies and 19 spontaneous abortions (group A). During this period 82 patients accepted for IVF became pregnant spontaneously: 44 before treatment and 38 after a failed IVF treatment cycle (group B). Of these, 14 were ectopic pregnancies and 8 were miscarriages. The main indications for acceptance on the IVF program were similar in the two groups if those patients who had a bilateral salpingectomy are excluded from group A. Causative factors for the high ectopic pregnancy rate are discussed. The authors suggest that after embryo transfer, migration of the embryo or embryos into the fallopian tubes occurs more frequently than realized, and the diseased tube is less likely than a normal tube to propel the embryo back into the uterus. The abortion rate in group B was similar to the rate in the general population.

Intestinal parasites of small mammals from Grand Teton National Park.

A study of the prevalence and identity of Giardia spp. in small mammals of Grand Teton National Park was undertaken. All 90 montane voles examined were positive for Giardia, as were 4 pocket gophers, 1 water shrew, 4 water voles, and 2 meadow voles. How and why these findings contrast with the findings of others are discussed.

An investigation into the levels of radiation exposure in diagnostic examinations involving fluoroscopy.

In order to investigate the levels of radiation exposure resulting from fluoroscopic examinations, area-exposure product measurements were performed on 6532 patients whilst undergoing a variety of examinations at a large district general hospital. Results for both the same and different types of examinations, performed in two different X-ray rooms by a number of different radiologists, are compared in order to highlight some of the factors which influence the wide variations in patient exposure which frequently occur in radiological examinations. Variations in exposure of patients of different weights are also presented.

Medical rehabilitation in the present and a promise for the future.

Advances in medical care have made it possible for individuals to survive serious accidents or illness. Many of these survivors will be disabled, a significant portion severely disabled. Dependence caused by disability results in enormous social costs. Medical rehabilitation can reduce such costs by helping the disabled utilise their abilities and increase their independence. Attempts to control costs of health care through health care financing have led to new approaches to providing rehabilitation services to those suffering from chronic impairments and long term disabilities.

Social and psychological changes after tubal sterilization: A reevaluation study on 425 women.

Four hundred and twenty-five women replied to a questionnaire designed to assess social and psychological changes occurring after tubal sterilization. Dissatisfaction with the outcome of the operation was unrelated to patient age at the time of the operation, parity, religious affiliation and history of emotional instability. While the incidence of negative social and psychological changes was low, a substantial proportion of the sample reported positive effects, particularly in emotional and sexual adjustment.

Redescription of flagellar arrangement in the duck intestinal flagellate, Cochlosoma anatis and description of a new species, Cochlosoma soricis n. sp. from shrews.

Cochlosoma anatis Kotlán (Zoomastigophorea, Retortamonadida, Cochlosomidae), isolated from the large intestines of domestic Rouen ducks, and Cochlosoma soricis n. sp., isolated from the small intestines of shrews, were observed by light and scanning electron microscopy. In both organisms, a single flagellum inserted on the dorsal surface at the same level as the insertion of 4 other flagella on the ventral surface. The 4 ventro-lateral flagella emerged from the left side of the anterior attachment disk below the margin and just above the lateral groove which extended the length of the organism. A 6th flagellum emerged from the margin of the attachment disk. The proximal ends of the flagella formed a bundle with the distal ends becoming unraveled like a rope. During motility, the bundle portion extended straight out from the cell and the free ends of the flagella produced a whipping motion. In C. anatis, the dorsal surface was covered with knob-like lumps and small pits and the cells had an axostyle that emerged slightly to the right of the midline in the posterior 1/3 of the body. The axostylar tip was shorter and thicker than the flagella and in most cells it also had an irregular, knobby appearance. The irregular cell surface and axostyle were absent from C. soricis. The margin of the attachment disk curved toward the center and terminated in C. anatis as a straight edge while in C. soricis it continued as a spiral. Indentations in the mucosal brush border similar to those produced by Giardia, but distinctly belonging to Cochlosoma, were interpreted as points of attachment to the host.

Splenomegaly and reticulocytosis caused by Babesia microti infections in natural populations of the montane vole, Microtus montanus.

A survey for Babesia microti in rodents was conducted at six sites within Grand Teton National Park, Wyoming. Blood and spleen smears, hematocrits, and reticulocyte counts were made on all of the animals to evaluate parameters for the diagnosis of babesiosis. Ticks were removed for identification. Of 257 Microtus montanus, 103 were infected with B. microti. In addition, five of 12 Microtus pennsylvanicus and one of three Arvicola richardsoni were parasitized by B. microti. Peromyscus maniculatus (n = 40) were not infected. Concurrent infections by Hepatozoon sp., Trypanosoma sp., and the bacterium, Grahamella sp., were noted in blood smears from a number of M. montanus. Splenomegaly and reticulocytosis were significant parameters associated with babesiosis while decreased hematocrit was not. Ticks removed from the voles were identified as Ixodes eastoni and were the probable vectors of the B. microti.
