Glucose-lowering effect of fenugreek in non-insulin dependent diabetics.

The effect of fenugreek on postprandial glucose and insulin levels following the meal tolerance test (MTT) was studied in non-insulin dependent diabetics (NIDDM). The addition of powdered fenugreek seed (15 g) soaked in water significantly reduced the subsequent postprandial glucose levels. The plasma insulin also tended to be lower in NIDDM given fenugreek but without a statistical difference. Fenugreek had no effect on lipid levels 3 h following the MTT. Fenugreek may have a potential benefit in the treatment of NIDDM.

Basal metabolic rate of Indian men: no evidence of metabolic adaptation to a low plane of nutrition.

Basal metabolic rate (BMR) was measured in 58 men of a rural Indian population who had low body weight and body fat stores. Comparisons of subgroups of subjects of different socio-economic status and different nutritional status showed no evidence of differences in BMR when corrected for differences in body weight and body fat content. This suggests that metabolic adaptation did not account for differences in absolute BMR between subgroups of this population. The BMR values agreed well with values predicted by equations based on BMR measurements in Asian men of higher body weight, but were below values predicted by the new FAO/WHO/UNU prediction equations by an average of 12.1 per cent (P less than 0.0001). This overprediction could be a reflection of allometric or climatic influences on BMR rather than evidence for metabolic adaptation to a low plane of energy nutrition.

Fingers or spoons to make oral rehydration solution?

The accuracy and variability of the composition of oral rehydration solution (ORS) prepared by village health workers using (i) a finger measurement technique and (ii) a special ORS measuring spoon were compared. The sodium and sucrose concentrations were measured in 130 ORSs prepared by each technique. All the spoon-measured ORSs had acceptable levels of sodium and sucrose, compared with 93% of the finger-measured ORSs. Only 2.3% of finger-measured ORSs had hypertonic sodium levels. The variability of sodium and sucrose levels was significantly greater with the finger measurement technique. This comparison should assist programme managers to decide which technique to adopt. Both techniques require careful instruction to ensure accuracy.

[Estimation of heart size--x-ray versus 2-dimensional echocardiography].

Radiologic heart volume measurement (HV) and two dimensional echocardiographic estimation of left ventricular volume and ejection fraction (EF) were carried out in 108 healthy subjects and 68 patients with hypertony, dilatative cardiomyopathy and hypertrophic-obstructive cardiomyopathy (HOCM). Healthy subjects show a close relationship between echocardiographic determined volume and absolute heart volume (HV), in patients with hypertony, dilatative cardiomyopathy and HOCM there is also a significant correlation. However, no correlation exists between EF and HV and HV/kg in patients with hypertony and HOCM, whereas patients with dilatative cardiomyopathy show a high significant inverse relationship. It is concluded that these results are caused by great differences in muscle mass. The clinical value of radiologic heart volume as a parameter of left ventricular systolic function seems to be low and depends on cause of the heart enlargement.

Coagulant property of an indigenous seed: home water treatment in Tamil Nadu.

Use of some vegetable substances as coagulant has been recorded in historical literature. A seed from the tree known as tettan cottai is used today in South Indian villages to purify water. The scrapings of the seed were shown in laboratory experiments to have coagulant properties. It would be extremely valuable to have additional measurements made in South India, where the practice is employed in removal of turbidity, and also to have evidence of the extent of removal microorganisms.
