Acquired pits of the optic nerve. Increased prevalence in patients with low-tension glaucoma.

The authors studied 232 glaucoma patients in an attempt to shed light on the implications of the pit-like localized cupping of the optic nerve often referred to as an "acquired pit of the optic nerve" (APON) both in low-tension glaucoma and in glaucoma associated with elevated intraocular pressure (IOP). Twenty (74%) of the 27 patients with low-tension glaucoma had APONs, whereas 31 (15%) of the 232 patients with elevated pressure did (P less than 0.001). Overall, there was no difference between the degree of field loss in patients with APONs and in those without. We suggest that APONs may signal an abnormal susceptibility of the optic nerve to the damaging effects of IOP.

Brain stem anesthesia after retrobulbar block.

Presented is a series of eight patients in whom neurologic sequelae developed after retrobulbar anesthesia. All patients demonstrated blockade of one or more cranial nerves and six progressed to apnea, requiring intubation and mechanical ventilation. Neurologic findings included amaurosis in the contralateral eye (5 patients), nonreactive pupil in the contralateral eye (6 patients), ductional defects (2 patients), and dysphagia (4 patients). In all cases, these findings resolved in 2 to 12 hours. In patients who progressed to apnea, spontaneous respiration resumed within 30 to 60 minutes. These findings are particularly significant in light of recent decisions to reduce anesthesia coverage for cataract surgery in some regions.

Bilateral retinal vascular hypoplasia associated with persistence of the primary vitreous: a new clinical entity?

Three infant boys with a bilateral congenital retinopathy characterized by retinal vascular hypoplasia and persistence of the primary vitreous are described. The infants were healthy and had no non-ocular physical abnormalities. None had histories of prematurity or low birth weight. Family histories were negative to similar retinopathy, and the fundi of all examined blood relatives were normal. The described congenital retinopathy is compared and contrasted with persistent hyperplastic primary vitreous, Norrie's disease, idiopathic retinal dysplasia, familial exudative vitreoretinopathy, retinopathy of prematurity, and incontinentia pigmenti.
