Atypical hyperlipidemia and nephropathy associated with apolipoprotein E homozygosity.

Hyperlipidemia has been implicated in the pathogenesis of experimental progressive glomerulosclerosis, but its role in human renal injury is controversial. This report describes a 12-yr-old boy presenting with massive proteinuria, hepatomegaly, anemia, severe mixed hyperlipidemia, and progressive renal failure. The initial renal biopsy disclosed large numbers of foam cells that were shown to be monocytes. Evidence is presented suggesting that apoprotein-E2 homozygosity in our patient, together with an 88% reduction in plasma lipoprotein lipase activity associated with severe nephrotic syndrome, is responsible for the atypical clinical features, lipoprotein phenotype III with chylomicronemia, and renal lipidosis. A regimen of dietary lipid restriction, gemfibrozil, and niacin resulted in significant but partial improvement of the dyslipidemia and resolution of the hepatomegaly and ascites. This report stresses the importance of characterizing unique lipid disorders in patients with nephrotic syndrome in order to prescribe effective lipid-lowering strategies. Moreover, the striking resemblance of the clinical and nephrohistologic features of this patient to those occurring in experimental models of coexisting glomerular injury and hyperlipidemia led to the speculation that, in this setting, the hyperlipidemia may contribute to the development of progressive glomerulosclerosis.

Morphologic and immunophenotypic characterization of a cell line derived from liver tissue with Epstein-Barr virus associated post-transplant lymphoproliferative disease.

A B-cell line was established from the liver of an 11-yr-old boy with Epstein-Barr virus (EBV)-associated post-transplant lymphoproliferative disease (PTLD). The cells were morphologically heterogenous, CD10 (CALLA) negative, and expressed several B-cell antigens, including CD23, in a manner reminiscent of lymphoblastoid cell lines (LCLs) reported in the literature. However, the cells also showed expression of the CD77 antigen, carried a 14q32+ chromosomal anomaly, and showed IgM-kappa immunoglobulin isotype restriction immediately after their outgrowth in culture. These latter properties are typically associated with Burkitt's lymphoma cell lines rather than LCLs. Aberrant expression of the L60 antigen on these B-cells was found as additional evidence of altered growth regulation in these cells. EBV infection was demonstrated by the abundant expression of EBNA-2 and LMP viral antigens in culture. The cell line described should be useful in planning in vitro experiments designed to understand the factors that modulate the growth of PTLD in vivo.

Blockade by theophylline of capsaicin-induced motor effects in guinea-pig airways.

The effect of theophylline was tested on the capsaicin-induced increase in insufflation pressure in anaesthetized guinea-pigs and contraction of isolated tracheal spirals. Theophylline (6.25-25 mg/kg i.v.) inhibited in a marked, prolonged and dose-related manner the increase in insufflation pressure elicited by i.v. injection of capsaicin (2-10 micrograms/kg). Theophylline, even at plasma concentrations as low as 15-23 micrograms/ml (which are within or slightly higher than the therapeutic range in humans) produced an inhibition of about 30-50% of capsaicin-induced bronchospasm. Theophylline was significantly less effective in antagonizing the bronchospasm elicited by maximally effective dose (100 micrograms/kg i.v.) of acetylcholine as well as by a dose equieffective to capsaicin 10 micrograms/kg (i.e. acetylcholine 50 micrograms/kg). Theophylline (30 microM-1 mM) also inhibited potently and concentration dependently the capsaicin-induced contraction in vitro and, again, it was significantly less effective against cholinomimetic-induced responses. These data suggest that theophylline can interfere, at therapeutic doses, with some process(es) selectively involved in capsaicin-induced neuropeptide-mediated bronchoconstriction. Since capsaicin-sensitive sensory fibers are thought to be involved in the pathogenesis of bronchial hyperreactivity and asthma, these inhibitory actions of theophylline might be relevant for its therapeutic effects.

On the causal association between exposure to leather dust and nasal cancer: further evidence from a case-control study.

A case-control study was performed on the incident cases of nasal cavity tumours which occurred between 1968 and 1982 among the residents of Vigevano (Lombardy region, northern Italy). This area is characterised by a high prevalence of shoemakers (especially in leather); the activity has predominated in Vigevano since the beginning of this century. Twenty one cases were identified (16 men and five women); 20 were histologically confirmed as nasal epithelial tumours; 17 had already died at the time of interview and the occupational history was obtained from the next of kin. Two controls per case were selected from the general population and matched by vital status, age, sex, and residence. The overall odds ratio for the subjects exposed to leather dust was 47.1 for men and 3.5 for women. The odds ratio was higher for adenocarcinoma and among the workers exposed to the worst working conditions. A significant trend for the level of exposure to leather dust was found. Nevertheless, even the jobs characterised by a relatively low exposure were found to have a significantly higher risk (OR = 7.5). Smoking habits and exposure to solvents are unlikely to confound the relation between exposure to leather and nasal tumours.

The role of ultrasonography in the diagnosis of tumours of the renal pelvis.

The authors have evaluated 22 patients with renal pelvis tumour (RPT) by means of excretory urography and ultrasonography. Six patients also underwent arteriography. The value of the three investigations in the diagnosis of the lesion, the nature, the pyelic origin and the extension are presented. It is concluded that urography remains the method of choice for the evaluation of RPT while ultrasonography may be considered complementary to urography in cases of non-functioning kidney, when the urographic filling device defect is uncertain and in the differentiation between the pyelic or parenchymal origin of the tumour.

Ultrasonography in the diagnosis of hydronephrosis in patients with normal renal function.

In order to define the sensitivity and specificity of ultrasonography (US) in the diagnosis of hydronephrosis, 125 patients with normal renal function were examined after urography using high-resolution real-time scanning. The overall diagnostic accuracy of US in detecting hydronephrosis was 85.2%, with a specificity of 84.4% and a sensitivity of 89.9%. It is concluded that US may be considered the screening test of choice for the diagnosis of hydronephrosis. However, urography is still required for the evaluation of renal function, site, and nature of the obstruction when US shows a dilated collecting system and in the patient with a normal US but renal colic.

[Percutaneous pyelostomy: technic and indications].

The authors evaluated the results of 24 percutaneous nephrostomies. Pelvic neoplasms involving ureters were the most frequent indications to the procedure (71% of the cases). In all patients the placement of a catheter was possible. The authors describe the techniques performed and evaluate their advantages and limits. The problems related to the use of nephrostomy in patients with widespread neoplasms and severely ill are discussed. The most frequent drawback was catheter dislodgement (25% of the cases). No serious complications occur. Percutaneous nephrostomy is a temporary or permanent form of urinary diversion in obstructive nephropathies, alternative to surgical nephrostomy.

Iopamidol in urography. A comparison between ionic and nonionic contrast media in patients with normal and impaired renal function.

The radiographic results obtained in urography with iopamidol, a new low osmolality contrast medium, given to 50 patients with normal renal function and 42 patients with impaired renal function, have been evaluated and compared with those obtained with ionic media of current use (meglumine sodium diatrizoate and sodium iothalamate). Based on this comparison, the authors believe there are no significant radiographic advantages to iopamidol.
