The cleaved cell content of non-cleaved (transformed) follicular center cell lymphomas and its clinico-pathological significance.

The relationship of cleaved cell content and survival was studied in 122 patients with transformed small and large non-cleaved cell types of the Lukes and Collins classification for non-Hodgkin's lymphomas. The large non-cleaved follicular center cell (FCC) type showed higher contents of cleaved cells than did the others; the geometric mean value differed significantly from that of small non-cleaved FCC Burkitt type (p less than 0.0001) or non-Burkitt type (p less than 0.01) lymphomas. There was a tendency toward a better prognosis with large fractions of cleaved cells but no statistical difference was found. The prognostic characteristics of the neoplastic clone are better revealed by the predominant cell.

Detection of herpes simplex virus in women with acute pelvic inflammatory disease.

Four women are described with acute salpingitis confirmed by laparoscopy who had herpes simplex virus (HSV) isolated from the cervix or the upper genital tract (endometrium, fallopian tube, or cul-de-sac) or both. None of the patients had overt genital herpes, but one had typical HSV cervitis on a cervicovaginal smear stained with Papanicolaou's stain, one had a significant change in level of antibodies to HSV, and one had an endometrial biopsy specimen positive for HSV antigen. There are at least three potential explanations for these findings: chronic viral shedding, viral reactivation caused by acute pelvic inflammatory disease (PID), or that the PID was actually caused by HSV. Further prospective studies are needed to document the role of HSV in causing PID.

Chlamydial endometritis.

Endometrial biopsies were obtained from 32 women with suspected pelvic inflammatory disease, of whom 23 (72%) had histopathological evidence of endometritis. Chlamydia trachomatis was isolated from the endometria of nine (39%) women (chlamydia group) but not from the other 14 (non-chlamydia group). Severe plasma cell endometritis and lymphoid follicles with transformed lymphocytes were significantly more common in the chlamydia group than in the non-chlamydia group. This suggests that C trachomatis is an invasive endometrial pathogen which often causes severe inflammation. The association was independent of predisposing factors such as use of intrauterine contraceptive devices.

Endometritis and acute salpingitis associated with Chlamydia trachomatis and herpes simplex virus type two.

A patient who had endometritis confirmed by endometrial biopsy and acute salpingitis confirmed by laparoscopy is reported. Chlamydia trachomatis and herpes simplex virus type two were isolated from the endometrial cavity and the fallopian tube. The histopathologic findings of the endometritis were similar to those frequently seen in chronic chlamydial eye disease or chlamydial cervicitis.

Prognostic factors for non-cleaved follicular center-cell lymphomas and immunoblastic sarcoma. A Bayesian approach.

The Bayesian multivariate statistical method was applied to determine the relative strength and optimal combination of 17 variables in predicting survival of 151 patients with non-Hodgkin's lymphomas assigned as non-cleaved follicular center-cell and immunoblastic sarcoma types according to the classification of Lukes & Collins. Considering all the factors simultaneously, the analysis showed that the combination of stage, Hb level and location of the lymphoma was included in the best predictive model at each survival time studied. Additional factors were erythrocyte sedimentation rate, thrombocyte count and leucocyte count. Of the histological variables, only growth pattern and mitotic ratio in the biopsy specimen remained significant. At manually controlled computer simulation with these best indicators, this model would have given a correct classification for 69-78% of the patients at the 4 survival times studied. One can thus expect about 70% correct prognoses using this model.

Intravenous leiomyomatosis.

Intravenous leiomyomatosis of the uterus with extension to the right common iliac vein in a 34-year-old woman is reported. The successful treatment consisted of abdominal hysterectomy, bilateral salpingo-oophorectomy, venotomy and extraction of 3 cm long mobile tumour from the right common iliac vein. The presence of cytoplasmic oestrogen and progesterone receptors in the tumours indicates that intravenous leiomyoma is hormone-dependent.

The Lukes and Collins classification of non-Hodgkin's lymphomas. I. A histological reappraisal of 301 cases.

Cytomorphological categories of the Lukes and Collins classification could be recognized in 92% out of a retrospective series of 301 non-Hodgkin's lymphomas. The follicular center cell (FCC) group constituted the majority (54%). Certain differences between the cytological subgroups were observed as regards age and sex and the growth pattern. Follicular features in the growth pattern were found in 16% of the cases. The large cleaved FCC type behaved differently from the other FCC types in many respects. The occurrence of cleaved FCC lymphomas appeared to be exceptional in childhood. There were no small lymphocytic and plasmacytoid lymphocytic lymphomas in children or young adults, and large non-cleaved FCC lymphomas and immunoblastic sarcomas were rare in these. In the small non-cleaved FCC group both the Burkitt type and the non-Burkitt type exhibited a bimodal age distribution with a peak under 10 years of age and another at age 60-70 years. Five convoluted lymphocyte lymphomas, between 2 and 29 years of age, were found in this series. The heterogeneous group of unclassified lymphomas included as a special case one lymphoma with deeply multilobated nuclei.

The Lukes and Collins classification of non-Hodgkin's lymphomas. 2. A survival study of 301 patients.

In a retrospective study of 301 non-Hodgkin's lymphoma patients the clinical and prognostic value of the classification of Lukes and Collins was studied. A slight majority of the cases were of the follicular center cell types. Stages III-IV were found in 46% of the entire group at the time of diagnosis. The lymphomas showing follicular center cell types were frequently localized (63%). About 65% of all cases survived one year and about 45% five years. The females showed a better survival. The nodal lymphoma cases had the poorest survival. Two prognostically different subgroups of non-Hodgkin's lymphomas were discerned. The more favourable group consisted of the small cleaved FCC type (five-year relative survival 74%), the small lymphocytic type (62%) and the plasmacytoid lymphocytic type (51%). The small non-cleaved FCC type, the large non-cleaved FCC type and the immunoblastic sarcoma type formed the other group with a less favourable prognosis. Mortality due to these types manifested itself to a level of 65% in the first two years.

Independent risk factors of benign and malignant breast lesions.

The purpose of this study is to evaluate the risk factors of benign breast disease and to compare them with the known risk factors of breast cancer in order to make inferences on the relationship between benign breast disease and breast cancer. All benign breast lesions diagnosed from the population of the city of Tampere, Finland in 1974-1977 were reclassified by two pathologists into two groups of dysplasias and two groups of tumors. In the risk analysis of 422 age-matched pairs no group of benign lesions had risk factors consistently similar to those of breast cancer. It is therefore likely that either benign breast disease is not associate with breast cancer or it is an independent risk factor, not associated with the other high risk indicators of breast cancer.

Flow cytometric analysis of DNA ploidy in large cleaved cell lymphomas.

The results of nuclear DNA content analysis of a series of 28 patients with large cleaved follicular centre cell non-Hodgkin's lymphomas (NHL) are reported. DNA aneuploidy was found in 11 (39 per cent) cases. The DNA indices of the DNA-aneuploid peaks ranged from 1.14 to 2.28. Seven (25 per cent) cases were tetraploid. DNA ploidy was not associated with prognosis. The percentage of S phase cells (SPF) ranged from 2.0 to 30.5 per cent (median 5.1 per cent). Lymphoma patients with SPF higher than 9.7 per cent had a worse survival rate than patients with lymphoma with less than 9.7 per cent S phase cells but the difference did not reach statistical significance. The results of the DNA ploidy and SPF were comparable to those of intermediate and high grade malignancy NHLs.

Accumulation of p53 protein correlates with tumour proliferative activity in EBV positive Burkitt's lymphoma.

p53 tumour suppressor gene is often found mutated in Burkitt's lymphoma (BL) cell lines and tumours. We analysed 35 BL tumours for the accumulation of p53 protein, and correlated the results with DNA flow cytometric data on the proliferative activity (SPF), and data on the presence or absence of Epstein-Barr virus (EBV) DNA. More than one-third (37 per cent) of the tumours showed accumulation of p53, which was considered to be consistent with mutation of the p53 gene. Tumours that were positive both for EBV DNA and p53 had significantly higher mean SPF than corresponding EBV DNA negative and/or p53 negative tumours. The proportions of tumour cells with accumulation of p53 appeared to correlate with tumour SPF only in EBV DNA positive BLs. However, there was no apparent association between accumulation of p53 and the presence or absence of EBV DNA. These findings are suggestive of multiple pathways in BL tumour progression.

Prevalence of nonspecific vaginitis and other cervicovaginal infections during the third trimester of pregnancy.

Nonspecific vaginitis (NSV) is a clinical syndrome characterized by a decreased concentration of normal acidophilic Döderlein flora in the vagina and increased concentration of nonacidophilic microorganisms, such as Gardnerella vaginalis, anaerobic bacteria, and mycoplasmas. Since several NSV-associated organisms are associated with complications of pregnancy, we studied the prevalence of NSV among 104 consecutive women during the third trimester of pregnancy. Overall, 14 women (14%) had NSV by clinical and/or biochemical criteria, two women had Chlamydia trachomatis isolated from the cervix, and 24 women (23%) had yeast isolated from the vagina. None had Neisseria gonorrhoeae or Trichomonas vaginalis isolated. There was a correlation between an abnormal succinate to lactate ratio (on gas-liquid chromatography) and cytologic atypia detected by cervicovaginal Papanicolaou smears. More extensive studies are needed to correlate NSV with prematurity.

Estrogen and progesterone receptors in breast cancer: relationships to tumour histopathology and survival of patients.

Estrogen receptor (ER) and progesterone receptor (PR) concentrations were measured in the tumours of 399 cases of primary breast carcinoma. Histological type and histological grading was also analysed. The correlation between survival and histological grading was observed and found to be of high significance statistically. Longer survival of patients with ER- and/or PR-positive tumours was also observed, but the ER and PR prognostic value did not reach the same magnitude as histological alone. The prognostic accuracy in breast cancer, when histological grading, ER and PR were used together, failed to reach statistically significant values. A lower proportion of ER- and PR-positive tumours were found in histological grade III. The majority of the tumours belonging to specific histological variants of carcinoma were ER- and/or PR-positive. Relationships between ER, PR, menopausal status, and age were also noted. It was apparent that the prognostic value of PR concentrations in the tumour was more relevant than that of ER alone.
