Correlation between serum IgE and specific IgE antibody titer to house dust mite in children with asthma.

Using the enzyme-linked immunosorbent assay (ELISA) for IgE specific to mites, we evaluated the relationship between total serum IgE levels and IgE antibodies specific to mite in 58 asthmatic children. Our results showed that there was a positive correlation between total serum IgE and IgE antibody specific to Dermatophagoides pteronyssinus (r = 0.57; p less than 0.001) and Dermatophagoides farinae (r = 0.59; p less than 0.001). There was also a significant correlation between D. pteronyssinus-specific IgE and D. farinae-specific IgE (r = 0.68; p less than 0.001). This suggests that there are common allergens between the two species. The close correlation between the ELISA assay and skin test suggests that the former will be useful for the diagnosis of mite allergy in asthma.

Detection of house dust mite allergens and immunoblot analysis in asthmatic children.

The mite allergen extracts (Dermatophagoides pteronyssinus and D. farinae), were fractionated by polyacrylamide gel electrophoresis and autoradiography. The molecular weight of the most apparent proteins of D. pteronyssinus was between 27,000 and 35,000, and the protein components of D. farinae were in the 9-100-kilodalton range. Specific IgE antibodies directed against mite allergens were utilized as probes to detect the allergens. Using immunoblot analysis and radiostained technique, D. pteronyssinus-sensitive patient sera displayed the major protein of allergens with molecular weights of 15,800 and 27,000-31,000, while D. farinae-sensitive patients' sera displayed the major protein of allergens with molecular weights of 12,000, 15,800 and 26,000-29,000. The electroblotting technique is fast, convenient, and highly suitable for both allergen composition studies and screening of antibody specificities.

Systemic lupus erythematosus in children.

A retrospective study was made of 137 children with systemic lupus erythematosus (SLE) seen at the Veterans General Hospital in the past 12 years. There were 116 females (84.7%), and 21 males (15.3%). The peak ages of onset were 13-14 years and 16-17 years. Malar rash, arthritis and fever were the most common presentations, as in previous studies. Central nervous system (CNS) involvement occurred in 24 cases (17.5%), and renal disorder was noted in 77 cases (56.2%). Renal biopsy in 67 cases showed a pathological transformation rate of 16.1% (5/31). Ten patients died during the study period, with renal insufficiency the primary cause of death.

[The significance of mite antigens in Kawasaki disease].

House dust mites have been suspected as a causative agent in Kawasaki disease. To assess the role of mite in children with Kawasaki disease, we studied 20 cases of Kawasaki disease from September 1985 through May 1986. Their ages ranged from 2 months to 4 years with a mean of 2.1 years, we assessed intradermal skin test reactivity to mite antigens (Dermatophagoides pteronyssinus and Dermatophagoides farinae) and anti-mites specific IgE (RAST) levels in 20 Kawasaki disease patients, 10 children for control, and 30 asthmatic patients for comparison. Intradermal skin test reactivity to mite antigens was not significantly increased in the 20 Kawasaki disease patients than in pediatric controls (p greater than 0.05), there was a more significantly reactivity in asthmatic patients than in Kawasaki disease and pediatric control (p less than 0.001). There was a correlation between serum IgE and anti-D. pteronyssinus specific IgE (r = 0.65, p less than 0.001) or anti-D. farinae specific IgE (r = 0.73, p less than 0.001) in Kawasaki disease. The anti-mite specific IgE's expressed as RAST score or mean values were not more significantly raised in Kawasaki disease than in pediatric controls (p greater than 0.05), there was a more significantly raised in asthmatic patients than in Kawasaki disease patients and in pediatric control. Coronary artery involvement was not significantly related to anti-mite specific IgE's in Kawasaki disease (p greater than 0.05). According to this study, the role of house dust mites is undetermined to Kawasaki disease.

Serum IgE, skin and radioallergosorbent tests for house dust and mites in asthmatic children.

Thirty asthmatic children were given an intradermal skin test, total IgE measurements and the radioallergosorbent test (RAST) against house dust and house dust mites (Dermatophagoides pteronyssinus and Dermatophagoides farinae). Results of the intradermal skin test and RAST were highly positive with overall agreement in 95.6%. The correlation coefficient between the total IgE and the RAST to house dust was 0.35 (p less than 0.05). There was no significant difference between the total IgE and the RAST to D. pteronyssinus and D. farinae (p greater than 0.05). The highly significant correlation coefficient between house dust and house dust mites (D. farinae, p less than 0.0001; D. pteronyssinus, p less than 0.00001) was determined by RAST. House dust has been found to be the most important allergen in children in Taiwan. The role of mites as a possible allergen in house dust also showed a highly significant correlation in our study.

Studies on hyperimmunoglobulinemia E syndrome in five children.

T cell subpopulations, lymphoproliferative response, autologous mixed lymphocyte reaction (AMLR), antibody-dependent cellular cytotoxicity (ADCC), natural killer (NK) activity and chemotaxis were studied in five hyperimmunoglobulinemia E (hyper IgE) syndrome patients. Both absolute OKT8 (suppressor T) cells and OKIa1 cells were decreased in number. The OKT4/OKT8 ratio increased. The lymphoproliferative response showed that the patients' lymphocytes cultured in medium containing autologous plasma proliferated much less vigorously, as expressed by transformation index, than those cultured in AB healthy control plasma. The peak proliferative response in AMLR in patients was significantly lower than that in the control group. The proliferative response of the fresh autologous mononuclear cells (MNC) exerted suppressor activity in the patients, while those from the normal controls exerted helper activity. Supernatant from AMLR and an isolated T cell culture of patients exerted suppressor activity, while those from the normal controls had none. In the ADCC reaction, patient's polymorphonuclears were significantly impaired in target cell lysis. The NK activity and Leu-11 cells in the patients also showed a significant decrease. In vitro neutrophil chemotaxis was consistently impaired in two out of five patients. Therefore, there is evidence of abnormal regulatory T cell functions and defect in PMN-ADCC activity and NK lysis assay in patients with hyper IgE syndrome.

Seroimmunity to poliomyelitis in hospital personnel.

Neutralizing antibody titers to poliovirus type 1, 2 and 3 were examined in paired serum specimens obtained from 43 female hospital personnel and laboratory workers at Veterans General Hospital. Before vaccination, 23.2% of those studied did not have neutralizing antibodies to one or more types of poliovirus, and 4.6%, 13.9% and 7.0% lacked serum neutralizing antibodies to poliovirus type 1, 2 and 3, respectively. In a comparison of the serological responses evoked by oral poliovirus vaccine in the same persons, a striking difference in antibodies was shown in the geometric mean titers and no lack of antibodies was found after oral poliovaccine immunization. Therefore immunization with oral poliovirus vaccine should be considered in the future as to improved protection for such high risk people.

Campylobacter jejuni enteritis in children.

Campylobacter jejuni has been recently recognized as a frequent cause of diarrheal disease in infants and children. To assess its importance as an enteric pathogen in this area, in our pediatric laboratory, campylobacter jejuni was isolated by selective culture from 35 out of 623 (5.6%) patients with a history of acute diarrhea between March 1981 to December 1981. The peak incidence was in the summer (from May to August), age ranged from 10 days to 8 years, with the high incidence in the very young children. The sex ratio of male to female was four : one. In general, Campylobacter enteritis is not a severe disease and not associated with dehydration. The most common signs were fever & frequent diarrhea. Most of the children recovered spontaneously on conservative management. The antibiograms for 30 strains showed that the Aminoglycosides, Chloramphenicol, Ampicillin, Erythromycin were the most effective drugs. Resistance to erythromycin was found in 13.4% of our series. This study shows campylobacter is the common cause of bacterial diarrhea in Taiwan.

Comparison of conventional phototherapy and fiberoptic phototherapy in the very-low-body-weight infants.

Phototherapy is effectively employed for the treatment of neonatal hyperbilirubinemia, but it may influence the physiological hemodynamics of the infants, such as skin blood flow, insensible water loss and the redistribution of cardiac output. This is a retrospective chart review study involved totally 42 very low birth weight prematures(birth body weight less than 1500 gm, VLBW) who received conventional or fiberoptic phototherapy. Infants with congenital malformation, severe cardiorespiratory instability and the requirement of diuretics, inotropics or blood transfusion were excluded. The body weight loss, daily intake/output and bilirubin level during the initial four days of phototherapy were analyzed. The results showed the efficacy of both methods was the same, while the fiberoptic therapy group had significantly more urine output(p < 0.05), the body weight loss was not statistically significant. It is possible that the fiberoptic therapy group had less insensible water loss and less attenuation of the decrease of renal blood flow, this resulted in more urine output.
