[Gastrointestinal bleeding caused by small bowel leiomyosarcoma].

The small bowel leiomyosarcoma is a infrequent tumour in the clinical practice. We report the case of a 50 year old male admitted to the hospital on two occasions for gastrointestinal bleeding. On the second occasion a push enteroscopy located in jejunum a tumour. The pathologic diagnosis was a low grade jejunal leiomyosarcoma. The patient required a surgical intervention with resection of the affected portion of the jejunum.

[Evaluation and improvement in the information of the patients who carry out a digestive endoscopy].

INTRODUCTION: The information is an important part of the attendance process though in occasions it can be inadequate or deficient. AIM: We detect an opportunity of improvement in the inadequate information of the patients who carry out an endoscopia. The aim is to evaluate and to improve the quality of the information. METHOD: We realize the first evaluation with 100 patients proceeding from consultations, detecting ignorance of the technology. Actions of improvement are applied, for 4 months, to realize the second evaluation, with the same number of patients and to evaluate improvement with regard to the situation of item. RESULTS: A deficit of information has been detected in the moment to indicate the endoscopia (little information contributed by the doctor prescriptor and in an important number of cases informed assent was not contributed) and after realizing the test, not contributing recommendations. We prioritize our action to this level with measures of improvement and after a new evaluation, a significant improvement decides in these situations. CONCLUSIONS: The studies of qualit level allow us to detect deficit situations and to improve situations in the services of health.

[Lactose malabsorption in patients with inflammatory bowel disease without activity: would it be necessary to exclude lactose products in the diet of all patients?].

INTRODUCTION: There are inflammatory bowel disease (IBD) patients avoid lacteal products without evidence of lactose malabsorption, probably because of incorrect patient perceptions and arbitrary advice from physicians and diet books. AIM: To evaluate the prevalence of lactose malabsorption in patients with IBP. METHOD: In 18 patients with Crohńs disease and 24 with ulcerative colitis and 25 controls the prevalence of lactose intolerance, as measured by lactose breath hydrogen tests. RESULTS: Observed deficiencies of absorption of the lactose in 7 (16.6%) patients with IBP and 5 (20%) subjects control. CONCLUSIONS: Not all the patients with IBD are intolerant to the lactose by the suppression of the lacteal produts must not be generalized in the diet of these patients.

[Amebic colitis in an immunocompetent host].

Amebic colitis is a low prevalence illness in developed countries. Its diagnosis is infrequent and is usually made in certain groups of risk. Images studies are not useful in differential diagnosis with other illnesses as Inflammatory Bowel Disease (IBS). Next clinical note shows the problems in the diagnosis of amebic colitis in an immunocompetent host without risk factors.

[Vascular complications associated with inflammatory bowel disease].

Thromboembolic episode is a well known extraintestinal manifestation of inflammatory bowel disease, but it is a clinical rare complication. Histological and hematological studies suggest that a hypercoagulable state is involved in the pathogenesis of inflammatory bowel disease. However, the exact mechanism of hypercoagulability is still unknown. During the acute recurrences there is an increase of factor VIII, fibrinogen, platelet, factor V and decrease of antithrombin III. Hematologic disorders seem markedly correlated with the activity of the disease. We report on two patients with Inflammatory bowel disease and hypercoagulable state. We review the literature and discuss about the pathogenic mechanisms of such complication.

[Complication associated with celiac disease].

Enteropathy associated T cell lymphoma is the most serious complication of celiac sprue. The incidence of malignancy, in general, is approximately twofold greater in celiac disease than in the general population, but the risk of specific gastrointestinal malignancies is markedly increased. Lymphoma of the small intestine, comprises approximately 50% of the malignancies, and the most are of T-cell origin. We report a case of celiac disease associated T-cell Lymphoma of the jejunum in a woman. At the age of 33 the diagnosis of Celiac sprue was made, after institution of a gluten free diet the patient improved, but, fifteen year later, the patient begin with fever and abdominal pain. Laparotomy showed a perforation of the intestine by a tumour.
