Mesigyna once-a-month combined injectable contraceptive: experience in Latin America.

A phase III clinical study was carried out among 534 fertile Latin American women to evaluate cycle control, side effects, and contraceptive efficacy of a once-a-month combined injectable, Mesigyna, consisting of 50 mg norethisterone enanthate and 5 mg estradiol valerate. The pregnancy rate at 1 year was 0 per 100 woman-years for a total experience of 4688 woman-months. The overall discontinuation rate at one year was 17.9%. Discontinuation rate for bleeding problems was 5.1%. The Colombian women had a significant increase (p <0.001) in bleeding problems compared to other countries. The discontinuation rate for amenorrhea was 1.1%. There were no significant differences between the groups regarding discontinuation for other medical or non-medical reasons. Mean weight gain after one year of use was 1.02 kg. Mesigyna is an appropiate once-a-month injectable contraceptive for Latin American women since it is highly effective and its perception of normal menstrual bleeding is of importance in the Latin American population.

A simple method for estimation of agonist activity at receptor subtypes: comparison of native and cloned M3 muscarinic receptors in guinea pig ileum and transfected cells.

We describe a simple method for calculating the pharmacological activity of an agonist (A) relative to a standard agonist (S) using only the concentration-response curves of the two agonists. In most situations, we show that the product of the ratios of maximal responses (Emax - A/Emax - S) and potencies (EC50 - S/EC50 - A) is equivalent to the product of the affinity and intrinsic efficacy of A expressed relative to that of S. We refer to this term as the IRA value of A. In a cooperative system where the concentration-response curve of the standard agonist is steep and that of the test agonist is flatter with a lower maximal response, the simple calculation of IRA described above underestimates agonist activity; however, we also describe a means of correcting the IRA in this situation. We have validated our analysis with modeling techniques and have shown experimentally that the IRA values of muscarinic agonists for stimulating contractions in the guinea pig ileum (M3 response) are in excellent agreement with those measured in the phosphoinositide assay on Chinese hamster ovary cells expressing the M3 muscarinic receptor.

Sexually transmitted diseases and related genital pathologies in oligozoospermia.

Sexually transmitted pathogens are associated with a wide range of anomalies and diseases, including recurrent genital herpes, urethritis, syphilis, prostitis, genital ulcers, perihepatitis, unexplained infertility, infant prematurity, low birth weight, and neonatal death or malformations. Prompt diagnosis of sexually transmitted diseases (STDs) is essential to ensure appropriate specific treatment and to reduce complications. Extensive studies on the etiology of male infertility include poorly treated infections such as STDs, epididymitis, complications associated with mumps, delayed treatment of undescended testes, repair of inguinal hernia, varicocele, endocrine disorders, anomalies of the reproductive tract and male accessory organs, trauma, surgery, heat-cold injury, and active-passive immunization.
