The bitemporal (bicoronal) flap is a well recognised technique for exposing the skull and upper face. Prospectively collected data over a 5-year period in which this technique was used for access in primary craniomaxillofacial trauma was analysed. Emphasis was placed on indications, complications and outcome. A total of 50 cases were audited. 84% were male. 74% had sustained midface or frontal bone trauma, 24% had lateral upper face injuries. Only 1 patient had a penetrating missile injury. Consistent complications of the procedure were altered sensation at the vertex in the early postoperative period and a small area of wound breakdown over the suture knot in the galea. In all cases the altered sensation greatly improved or resolved, 12% developed a wound infection, however, 83% of these were trivial requiring no active treatment. There was an isolated case of pressure necrosis over a drain site. 12% had a demonstrable transient deficit of the temporal branch of the facial nerve, all of which improved.
50 patients who underwent major surgery for head and neck cancer and reconstructed with radial forearm microvascular free flaps, were assessed with regard to social adaptation, appearance and in particular mouth function. The results were compared with a similar group of patients who underwent reconstruction with either primary repair or pedicled flaps.

We have developed a cell-free system in which the bacterial transposon Tn7 inserts at high frequency into its preferred target site in the Escherichia coli chromosome, attTn7; Tn7 transposition in vitro requires ATP and Tn7-encoded proteins. Tn7 transposes via a cut and paste mechanism in which the element is excised from the donor DNA by staggered double-strand breaks and then inserted into attTn7 by the joining of 3' transposon ends to 5' target ends. Neither recombination intermediates nor products are observed in the absence of any protein component or DNA substrate. Thus, we suggest that Tn7 transposition occurs in a nucleoprotein complex containing several proteins and the substrate DNAs and that recognition of attTn7 within this complex provokes strand cleavages at the Tn7 ends.
Malignant tumors of salivary glands in children are extremely rare. A case of an adenoid cystic carcinoma affecting the palate in a 9-year-old child is described. This tumor has not previously been reported as affecting the minor salivary glands among patients in this age group.

Two cases are reported in which the treatment of pan-facial fractures via a bi-temporal flap approach was complicated by cessation of activity of the sinuatrial node (sinus arrest). The anatomical, physiological and anaesthetic implications of this potentially fatal complication are discussed.
A review of the literature suggests that the oculocardiac reflex is part of a wider phenomenon involving any structure supplied by the trigeminal nerve. This can lead to potentially fatal complications in maxillofacial surgery. A review of the literature is discussed and two further cases are reported.

Sialolithiasis is the occurrence of calcified masses within salivary ducts or glands, formed by deposition of calcium salts around a central nidus of organic material. A case is reported which affected an accessory salivary gland of the upper lip.
A modification is suggested to the standard cast metal cap splint which allows accurate and easier localisation in the operative and post-operative phases.

An interaction between a low-dose oral contraceptive and a long-acting, narrow-spectrum penicillin preparation is reported, and a mechanism for the interaction is proposed. Various drugs that can affect the efficacy of the contraceptive pill and their modes of interaction are listed.
