Vascular lesions of the lumbar epidural space: magnetic resonance imaging features of epidural cavernous hemangioma and epidural hematoma.

The authors report the magnetic resonance imaging diagnostic features in two cases with respectively lumbar epidural hematoma and cavernous hemangioma of the lumbar epidural space. Enhanced MRI T1-weighted scans show a hyperintense signal rim surrounding the vascular lesion. Non-enhanced T2-weighted scnas showed hyperintense signal.

[Mechanical study of stiffness in LIM-41 external fixator assemblages submitted to torsion strain].

The mechanical stiffness of LIM-41 external fixator assemlages submitted to torsional strain was studied. A condition of unstable fracture was reproduced. The authors evaluated the effect of of Schanz pin configurations, defined as "distal", "standard" and "proximal", and of the distance, 20 mm, 40 mm or 60 mm between the external fixator and the element employed to simulate bone. The authors conclude that stiffer LIM-41 assemblages are obtained if the "standard" or "proximal" configurations are employed, on which some pins are placed near the fracture simulation site, and if the external fixator body is fitted closer to the bone simulating element, having the former variable greater influence over the stiffness coefficient.

Atlanto-axial dislocation in children.

The authors present 14 cases of C1 C2 dislocations in children (8 boys and 6 girls; mean age, 6.9 years) seen at the Orthopedics and Traumatology Institute of the University Hospital, Faculty of Medicine, University of São Paulo from 1978 to 1990. In view of the disappointing results obtained by conservative treatment in 6 of the 14 patients, the authors recommend early atlanto-axial arthrodesis in cases of C1 C2 dislocation in children.

Herniation of the lumbar disk in adolescents.

The authors review eight cases of lumbar intervertebral disk herniation in adolescents aged less than 16 years, with a minimum follow-up treatment of 2 years during an 18-year period. All patients presented lower back and sciatic pain, antalgic scoliosis, and a positive Lasègue sign. Nerve compression signs were present in 25+ of the cases. Seventy-five percent of the cases underwent myelography and 25% underwent computerized axial tomography. Results were good in all cases.

[Magnetic resonance of spinal osteoid osteoma].

The authors report one case of spinal osteoid osteoma localized in the thoracic spine that was imaged through M. R. I. They report the M. R. I. aspects of the tumor. Precise localization is necessary for the correct surgical approach.

[Back pain and lumbo-sciatic pain: psychosomatic approach].

The authors call attention to the importance of an integrated multi-professional attendance of patients with low back pain. It is pointed out that social economic and psychologic factors contribute to manifestations of many lumbar spine disorders and that the knowledge of psychosomatic concepts will help to a better understanding of the symptoms presented by these patients.

[Traumatic disk hernia of the cervical spine].

The authors present 26 cases of traumatic disc herniation in the cervical spine submitted to anterior discectomy. All of the patient presented neurologic deficit, being 18 classified as Frankel A, three as Frankel B, 2 Frankel C and 3 Frankel D. There were observed three deaths in the immediate post-operative period due to respiratory insufficiency in patients classified as Frankel A. The other 23 cases have a follow-up of 14 months (6-84 months), presenting improvement of the neurologic deficit in six patients classified as Frankel B, C and D and only one as Frankel A. The authors emphasize the importance of the image methods in the diagnosis of this lesion.

[Surgical treatment of idiopathic scoliosis in adolescents by the Harrington method].

Forty six patients with idiopathic scoliosis were treated according the Harrington method. The surgical treatment was done with the Harrington distraction rod, and vertebral arthrodesis with autogenous iliac bone graft. The patients wore a Risser-Cotrel body-cast for nine months but started walking two days after casting. Good clinical results were obtained in all the patients. Final curve correction was 40%. In this series of patients no pseudoarthrosis occurred.

[Complications of herniated disk surgery].

The author studies the complications that can occur in the lumbar spine operations particularly in the disc protrusion surgeries. The pre, intra and post-operative complications as well as the early and late complications were analysed. The factors that may lead to bad surgical results are discussed.

[Traumatic spondylolysis].

The authors present the results of treatment of seven cases of spondylolysis with a two years follow up. Six patients were treated with the thoracolumbosacral orthosis and one with L5-S1 fusion. At the end of the follow up all patients were pain free.
