Sinusoidal and postsinusoidal vascular systems in liver cirrhosis.

The morphology of the sinusoidal and postsinusoidal hepatic vein system in liver cirrhosis is shown by postmortem venography, microangiography and micropreparation. The normal angioarchitecture of the sinusoidal and hepatic vein system in lost in cirrhotic alteration of the liver. The hepatic veins are partly stenosed and displaced. Regenerate nodules, pseudolobules and extremely vascularized connective tissue septa, characterize the venous parenchymogram and microangiogram. The size of the regenerate nodules corresponds well to a granulomatous contrast of the parenchyma. The disordered intrahepatic angioarchitecture has clinical significance for hemodynamics and hepatic venous pressure measurement.

[Dacarbazine (DTIC) therapy and the Budd-Chiari syndrome].

A case of a 58 years old female is presented, who died because of a Budd-Chiari-syndrome. This complication had developed during treatment with Dacarbazine (DTIC), which was given after surgical removal of a malignant melanoma. The causative relation between the Budd-Chiari-syndrome and this cytostatic drug is discussed, even though the influence of other drugs cannot be excluded.

[Collodion baby and harlequin fetus. Comparison of 2 cases].

Two cases of severe congenital ichthyosis are presented: "collodion baby" and "harlequin fetus". The collodion baby is no clinical entity, but a symptom occurring in several forms of ichthyosis. In our patient the collodion skin was a symptom of ichthyosis congaenita (lamellar ichthyosis). Treatment with orally administered steroids was life-saving. The harlequin fetus represents a distinct entity characterized by a lethal genotype. There is a molecular defect of keratinization.

New chromosomal dysmorphic syndromes. 3. Partial trisomy 3q.

Chromosome analysis in a newborn, the daughter of diabetic parents, who showed multiple dysmorphic signs and malformations revealed direct duplication of a long arm segment of chromosome 3(3q2100 leads to 3q2700). Both parents have normal karyotypes. Compilation of the phenotype stigmata with those of 7 other patients and 1 fetus with partial trisomy 3q confirmed that clinical recognition of this syndrome is possible. It is characterized by hypertrichosis, typical craniofacial dysmorphia, frequent organ malformations and skeletal anomalies, as well as a peculiar dermatoglyphic pattern. It is a severe genetic disturbance, leading to death in the first months of life in many cases and only symptomatic care is advised.

Pathological and immunological observations in a case of Lesch-Nyhan-syndrome.

A patient with clearly developed features of the full Lesch-Nyhan syndrome and complete lack of activity of hypoxynthine-phosphoribosyltransferase is described. The clinical picture was characterized by absence of spasticity, good control of autoaggression by behavior therapy, and no signs of renal insufficiency. After death, which was caused by a viral infection, pathological examination and a search for material immunologically cross-reacting with hypoxanthine-phosphoribosyltransferase were possible. In spite of increased serum urate levels and raised urinary uric acid excretion there were no signs of urate deposits or damage in the internal organs, including the kidneys. Crossreactive material was found in the liver, kidneys and spleen, a relatively rare finding in the full Lesch-Nyhan-syndrome. The absence of any specific pathological changes in the brain of this patient is in agreement with earlier reports.

[A comparative study between postmortem venography and the pathological anatomy of the liver (author's transl)].

A barium sulphate-gelatine mixture was injected into the veins in the left lobe of 50 human livers which were then x-rayed. The radiographs were compared with the pathological and anatomical findings and enlargements were studied and compared with the histology. A correlation was found between the venogram and the macroscopic and microscopic changes, particularly in various forms of cirrhosis of the liver. The scope and limitations of hepatic venography were studied with respect to solitary and diffuse liver metastase.

[Complications following transfemoral embolization of renal carcinoma. Survey and experience (author's transl)].

A comprehensive survey of the complications following 151 transfemoral embolizations of renal carcinoma is presented, including 9 of our own patients. The main side effects seen after this procedure are (in decreasing frequency): elevation of body temperature, transient elevation of arterial blood pressure, impairment of renal function, infection of the necrotic tumor, thromboembolic incidents, and accidental embolization of other organs. Total lethality is about 3.3%. Possible contraindications and precautionary measures are derived from the discussion of the pathophysiologic reactions in the course of the complication: avoidance of therapeutic embolization immediately after diagnostic angiography, strict asepsis, addition of antibiotics to the embolization mixture, prophylactic administration of heparin, and the use of flowdirected balloon catheters.

[Intrapulmonary thymoma].

A primary intrapulmonary left-sided thymoma is described in a 23-year old man. Eight similar observations can be found in the world literature. The histology is quite uniform. The tissue shows characteristic epithelial and lymphatic cellular elements but organoid differentiation into medullar and cortex is lacking. Hassall's granules are only rudimentary. Giant cells in all stages of development, tissue mast cells, varied supporting tissue, epithelial border layer and connective tissue capsule are all present. There is no blastomatous degeneration. Theoretical indications to explain the sudden onset of tendency to proliferation of the ectopic thymus independant of age and sex are discussed.

Sarcomas originating in the small bowel diverticula.

Sarcomas of the small bowel are rare and particularly unusual to originate within the Meckel's or jejunal diverticula. We report two cases of malignant tumors involving neural or muscular fibers in the wall of small bowel diverticula, and briefly review the pertinent literature.

Carcinoma of the breast behind a prosthesis--comparison of ultrasound, mammography and MRI (case report).

Silicone prostheses are known to impair the diagnostic evaluation of the breast. The authors report about a case of a small carcinoma of the breast, which was examined by MRI, ultrasound and mammography. The diagnostic capabilities of these imaging modalities are discussed and MRI is presented as a new possible tool for an improved evaluation of breasts with silicone implants.

MR imaging of the breast using gadolinium-DTPA.

In a preliminary study 20 patients underwent breast examinations by magnetic resonance (MR) imaging without and with Gd-DTPA as contrast medium. All carcinomas enhanced, whereas dysplastic tissue enhanced slightly or not at all. Significant additional diagnostic information was available on the Gd-DTPA examinations in at least four of 20 cases compared with MR without contrast medium and X-ray mammography. Our preliminary results indicate that MR imaging of breast using Gd-DTPA may be helpful for the evaluation of dense breasts and the differentiation of dysplasia and scar tissue from carcinoma.

MR imaging of the breast: comparison with mammography and ultrasound.

Fifty-seven biopsy proven breast masses have been examined at our institution by magnetic resonance (MR), mammography, and ultrasound. The three techniques have been evaluated concerning their capability to visualize the lesion and the diagnostic information obtained. Magnetic resonance proved comparable to mammography and superior to sonography in the fatty to medium dysplastic breasts but inferior to the combined examination by mammography and sonography in the dense breasts. In some selected cases, however, special advantages of MR have been found.
