Subnasal modified Le Fort I for nasolabial aesthetics improvement.

We report our experience with a modified Le Fort I osteotomy developed to avoid nasal tip upturning, alar base widening, and upper lip flattening in anterosuperior repositioning of the maxilla. We compare the aesthetic results obtained with this variation of the surgical technique to those obtained using the more traditional Le Fort I osteotomy combined with the alar cinch suture and the anterior nasal spine reduction procedures on a sample of 20 patients.

[Leiomyoma of the masseter muscle. Report of a case].

The authors present a clinical case of leiomyoma, localized in the anterior margin of the masticator muscle. The clinical diagnosis was formulated following the histological test on the operating part. It is a case a rare pathology of the oral cavity that isn't commonly suspected. The symptoms of the patient let think a pathology of ATM and it was not typical of benign or malign neoplasm of the soft tissue of oral cavity. Only the use of a special clinical test, permitted the possibility to diagnose the exceptional case of masseter muscle on the left. The surgical therapy, being the neoplasm case benign and well capsuled resulted to be treatable and permitted the definitive resolution of the case.

[Trauma to the anterior cranial base. Report of a case].

The authors present a case of foreign body retention, following car accident, localized on superior medial corner of the right orbit. The traumatic impact, involving endo-orbital structures, owing to the peculiar direction of the foreign body, caused a fracture in the anterior cranial base and laceration of the dura madre. As the anatomical structures involved did not concern only the MaxilloFacial district, the choice of the protocol to be applied to the patient was taken in accordance with the neurosurgical équipe. An intra cranial surgical approach has allowed to remove, by retrograding the foreign body on its trajectory path, in order not to injure further the already damaged structures involved. Then the tissues have been reconstructed; in particular, the anterior cranial base was restored through the use of pericranial strip. This technique is the elective surgical treatment in lesions of such small entity, as it is able to grant a solid support to the overhanging cerebral parenchyma. Besides, this reconstruction technique provides a complete protecting isolation of the ocular globe from compressive and pulsative phenomena, due to the neurocranial adjacent structures. With this technique, after a 8 month's follow-up. The patient has obtained optimum functional and aesthetic results.

Firearm injuries in maxillofacial region reconstructive surgery.

Firearms induce severe morphological and structural alterations on both soft and bony tissues of the face. It is therefore essential to restore their previous functionality. In our experience, maxillofacial lesions due to firearm shooting must be divided, from a locational point of view, into those lesions involving the upper third, those involving the medium third, and those involving the lower third of the face. Lesions of soft and bony tissues must be evaluated precisely through instrumental diagnostic examinations and axial and coronal computed tomographic projection, preferably with a three-dimensional construction, to be able to restore the previous functional integrity of the maxillofacial region. At a subsequent surgical time, it may be necessary to plan aesthetic corrections for recovery of the previous facial harmony.

Aesthetic and functional reconstruction in intraosseus hemangiomas of the zygoma.

The incidence of intraosseus hemangiomas is very low. The mandible, the zygoma, the maxilla, and the frontal bone are the most frequent areas of localization in the craniomaxillofacial region. Surgery, without preoperative embolization, is always the best treatment for intraosseus hemangiomas of the zygoma. The radical removal of the tumor frequently causes a contour defect that has to be corrected. Calvarial grafts provide a good solution to the problem of reconstruction of bone loss. They are easy to prepare, near to the implant zone, and they do not require changes in the patient's position during the operation. The implants fixed by micro- and minifixation systems provide a good functional and aesthetic result. We, after a review of the literature on intraosseus hemangiomas of the craniomaxillofacial region, report two cases of intraosseus hemangiomas of the zygoma in which removal of the tumor and reconstruction with calvarial grafts have been performed.

[Surgical and local treatment in a case of fungal sinusitis in a patient with bone marrow aplasia].

We report a clinical case of severe medullary aplasia complicated by fungal antritis. The treatment adopted for this patient consists in a clean operation of the infective focus and the local instillation of Amfotericina B during the post operative period. In this way the systemic circulation is not interested by the use of Amfotericina B, which is extremely important to avoid the inevitable onset of several unwanted side-effects; besides, we avoid the progression of the infective focus and its systemic diffusion.

[Diagnostic and surgical problems in a case of post-traumatic retention of a foreign body in the submandibular gland].

We report a particular case of a traumatically introduced foreign body in the sub-mandibular gland. A breaking object (plastic pen) penetrated, through the anterior oral floor, the submandibular gland. Such foreign bodies should be removed before suturing the wounds, as they can disturb healing and cause problems long after the accident. An adequate history, clinical and imaging studies should reveal the possibility of foreign body penetration and its localization, when it isn't previously removed. We compare the use of ultrasound detection of foreign bodies in soft tissue and conventional plain radiography, computed tomography, and magnetic resonance imaging. The usefulness of US in this kind of research does not justify TC or RM studies and allows a less invasive surgical operation.

[The maxillofacial functional and esthetic surgical aspects in a case of osteopetrosis].

Osteopetrosis is an extremely rare condition characterized by an increase in osseous cell density. It shows three different hereditary patterns. Diagnosis is made by means of a 3D CT and an accurate study of hepatic and renal functions, as well as audiometric and labyrinth examination. In the frequent case of serious osseous alterations of the facial profile, surgical treatment is essential in order to obtain the best aesthetic and functional results.

[Dental and skeletal malformations: 434 cases of II and III dental skeletal class. Our experience and review of the literature].

BACKGROUND: This study tests the patients who underwent to orthognatic surgery to compare the perioperatory treatment and the results with what related by international bibliography. METHODS: We have made a retrospective analysis on 434 patients affected by dentalskeletal malformations and surgically treated at the Department of Maxillo-Facial Surgery of the University of Rome "La Sapienza", from 1-1-1988 to 31-12-1997. We have examined these data: clinical evaluation of patient, pre and post-surgical orthodontic treatment, minimum age for surgical correction, surgical technique, use of osseus-synthesis with wire or RIF for mobile segment osseus stabilization, use of reference articular plaque and the possible association of additional surgical technique during the same time of maxillomandibular correction. We have made in all patients orthognatic surgery, which consist in osteotomy Le Fort I for upper jaw and sagittal osteotomy for mandible. RESULTS: In 25 patients was necessary remove the plaques and the screws which have made an infection. In 15 patients we have executed a subsequent operation for relapse of the dentalskeletal malformation. In other patients we have got a good functional and aesthetical result. CONCLUSIONS: From our study result that an exact presurgical analysis and a correct orthodontic-surgical treatment have permitted to obtain a good functional and aesthetical result in all patients.

Surgical treatment of fibrous dysplasia of the cranio-maxillo-facial area. Review of the literature and personal experience form 1984 to 1999.

OBJECTIVE: The aim of this study is to report personal experience on the evolution of the therapeutical approach and surgical treatment in 66 patients with fibrous dysplasia (FD) of the cranio-maxillo-facial area. METHODS: From January 1984 to December 1999, 66 cases of FD of the cranio-maxillo-facial area were diagnosed. 21 patients were still growing (under 17 years), while 45 were adults (over 17 years). 50 (75.7%) patients showed FD in monostotic form; 15 (22.7%) patients showed FD in polyostotic form; one (1.51%) patient affected by Mc Cune-Albright Syndrome presented the involvement of the left hemimandible. In 24 (36.3%) patients the clinical onset of the disease was insidious, lacked evident symptomatology and the diagnosis was fortuitous. The remaining 42 (63.6%) patients showed different signs and symptoms according to the location of the pathology. RESULTS: In this time the patients who underwent surgical treatment showed the resolution of the functional and aesthetical problems; the patients who did not undergo surgical treatment, in relation to age and type of pathology, showed an arrest during clinical and radiographic checks. CONCLUSIONS: We have pointed out the importance of a careful differential diagnosis of fibrous dysplasia with other pathologies with clinical and radiographic conditions similar to it and the importance of a correct surgical planning, carried out according to the patient's age, location, mono or polyostotic form of fibrous dysplasia and to the presence or absence of functional limitations, and aesthetic problems. Further-more we have pointed out the great possibilities of reconstruction of the surgical defects deriving from the utilization of autologous bone grafts and the benefits deriving from the introduction of the internal rigid fixation for the stabilization of these grafts in the surgical defects.
