Echocardiographic and surgical correlations in bacterial endocarditis.

The results of preoperative echocardiography were compared with the pathologic findings at the time of surgery in 24 patients undergoing valve surgery for endocarditis. Of the 32 valves involved by vegetations, 27 (84%) were identified preoperatively. Valve destruction was correctly predicted in 16 of 18 cases. Myocardial abscess formation was detected in only one of the five patients in whom it occurred. Overall, the echocardiograms satisfactorily predicted the pathologic anatomy in 20 cases. In the remaining four patients, the echocardiographic description was seriously incomplete or misleading. Thus, surgery can be recommended on the basis of the clinical and echocardiograhic findings for patients with endocarditis. Cardiac catheterization is reserved for patients in whom significant coronary artery disease or intracardiac shunts are suspected or in whom a satisfactory echocardiogram cannot be obtained.

[Biological availability of Ossin, a sodium-fluoride dragee for osteoporosis therapy].

The bioavailability of Ossin, a drug for the treatment of osteoporosis, containing 40 mg of NaF, was investigated. Because of the enteric coating of the tablet, intestinal absorption occurs when the tablet leaves the stomach. The sustained release of the F--in the intestine results in a slow F-- -absorption. Maximal F-- -serum concentrations are reached more than 4 hours after absorption has begun. In contrast to non-sustained release NaF tablets the slow absorption of Ossin leads to lower, and less variable F-- -serum concentrations. Following administration of 1 tablet Ossii b.i.d., the steady state concentrations were between 130 and 220 ng/ml. The absorption was at least 86%. As calculated by the renal F-- -elimination, the therapeutically important F-- -retention in bone amounted to more than 50%. The enteric coating of the Ossin tablets, the low and relatively steady F-- -serum concentrations and the high F-- -retention in bone, are expected to contribute to an improvement in tolerance and therapeutic efficacy in the treatment of osteoporosis.

Right atrial myxoma: unusual clinical presentation and atypical glandular histology.

A 57-year-old black female presented with a 1-month of right-sided congestive heart failure and clinical evidence of pulmonic and tricuspid valvular stenosis and insufficiency. The echocardiographic examination and ventriculography demonstrated a large right atrial tumor interfering with the function of both right-sided valves. The patient underwent successful surgical resection of the tumor. Histologically, the tumor had cellular areas typical of myxoma, as well as glandular areas, a feature which has been described very rarely in this lesion. Electron microscopy of the glandular zones, which has never been reported previously, shown cells having essential homology with the usual myxoma elements. The atypical histopathology of this lesion supports the theory that atrial myxomas are true neoplasms, and are not derived from unusually organized mural thrombi.

[Various problems in the development of the child with clefts from the viewpoint of rehabilitative education].

Extensive studies and experience of many years evidence that optimal rehabilitation of the cleft patient may be achieved above all by early complex measures on the part of the sciences involved. To evaluate the efficiency of logopaedic early treatment in the cleft patient, the development of cleft children was studied during the first scholastic years, special attention being paid to their learning how to manage the written language (reading and spelling). A comparative study was undertaken in cleft children and children with normal speech at the beginning and during the third year of school education, assessing their discriminating performance in speech-motor, phonemic, visual, musical and rhythmical fields (modified Breuer-Weuffen test) and their performance in reading and spelling. The results obtained permitted to draw conclusions as to the early education of cleft patients.

[Interaction between naproxen and tolbutamide on metabolism in diabetics].

In a double blind study 16 maturity onset diabetics were treated with d-2-(6-methoxy-2-naphthyl)-propionic acid (naproxen) and tolbutamide with a view to possible interactions between both drugs. There were no significant differences between blood glucose levels after placebo and naproxen, respectively. The concentrations of tolbutamide were not significantly different, either. Thus diabetics on tolbutamide can be treated additionally with naproxen without any clinical side effect on carbohydrate metabolism.

Amount and properties of uptake forms in preparations of alpha-mannosidase from pig kidney.

alpha-Mannosidase (alpha-D-mannoside mannohydrolase, EC 3.2.1.24) from pig kidney has been shown to exist in multiple forms differing in their capability to be endocytosed by alpha-mannosidase deficient cultured cells. A method is presented to evaluate the amount of "uptake" forms in different preparations of the enzyme. Preparations with different rates of uptake were shown to contain different amount of "uptake" forms and "non-uptake" forms. The content of "uptake" forms in a preparation was identical with that of enzyme molecules bearing a phosphorylated carbohydrate group necessary for the recognition by cell surface receptors.

Alkaline Phosphatase

Computed tomography diagnosis of renal angiomyolipoma.

Angiomyolipomas with a high fat content can be diagnosed by CT, thus eliminating the need for angiography. Surgery can then be tailored for maximal renal tissue preservation. One unilateral and two bilateral surgically confirmed cases of renal angiomyolipoma are presented with a typical fourth case in which surgery was cancelled based on the pathognomonic CT findings.

Titanium implants as a temporary replacement of mandible. A report of 30 cases.

In a 5-year period, 30 titanium implants (Bowerman and Conroy 1969) were utilised for the reconstruction of mandibular defects following tumour operations. Of these, 20 implants were removed prematurely. Reasons for early removal were: 1. postoperative dehiscence of wound (5), 2. perforation through the skin or mucosa (5), 3. infection of graft bed (7), 4. breakage of the implant (1), 5. tumour recurrence (2). A total of 9 implants were retained for one or more years. The results are compared with those of Bowerman (1974). Reasons for the early loss as well as the clinical implications are discussed.

Comparative flatulence activity of beans and bean fractions for man and the rat.

To evaluate H2 production in the rat after ingestion of legume products as a predictive bioassay of their flatulence activity for man, responses of man and the rat to (1) a bland formula diet, (2) cooked California Small White bean (CSW) solids, (3) 70% ethanol extract of (2), (4) oligosaccharide fraction of (3), (5) non-protein nitrogen fraction of (3), and (6) combination of (4) and (5) were compared. Six human subjects were fed an 800 kcal breakfast of (1) or containing the amount of a test item equivalent to 100 g CSW. Each subject received all items once plus a single replicate of three items at the rate of one per week in random order. Total flatus volume, H2, CO2, and CH4 volumes and breath H2 concentration were measured for the interval 2.5 to 8.5 hours after breakfast. Young rats in metabolic chambers received known amounts of items 2 to 6 mixed with a bland basal diet. H2 trapped in the chambers was measured after 20 hours. Correlation of rat H2 and to human breath H2 measurements and flatus gas volumes were significant and positive except for negative correlation with flatus methane volume. Measurement of hydrogen production in the rat has potential for a predictive bioassay for flatulence in man.

Noninvasive determination of glomerular filtration rate using x-ray fluorescence.

Accurate glomerular filtration rates (GFR) can be calculated based on the infusion of small amounts of nonradioactive iothalamate and collection of plasma samples for assay by x-ray fluorescence. This innovation permits frequent clearance determinations in patients without the hazard of repeated radiation exposure and without the necessity of catheterization of the bladder for urine samples. Thus, the technique becomes feasible in children, transplant patients, and other needing accurate and frequent GFR determinations.

[Fluorometric determination of glibornuride in plasma and serum using 7-chloro-4-nitrobenzo-2-oxa-1,3-diazole].

Glibornuride, tolbutamide and gilbenclamide react on heating with 7-chloro-4-nitrobenzo-2-oxa-1.3-diazole (NBD-chloride) in amylacetate to form a highly fluorescent NBD-amine. A fluorometric determination of glibornuride is presented. Reaction conditions and the extraction of glibornuride from plasma and serum are described. The method is sensitive to measure drug levels after therapeutic doses. The detection limit is ca. 40 ng/ml and the recovery rate is ca. 100%. The coefficients of variation for the determination of drug levels of 1 to 0.1 mug/ml range from 4.1% to 12.1%.
