This article reports on the incidence of success following single-visit endodontic therapy. Non-surgical endodontic therapy was performed on 210 teeth in 167 patients who were rapidly deployed during the initial stages of Operation Desert Shield. Upon their return, 97 patients were recalled for follow-up evaluation. This study evaluated 102 teeth. The recall rate was 58% with a success rate of 89%.
A case report of a maxillary first molar is presented in which a drainage tube from a decompression procedure was removed 2 days following placement. The oral surgeon who removed the tube scheduled the patient for surgical enucleation of the large periapical lesion. The surgical appointment was subsequently cancelled and routine endodontic follow-up showed continued osseous repair at 3, 6, and 12 months. The decompression of a large periapical lesion is a conservative alternative to surgical enucleation.
Adjuncts to posterior endodontic surgery include fiberoptics, high-torque handpieces, and optical magnification. These current technologies allow the clinician to expand and amplify his surgical techniques. The patient can now have the benefit of treatment options not available previously.

Two case reports are presented to illustrate the effectiveness of cold in localizing the offending tooth in difficult diagnostic situations where painful pulpitis is present. A review of the literature reveals that there are several proposed theories concerning the mechanisms by which cold relieves the painful symptoms present in some cases of irreversible pulpitis. The hydrodynamic theory seems to provide the most feasible explanation of the events that occur.

A case of cuspid gigantism is reported. A technique used to diagnose and complete the root canal therapy is discussed. Modifications of conventional techniques and materials are shown to be effective in managing this case. Routine use of the bitewing film on long cuspids is not recommended. It is suggested only in extreme cases of unusual anatomic features that lend themselves specifically to this technique. It should be kept in mind that such circumstances are uncommon.
