Investigations on the effect of metamizol on ureteral motility.

The aim of this comparative study was to obtain quantitative information on the effect that metamizol has on ureteral motility. Renal Tc-99m-MDP excretion and ureteral urine transport against gravity were examined in five patients, once without (control) and, several days later, with metamizol treatment. In each of these ten scanning sessions, renograms of both kidneys were recorded. They were corrected for background activity and compared with respect to the following parameters: the curve amplitude at 30 min p.i. normalized to the respective maximum amplitude; the 6-30 min and 40 s-6 min integrals of a corrected curve. Under metamizol medication, the flow of radioactivity to the bladder was thus found to be significantly reduced (p less than 0.001). The comparison of the Tc-99m-MDP retention curves obtained with and without metamizol treatment did not reveal a statistically significant drug effect on renal Tc-99m-MDP excretion.

Tests for the absorption of 75Se-labelled homocholic acid conjugated with taurine (75Se-HCAT).

The absorption of selenomethionine Se 75-labelled homocholic acid conjugated with taurine (75-SE-HCAT) was tested in 46 patients. Retention measurements using (1) an uncollimated gamma camera and (2) a measuring arrangement similar to a human-body counter were compared in order to obtain a quantitative assessment of the absorption capacity of the terminal ileum for bile acids. The retention curve obtained after the oral administration of the 75Se-labelled bile-acid analogue showed a monoexponential decline; in the case of unimpaired absorption, the half-life was greater than 2.5 days. When more than 30 cm of the ileum had been eliminated by inflammatory infection or resection, the measured half-life was below 0.5 days due to malabsorption. We also performed a quantitative determination of the hepatic secretion of 75-Se-HCAT into the gall bladder. If more than 80% of the activity administered is found in the gall bladder, disturbed absorption of bile acids in the terminal ileum can be excluded. Values smaller than 80%, however, do not provide proof of disturbed absorption.

Gray platelet syndrome: selective alpha-granule deficiency and thrombocytopenia due to increased platelet turnover.

Clinical and laboratory studies of two siblings, both suffering from gray platelet syndrome (GPS) are described. The patients had a mild bleeding disorder, their platelets were blue-gray in panoptic stains, and alpha-granules were markedly reduced, as shown by electron microscopy. The platelet content of platelet factor 4 and that of beta-thromboglobulin were significantly reduced (3%-7% of normal). Platelet count was decreased (33-150 X 10(9)/1) and small platelets were increased in platelet volume distribution. Bleeding time was prolonged on most occasions. Bone marrow aspiration was performed in one patient and revealed increased reticulin fibers, however, megakaryocyte count was normal. The mean platelet survival was 4.8 days using 111indium-labelled platelets. In this patient, platelet-associated IgG was within the normal range. Prednisone therapy failed to increase platelet count. Dental surgery was performed under cover of desmopressin and no bleeding complication occurred; however, no improvement of bleeding time was observed. The patient delivered a healthy male infant without hemorrhaging while under concurrent platelet transfusion therapy.

[Localization diagnosis and differential diagnosis of postoperative intraperitoneal abscesses using 99m technetium-stannum-colloid-marked granulocytes].

Occult intraperitoneal abscesses are difficult to locate and dispose of. Therefore, a method of examination is needed that is specific, can be used at any time, is not expensive, and yields early results within 2-3 h. This seems possible with a new methods of examination. 99mTc-Sn colloid was used to label autologous polymorphonuclear leukocytes which, when reinjected, emigrate to the site of the suspected abscess. Standard scintigraphy localizes the labeled inflammatory cells. In 32 surgical patients, 34 scans were performed with good results (controlled by greyscale ultrasound); 17 (53.1%) scans represented intraperitoneal abscesses; 3 (9.4%) scans demonstrated true positive extra-abdominal inflammations.

Cell labelling with colloidal substances in whole blood.

A method for the labeling of leucocytes with 99mTc-colloid is described. The labelling can be done in samples of whole blood, because the colloid is only taken up by the phagocytosing cells, the monocytes and the granulocytes. The part of the colloid that is not phagocytosed is brought to a soluble state with Na-citrate, so that only the phagocytosed colloid is reinjected. The labelling efficiency with this method is between 80% and 90%. Measurements of the activity in the leucocytes 3 h after reinjection, have shown that at least 50% of the labelled cells are at this time still available in the blood pool. The clinical results on 32 patients with the tentative diagnosis of an abdominal abscess and on 42 patients with the tentative diagnosis of septic loosening of an endoprosthesis have shown that the labelled leucocytes are very well suited to show up local foci of inflammation.

The application of radioactive labeled leucocytes for the proof of inflammations.

The methods we have tested are reliable for the labeling of granulocytes and monocytes as well as with colloidal substances and with soluble 99mTc-phytate. Supravital colorations and electronmicroscopic pictures do not show any damage of the cells caused by the labeling procedures. In the clinical application after the reinjection of the labeled granulocytes and monocytes, inflammations in the abdomen and in the bone could be proved and localized exactly. A final evaluation of the described methods and its differentiation from other diagnostic possibilities will only be possible after further investigations.

Tumoral calcinosis: accumulation of bone-seeking tracers in the calcium deposits.

Rapid and preferential uptake of 47 calcium 87m Strontium by the multinodular calcareous masses was seen in a patient with tumoral calcinosis. Using 87m Strontium, scintigraphic imaging of the tumors was achieved, while the skeleton was barely visible. More intense accumulation of 87m Sr. in one tumor suggested a higher rate of calcium depostion. This was in agreement with the observation that this tumor showed less tendency to decrease in size during calcium and phosphorus deprivation therapy. Quantitative measurements of the uptake of bone seeking tracers by the tumors may be of value in estimating calcium turnover in the calcified masses.

[Peripheral conversion of T4 and T3 in man].

Our investigations of the peripheral metabolism of T4 and T3 in man demonstrate clearly that T3 is secreted by the thyroid gland and is peripherally formed by conversion from T4. The calculation of the disposal rates indicate furthermore that 40% of the daily metabolised T4 is converted to T3. In this way more than 60% of the daily production of T3 results from peripheral conversion of T4. Our results support the hypothesis that the physiological action of the thyroid hormones is effected mainly by T3.
