[Histopathological changes at the surgical site in experimental animals following partial cricoid resection and thyrotracheal anastomosis].

OBJECTIVE: To study in an animal model the local healing process after partial cricoid resection with thyrotracheal anastomosis. MATERIAL AND METHODS: Partial cricoid resection with thyrotracheal anastomosis was performed in 17 New Zealand rabbits. Experimental animals were gradually sacrificed and the laryngotracheal complex was removed to proceed to a histopathological study. RESULTS: An acute inflammatory response followed by a chronic inflammatory process was found in the surgical site. Normal tissue architecture was established after an eight week period. Anteroposterior and transverse diameters at all levels of the upper respiratory tract were not significantly altered at any time along the healing process. Experimental animals remained free from symptoms during the follow-up period. CONCLUSIONS: These results support that partial cricoid resection with thyrotracheal anastomosis is a safe procedure for the treatment of severe laryngotracheal stenosis.

T3 glottic cancer: oncologic results and prognostic factors.

OBJECTIVES: To analyze oncologic results, prognostic factors, and consideration of transglottic tumors as a separate entity in 73 patients with T3 glottic carcinoma treated by surgery +/- radiation therapy at La Paz University Hospital from 1984 to 1994. METHODS: Sixty-four patients underwent a total laryngectomy; neck dissection was performed in 50 patients; 29 patients received postoperative radiotherapy. RESULTS: The 5-year overall survival, cause-specific survival, and relapse-free survival rates were 58.1%, 66.8%, and 63.7%. Univariate analysis revealed prognostic significance for age, dyspnea, initial tracheostomy, and pathologic lymph node status. No difference was found when comparing survival and relapse rates between transglottic and glottic tumors. CONCLUSION: Surgery provides acceptable rates of cancer control and survival for patients with T3 glottic carcinoma. Older patients, patients with positive neck nodes, and those presenting with dyspnea have worse prognosis. The term transglottic is merely descriptive and has little impact on prognosis.

[Results of surgery for hyperparathyroidism].

Since 1983, we have performed 110 parathyroidectomies in 109 patients. The hyperparathyroidism was diagnosed as primary in 102 (93.6%) patients, secondary in 6 (5.5%), and tertiary in 1 (0.9%). Sixty-one patients (55.9%) had single adenomas, 2 (1.8%) had double adenomas, 44 (40.3%) had hyperplasia and 1 (0.9%) had parathyroid cancer. Sixty-five adenomas were resected (59.6%). Subtotal parathyroidectomy was performed in 34 cases (31.2%) and total parathyroidectomy with autotransplant in 10 cases (9.2%). In two cases, primary surgery did not solve hypercalcemia; one underwent successful surgery several months later. Twenty-two patients (20.2%) developed transient hypocalcemia. Persistent hypoparathyroidism was detected in 8 patients (7.3%). After exposure of 220 nerves, 2 patients (0.9%) developed permanent unilateral recurrent paralysis. Our results were similar to those of other series.

Pulmonary carcinoma metastatic to the larynx.

Metastases to the larynx from distant primaries are rare. We report a case of a pulmonary epidermoid carcinoma metastatic to the larynx that simulated a primary cancer. A review of the literature showed nine previously reported cases of pulmonary metastases to the larynx--only two of them were epidermoid carcinomas. Treatment of a secondary lesion in the larynx is justified only when other organs have not been affected. The diagnostic and therapeutic considerations of this condition are discussed.

Malignant fibrous histiocytoma of the larynx.

Malignant fibrous histiocytomas of the upper respiratory tract are rare, aggressive mesenchymal neoplasms. We report a case of a glottic malignant fibrous histiocytoma of the larynx on a 54 year old man. Only a few have been reported in the English literature. The clinical behaviour and degree of malignancy of these tumours cannot be predicted. Wide, aggressive excision of the tumour with a margin of normal tissue appears to be the treatment of choice. About two years after total laryngectomy the patient is well and free of disease.

[Ketotifen in allergic rhinitis].

Double-blind experiment (at random) comparative of the efficacy of ketotifen and Clemastina, for the treatment of periodic allergic Rhinitis. The study embraced 40 patients (21 men and 19 women) between 13 and 57 years old. The program was conducted on a fifty-fifty shares between both drugs. The clinic efficacy of ketotifen has been proved either as regards to clinic improvement or the less good results attained with the use of clemastine. The AA. guess on the prophylactic influence of ketotifen in asthmatic bronchial crisis among patients suffering allergic rhinitis.

[Cervical recurrence in the pathologically negative neck (pNO): finding a needle in a haystack].

Recurrence in the pathologically negative neck (pN0) is one of the most striking events occurring in head and neck tumors. It is not even mentioned in many neck dissection studies. The presence of micrometastases has been suggested as an explanation for this event. In accordance to a recent study performed in our institution, we bring up this controversial issue. A definitive explanation for recurrence in the pN0 has not yet been proposed.
