CD4 and CD8 counts are widely used prognostic markers to assess the degree of immune impairment in HIV seropositive individuals and to monitor anti-retroviral therapy (ART). Pregnancy is considered as a physiologically immunocompromised state, hence alterations in T lymphocyte subsets may occur during pregnancy. There is a need to establish base-line values of these counts, especially in healthy pregnant women. One hundred healthy HIV seronegative pregnant women (mean age 22.5 +/- 2.99 yr) in their third trimester of pregnancy and 30 non-pregnant women (mean age 22.7 +/- 3.01 yr) were tested for their CD4 and CDS counts. In pregnant women, the CD4 and CD8 cell counts/microl were 764 +/- 249 and 547 +/- 196 and the CD4 and CD8 per cent were 56.49 +/- 8.3 and 38.03 +/- 7.2 respectively. In the non-pregnant women CD4 and CD8 counts/microl were 965 +/- 267 and 639 +/- 211 whereas the CD4 and CD8 per cent were 55.27 +/- 5.99 and 36.17 +/- 6.44 respectively. Absolute counts were significantly lower (P < 0.05) in the pregnant group as compared to the controls. A wide variation was seen in the CD4 and CD8 counts in both the groups. However, the variations in the mean CD4 and CD8 per cent were much smaller. Thus CD4 and CD8 per cent may be considered as a useful indicator of immune function rather than absolute counts, in pregnant women.
The mitotic checkpoint blocks the activation of the anaphase-promoting complex (APC) until all sister chromatids have achieved bipolar attachment to the spindle. A checkpoint complex containing BubR1 and Bub3 has been purified from mitotic human cells. Upon checkpoint activation, the BubR1-Bub3 complex interacts with Cdc20. In the absence of Mad2, BubR1 inhibits the activity of APC by blocking the binding of Cdc20 to APC. Surprisingly, the kinase activity of BubR1 is not required for the inhibition of APCCdc20. BubR1 also prevents the activation of APCCdc20 in Xenopus egg extracts, and restores the mitotic arrest in Cdc20-overexpressing cells treated with nocodazole. Because BubR1 also interacts with the mitotic motor CENP-E, the ability of BubR1 to inhibit APC may be regulated by kinetochore tension or occupancy.
In mitosis, the anaphase-promoting complex (APC) regulates the onset of sister-chromatid separation and exit from mitosis by mediating the ubiquitination and degradation of the securin protein and mitotic cyclins. With the use of a baculoviral expression system, we have reconstituted the ubiquitin ligase activity of human APC. In combination with Ubc4 or UbcH10, a heterodimeric complex of APC2 and APC11 is sufficient to catalyze the ubiquitination of human securin and cyclin B1. However, the minimal APC2/11 ubiquitin ligase module does not possess substrate specificity, because it also ubiquitinates the destruction box deletion mutants of securin and cyclin B1. Both APC11 and UbcH10 bind to the C-terminal cullin homology domain of APC2, whereas Ubc4 interacts with APC11 directly. Zn(2+)-binding and mutagenesis experiments indicate that APC11 binds Zn(2+) at a 1:3 M ratio. Unlike the two Zn(2+) ions of the canonical RING-finger motif, the third Zn(2+) ion of APC11 is not essential for its ligase activity. Surprisingly, with Ubc4 as the E2 enzyme, Zn(2+) ions alone are sufficient to catalyze the ubiquitination of cyclin B1. Therefore, the Zn(2+) ions of the RING finger family of ubiquitin ligases may be directly involved in catalysis.
Fifty cases of hepato-renal dysfunction of unknown etiology were studied over a two-year period. Urine samples were examined microscopically and cultured for Leptospira. Serum samples were examined for antibodies against Leptospira by the Macroscopic slide agglutination test (MSAT). Seventeen out of fifty patients (34%) showed evidence of Leptospiral infection by at least two diagnostic techniques used. 15/17 i.e. 88.2% were positive by dark ground microscopy, 7/17 were diagnosed by culture technique and 16/17 i.e. 94% were confirmed by serology. There was a good correlation between Microscopic agglutination test (MAT) and MSAT. Thus Leptospires seem to play a major role in the causation of hepato-renal dysfunction in and around Pune, Maharashtra.
Thirty one cases of epithelial borderline tumours of the ovary recorded over a period of six years were reviewed. The incidence of borderline tumours was 6% in relation to ovarian epithelial malignancies, with serous and mucinous types comprising three fourth of the lesions. The serous tumours were bilateral in 39%, revealed surface growth in 17% and had peritoneal implants in 11% of cases. The mucinous tumours were bilateral in 11% and had associated pseudomyxoma peritonei in 22% of cases. Nuclear grade appeared to correlate with extraovarian spread and surface growth in the serous borderline tumours, but not in the mucinous borderline tumours. The endometrioid borderline tumours and mixed epithelial borderline tumours were rare lesions. Twenty one patients (68%) presented in Stage-la. Surface growth correlated with recurrences. The prognosis remained good in serous borderline tumours even in the presence of implants as these were non-invasive. The mean disease free survival was 43.03 months. There was no statistical difference in disease free survival of patients with and without implants.

Changes in lipid peroxidation and superoxide dismutase level in rat brain due to single exposure to diethyl ether (anaesthetic) were studied in 100 and 300 day old rats. Enhancement of superoxide dismutase activity was more pronounced in young rat brain, while the reverse was the case with lipid peroxidation. The neurotoxic effects of diethyl ether may involve active oxygen species and superoxide dismutase activity increases as a defensive adaptation.
Fifty patients with burns ranging from 30 to 50 per cent of their body surface area were monitored for sepsis throughout their hospital stay using swab, blood and full thickness biopsy culture techniques. The relative merits of these techniques in the diagnosis of burn wound sepsis were evaluated. Only 62.5 per cent of the patients with a positive surface culture showed signs of clinical sepsis, while 87.5 per cent of the patients with significant bacterial count on biopsy culture showed signs of clinical sepsis. A decrease in bacterial count on follow up correlated with clinical improvement while a count of 10(8) orgs/gm indicated a bad prognosis. Wound surface cultures, though the simplest method gave poor indication of the organisms invading into the burn wound. Blood cultures were of only prognostic value. Full thickness biopsy culture and quantification of the number of bacteria in the burn wound was felt to be the best method for rapid diagnosis and for assessing the progress of burn wound infection.

BACKGROUND: Fine needle aspiration cytology is an established method for cytodiagnosis. Its application is particularly suited to the pediatric age group. A correct cytodiagnosis of especially large tumors can result in timely cytoreductive chemotherapy, thereby facilitating surgery. While the cytomorphology of Wilms' tumor is well documented, there is a paucity of literature concerning other uncommon pediatric renal tumors. CASE: A 2-year-old female underwent surgery for a renal mass following aspiration cytology. The histologic diagnosis was clear cell sarcoma of the kidney (CCSK). The cytologic smears were cellular and pleomorphic. The main feature of cytologic interest was the presence of deep nuclear indentations and grooves in many of the tumor cells. CONCLUSION: The cytologic features of CCSK are distinct and different from those of other renal tumors in children. Its recognition in cytology is important because its behavior is more aggressive than that of Wilms' tumor. A correct diagnosis can result in the institution of appropriate treatment.
Pulmonary epithelioid hemangioendothelioma (PEH) is an uncommon neoplasm. The reported case reveals unusual clinical manifestations pericardial effusion and raised CA-125 level.
Lymphoepithelioma like carcinoma is rare in locations other than nasopharynx. We report the second case of this tumour in trachea, in a young female patient, who was managed with concomitent surgery, radiotherapy and chemotherapy. The patient is disease free in the one-year follow up. The tumour presented difficulties during intubation for general anaesthesia and during surgery. Association with Epstein-Barr virus was not found in our case.
Primary tracheal tumours are rare and trachea is an exceedingly rare site of extramedullary plasmacytoma. We report a case of extramedullary plasmacytoma involving trachea and causing symptoms of airway obstruction in a 44-year-old man. Chest radiograph was normal. Flow-volume loop showed characteristic pattern localizing the lesion at thoracic inlet. Computerised tomography and bronchoscopy demonstrated a broad based pedunculated intratracheal mass obstructing 85% of the lumen. Tracheal tumour was excised via low tracheostomy. Histopathology of resected mass revealed extramedullary plasmacytoma. Relevant investigations excluded multiple myeloma. He was managed with radiotherapy with good response and there has been no recurrence after one-year follow-up.
A 44-year-old man presented with palpable right supraclavicular swelling having no symptoms pertaining to respiratory tract. A routine chest radiograph showed an anterior mediastinal mass. Computerised tomographic scan (CT-scan) of chest showed an enhancing and homogeneous mass in anterior mediastinal space with few tiny calcific specks within the mass. Biopsy of cervical swelling showed cavernous haemangioma with chronic non-specific lymphadenitis. A sternotomy was undertaken and an encapsulated 9x7.5x5 cm, dark purplish mass was seen in anteior mediastinum adherent to pericardium and right pleura. The cut-surface was elastic and soft. Histology confirmed cavernous haemangioma. Post-operative course was uneventful. Mediastinal cavernous haemangioma is extremely rare and surgical excision is the treatment of choice.
