Year by year the actual number of transplanted organs has been growing in Poland. In the event of the Województwo Zachodniopomorskie (Western Pomeranian Province) the primary activities were focused on informing medical personnel at hospital departments about approaching donors through trainings for such medical personnel. Such activities resulted in both acceptance and support, especially by medical personnel of medium as well as lower ranks. Since 1983 two transplant units have been functioning in the Province: SPSK-2 and S. P. W. S. Z. in Szczecin, and since 2000 two co-ordination offices, too. Main result of our activities can be seen in the number of organs procured for transplantation. In 2000 we had 27.7 procurements per million (ppm), in 2001 there were 39.9 ppm.
The article presents briefly historical development of death criteria from the modern times to the present. The criteria which are used for identification and diagnosing death on the base of respiratory and circulatory death definition are described. This work underlines the inadequacy of the definition of the brain death in relation to patients with persistent vegetative state and in relation to anencephalic newborns. The author describes the pathology and clinical and laboratory evidence of the brain stem death, which gave the possibility to justify the thesis that in case of the brain stem death ontological arguments are sufficient for diagnosing the death of a human being. The attention of the ethic of the life sanctity (on the base of halachic's law) and its opposing influence on the evolution of the medical definition of death has been paid. The recognition of the brain as the death of an individual is a cultural shock, which from scientific point of view changed the ways of thinking, almost immediately but did not in the awareness of the society. The work also underlies the fact that utilitarian argumentation can not be a criterion for making a decision concerning the life of an individual.

Urological complications of allogenic kidney transplantation include vesicoureteral reflux which can result in graft threatening urinary tract infection. To prevent this complication several ureterovesical anastomosis techniques have been developed. Authors present a comparison of three different techniques: extravesical without antireflux mechanism, extravesical Witzel-Lich with antireflux mechanism and intravesical Laedbetter-Politano with antireflux mechanism. 39 patients were selected randomly from a cohort of 420 allogenic kidney recipients (follow up time 10-147 months). All patients had voiding cystography and urine culture performed. The incidence of vesicoureteral reflux varied from 13.3% to 50%, depending on the anastomosis technique. No correlation between type of anastomosis and urinary tract infection was found.

Rabbits were repeatedly immunized with Staphylococcus aureus and a number of immunological parameters concerning humoral and cellular immunity of homologous and heterologous strains was studied. Immunological activation with reference to all the strains, was presented and it proved to be higher in the case of homologous strain. This finding proves the specific and non-specific stimulation of the immunological mechanisms by staphylococcal vaccine.
In Poland there is growing demand for biostatic allogeneic bone transplantation mostly for traumatologic operations and orthopedic reconstructions. The bone material is primarily harvested during postmortem examinations in forensic and pathology laboratories. Nevertheless, the collected amounts are not sufficient, so that material needs to be acquired from alternative sources, such as multiorgan donors. Between 1998 and 2003, 2331 potential donors were registered by the Transplantation Coordinating Center in Warsaw, which was adjusted to 1794 donors who would have been accepted as donors of the bone tissue. Unfortunately, due to denials from family members and public prosecutors, the sample was only 1416 donors, which would cover about 40% of the clinical orthopedic demand in Poland.
There is an increasing demand for bone allografts that are widely used in orthopedic reconstructive procedures. The bone tissue may be harvested from two sources: cadavers and multiorgan donors. Providing safe and valuable bone allografts is of paramount importance. Contamination of allografts during bone retrieval seems to be one of the most important problems since pathogenic microorganisms might be responsible for postoperative infections and complications in the healing process. The purpose of our study was to identify all factors contributing to bacteriological contamination of harvested bones. Therefore, we have considered factors such as harvesting environment, explantation techniques, storage and preparation of allografts, number of preceding procurements from the same donor, procurement duration, and time interval between death and tissue procurement. The microbiological evaluation of allografts has been performed by taking cultures from all collected bones. Our study revealed significantly greater contamination rates of bone allografts harvested from morgue than from multiorgan donors. According to this observation, we suggest that orthopedic surgeons should pay particular attention to obtain more bones of the highest quality, personally participating in multiorgan procurements.
INTRODUCTION: Infections are one of the most common complications after organ transplantation. Viral infections such as hepatitis type B (HBV) and C (HCV) or cytomegalovirus (CMV) infections are among the most serious ones. A high frequency of HBV and HCV infections has been recognized in kidney recipients. Viral infections play a special role in graft recipients because of clinical symptoms influencing graft function and recipient survival. Immunosuppressive treatment to decrease immunological reactions after organ transplantation may increase the risk of viral infections. The aim of this study was to evaluate the impact of the presence of HBs antigen and HCV and CMV antibodies on patient and graft survivals. MATERIAL AND METHODS: Two hundred one enrolled kidney transplantation patients (96 women and 105 men) were treated with the same immunosuppressive regimen. Age, sex, and viral state (HBs antigen, anti-HCV and anti-CMV antibodies) were evaluated in every patient. Statistical analysis was performed with the Gompertz model, Kaplan-Meier curves and Cox proportional hazard tests. RESULTS: The presence of HBs antigen was detected in 161 patients (20.4%), HCV antibodies in 61 recipients (30.3%); and CMV antibodies in 12 patients (5.9%). Eighty-seven recipients (43.4%) were seronegative. Average recipient age was 38.5 years. CONCLUSION: Time of graft function was independent of the presence of HBs antigen or HCV or CMV antibodies.
The authors present the results of spinal cord function evaluation in organ donors by examination of reflexes. The knee reflex and ankle jerk, as well as abdominal reflex and foot withdrawal reaction were tested in 31 donors. At least one of the above was seen in 25 individuals. It is suggested on the basis of data obtained not to hamper the transplant procedure in cases of tested reflex persistency when brain death is revealed on relevant examination.
