Attitudes and knowledge about preventive dental care in Chilean refugees in Sweden.

The aim of this study was to evaluate the effects of a simplified oral health programme on attitudes to and knowledge of preventive dentistry. The subjects were Chilean refugees and the programme was delivered at one or two sessions in the form of group information/discussion. Because of increasing immigration, Sweden has become a multicultural society. The number of non-Nordic immigrants has doubled in the past decade. The major refugee groups have come from Iran, Chile and Poland. The subjects comprised 193 Chilean refugees: 106 in a single-visit group and 87 in a two-visit group. The oral health programme was completed by 94 and 65 subjects respectively and was evaluated after 6 months. Positive effects were discernible in attitudes to and knowledge of preventive dentistry, particularly with respect to oral hygiene. A key to success may have been group discussion in which the refugees could relate oral health problems to their own ethnic group. This could have an important function in bridging cultural, linguistic and situational barriers. Different forms of outreach programmes for oral health via groups, organisations or authorities in close contact with refugees shortly after arrival in Sweden are proposed. This approach may be particularly effective in a multicultural society and also in the context of the turbulent conditions the newly-arrived refugee experiences.

Pulpal reactions in rat incisors to Caridex.

This study examined the in vivo effects of Caridex, a chemomechanical caries removal system, on rat pulpal tissue. Rat incisors were opened and the pulps exposed to Caridex or physiological saline and sealed with calcium hydroxide. After various time periods, teeth were extracted and examined by light microscopy. Histological evaluation revealed an almost identical response in both test and control teeth which consisted of a transient inflammatory reaction and a limited necrosis in adjacent pulp tissue. Within seven days, formation of hard tissue matrix was seen below the necrotic area and on pulpal walls. It was suggested that the high pH of Caridex may have contributed to the necrotizing effect of calcium hydroxide in adjacent pulp tissue and the formation of hard tissue matrix. Additionally, the solution is most probably bactericidal. The results suggest that the system can be used as a caries removal agent on humans without unfavourable side effects on the dental pulp.

An estimation of dental treatment needs in two groups of refugees in Sweden.

The aim of this study was to estimate dental treatment need in groups of Chilean and Polish refugees in Sweden. Of the Nordic countries, Sweden accepts the greatest number of refugees. An average of 5000 refugees arrived annually in 1981-85, increasing to 15,000 during 1986-87. Refugees and their families now comprise 93% of non-Nordic immigration. In 1981-83 a sample of 193 Chilean and 92 Polish refugees in the county of Stockholm was selected for this study. Dental treatment needs were calculated in accordance with CPITN and the working study of Swedish dentistry, which formed the basis for the Swedish scale of dental fees for the National Dental Insurance Scheme. The estimated mean treatment time (+/- SD) in the Chilean sample was 6.9 +/- 2.3 h and in the Polish group 8.4 +/- 3.0; in comparison with estimated treatment needs in a Swedish material, both would be classified as extreme risk groups. There was no correlation between the number of months in Sweden and the estimated treatment needs. The results indicate a cumulative, unmet need for dental care in these groups. Barriers to ensuring adequate health care for immigrants persist; special outreach programmes, conducted by dental health personnel, may be an effective means of introducing immigrants to the Swedish dental care system.

Controlled and uncontrolled thrombocytosis. Its clinical role in essential thrombocythemia.

Only two of 19 patients with spontaneously evolving essential thrombocythemia remained asymptomatic in a 421 patient-month observation. The rest of the patients showed hemorrhagic diathesis (four patients), nonspecific neurological semiology (two patients), and occlusive vascular illness in cerebral, myocardic, arterial, and often multiple locations (total, 12 patients). Peripheral neuropathy was found in five of 10 patients studied. In this series the incidence of cerebral ischemia in the uncontrolled condition was 180 times higher than the epidemiologic expectancy in a population not affected by the disorder. Of 35 ischemic attacks, 22 occurred when the platelet count was more than than 1000 X 10(9)/l, 13 when the count ranged from 650 to 990 X 10(9)/l, and none occurred at counts of less than 650 X 10(9)/l. In contrast, therapeutic control of the thrombocytosis caused all complications to disappear. These findings point out the danger of the natural course of the illness and justify active therapy. At the same time they call into question some of the most commonly used criteria in the diagnosis of essential thrombocythemia.

Dental health status in two groups of refugees in Sweden.

The aim of this study was to determine dental health status in two separate groups of Chilean and Polish refugees in Sweden. In Scandinavia, Sweden has the largest number of immigrants--1 million out of a population of 8.3 million. Since 1975, most immigrants have been refugees and their families. During 1978-82 Sweden granted residency to 20,000 refugees, the two largest groups being Chileans and Poles. In 1981-83 a sample of 193 Chilean and 92 Polish refugees in the county of Stockholm were selected for this study. The investigation consisted of a questionnaire followed by clinical examination, including roentgenograms. The average age was 34.0 years in the Chilean group and 34.8 years in the Polish group. The Chileans had been in Sweden for 17.3 months on an average and the Poles for 16.0 months. The Chileans had an average of 10.0 carious surfaces, D(s), and the Poles 11.3. Gingivitis was recorded in 87% of the total number of sites examined in the Chilean group. The corresponding figure in the Polish group was 79%. Of the Chileans 36.5% and of the Poles 32.5% had periodontal pockets measuring more than 5 mm. The results indicate that, when compared with Swedish individuals of a corresponding age, the refugee groups have a high prevalence of caries and periodontal disease.

Adjuvant chemotherapy with and without tamoxifen in the treatment of primary breast cancer: 5-year results from the National Surgical Adjuvant Breast and Bowel Project Trial.

In this National Surgical Adjuvant Breast and Bowel Project (NSABP) clinical trial, 1,891 women with primary operable breast cancer and positive axillary nodes were randomized between Jan, 1977 and May 1980 to receive L-phenylalanine mustard (L-PAM) and 5-fluorouracil (5-FU) either with or without tamoxifen (TAM)-PFT. This report presents life table probabilities, cumulative odds ratios, and P values for disease-free survival (DFS) and survival at yearly intervals through 5 years of observation (mean time on study, 72 months). When patients were examined overall without regard for any discriminant associated with outcome, ie, age, number of positive nodes, or tumor receptor status, there was a significant prolongation of DFS (P = .002), but not survival through the fifth postoperative year. The benefit was almost entirely restricted to those greater than or equal to 50 years with greater than or equal to 4 positive nodes. In that group there was a 66% greater chance of remaining disease free if PFT was received (P less than .001), and there was also a significant survival benefit (P = .02). The advantage from PFT was found to be associated with tumor estrogen receptor (ER) and progesterone receptor (PR) as well as patient age and nodal status. Overall there was a significant improvement in DFS from PFT in those having tumors with an ER or PR level greater than or equal to 10 femtomole (fmol) (P = .01 and .009, respectively). No significant benefit in DFS or survival has been observed in patients less than or equal to 49 years old related either to nodes or tumor receptor status. Survival continues to be adversely affected by TAM in those patients (less than or equal to 49 years old), particularly when their tumors have a PR of 0 to 9 fmol (P = .007). In patients greater than or equal to 50 years old with four or more positive nodes, a significant DFS benefit persisted through the fifth year of observation in those having tumor ER or PR levels greater than 10 fmol (P less than .001 and .002). The advantage was observed in patients 50 to 59 years old as well as those 60 to 70. Women in the older decade demonstrated some advantage from PFT when their tumor ER or PR was 0 to 9 fmol. The most likely explanation for this finding is analytical error in receptor analyses.(ABSTRACT TRUNCATED AT 400 WORDS)

The number, identity, meaning, and salience of ascriptive attributes in adult person perception.

The multidimensional scaling of similarities data was employed in a study of person perception in five adult-aged groups of both male and female respondents. Stimulus persons were males and females of the same five ages. Regardless of the age or gender of the respondent group studied, an identical set of three basic attributes was identified: perceived age, perceived gender, and perceived autonomy. However, respondents also assigned different saliencies to these attributes and used them in quite different ways to categorize the stimulus persons. While the amount of between-group variability observed in these data is large enough to suggest that adult person perception is somewhat idiosyncratic in nature, it was still possible to discern several generalizable trends for the attributes of perceived age and perceived autonomy. These trends and several methodological issues are presented and discussed within the text of this article.

Effects of venesection on cerebral function in chronic lung disease.

Regional cerebral blood flow measurements and neuropsychological testing were conducted before and after venesection on 6 patients with polycythemia secondary to chronic obstructive pulmonary disease. Venesection resulted in lowered viscosity and hematocrit, and an accompanying improvement in cerebral perfusion and mental function. Blood flow was significantly increased in the left cerebral hemisphere following phlebotomy, and there was significant improvement in sensory/mental function. Cerebral function would appear to be related to blood flow alterations influenced by the viscosity of the blood.

Metastatic squamous cell carcinoma of the skin.

Sunlight induced squamous cell carcinoma of the skin is common and produces low incidence of metastases. Non-actinic squamous cell carcinoma, however, possesses a metastatic potential even when well differentiated. A representative case of keratinizing squamous cell carcinoma arising in a lower extremity with development of widespread metastases is discussed. The prognostic factors associated with metastasizing de novo squamous cell carcinoma of the extremity include: site of origin, duration of lesion, degree of differentiation, sex of patient, and size of the primary lesion. Organs prone to metastasis include: regional lymph nodes, liver, lungs, and bone. As skin cancers of this variety metastasize, the clinician must recognize this potential when considering therapeutic strategy.

Serum-prolatic in women with premenstrual syndrome.

Serum-prolactin was measured by radioimmunoassay during the menstrual cycle in 28 women who had the premenstrual syndrome (P.M.S.) and in a control group who did not have P.M.S. symptoms. Throughout the menstrual cycle mean serum-prolactin was signficantly higher in women with P.M.S. than in the controls. The average individual increase in serum-prolactin during the premenstrual period compared with the first 3 weeks of the cycle was also significantly higher in women with P.M.S. It is not clear whether the increase in serum-prolactin was merely an indicator of stress or was actually involved in producing some of the symptoms which constitute P.M.S.
