Ropivacaine 0.1% with fentanyl 2 microg mL(-1) by epidural infusion for labour analgesia.

BACKGROUND AND OBJECTIVE: To evaluate the efficacy of 0.1% ropivacaine with fentanyl 2 microg mL(-1) via epidural for analgesia in labour. METHODS: In a randomized study, 80 nulliparous parturients in labour had epidural analgesia initiated with 0.2% ropivacaine and fentanyl and were then randomized to receive either 0.1% ropivacaine with fentanyl 2 microg mL(-1) at 10mL h(-1) (Group R1, n = 38) or 0.2% ropivacaine with fentanyl 2 microg mL(-1) at 8 ml h(-1) (Group R2, n = 39) as epidural infusions. Supplementary analgesia was provided on request with ropivacaine 0.2% 5 mL as an epidural bolus. RESULTS: There were no significant differences between the visual analogue pain scores either with respect to motor block or sensory block. The amount of local anaesthetic used was lower in the 0.1% ropivacaine group than in the 0.2% ropivacaine group (P = 0.001). Side-effects, patient satisfaction, labour outcome and neonatal outcomes were similar in both groups. CONCLUSIONS: An epidural infusion of 0.1% ropivacaine with fentanyl 2 microg mL(-1) at 10 mL h(-1) provided adequate analgesia in the first stage of labour. The level of analgesia was similar to that obtained using 0.2% ropivacaine with fentanyl 2 microg mL(-1) and with no differences with regard to motor or sensory block.

[Study of comparative immunity of patients with M. tuberculosis and atypical Mycobacteria infection and infection by human immunodeficiency virus (HIV)].

The purpose of this retrospective study was to determine the basal immune status, in patients with human immunodeficiency virus (HIV) infection, who were affected from tuberculosis (TB) caused by Mycobacterium tuberculosis or atypical mycobacterium. In the current study, we report our experience with 280 patients diagnosed TB and HIV: fifty cases were eligible for enrollment at random with M. tuberculosis (group I) and we compare with 23 cases of atypical mycobacterias (group 2). When we analyze statistically both groups, we detected in group 2 the existence of a CD4 Lymphocytes count decrease and a mean IgA raised, opposite to group I (P < 0.0001). On other side, our investigation on survival showed that this was lower in atypical mycobacterias group (group 2) with a median time of survival of 10 months, opposite to 2,9 years in group I.

[Mycobacteriosis from Mycobacterium szulgai in a patient with human immunodeficiency virus infection].

We present a case of renal and, probably, bone mycobacteriosis by M. szulgai in a patient infected by the human immunodeficiency virus (HIV). M. szulgai is an atypical skotochromogen mycobacterium rarely described in the literature. Clinically, it is characterized by the predominance of pulmonary manifestations undistinguishables from M. tuberculosis, and rarely affecting other organs. In our environment, such mycobacterium has never been isolated in the main series published in the literature. The evolution of the patient was good, disappearing the clinical signs after the tuberculostatic treatment. The antibiogram showed resistance to rifampicine, piracinamide and ethambutol.

[Pneumocystis carinii pneumonia in AIDS and prolonged fever].

Pneumonia by Pneumocystis carinii (NPC) presents a high incidence in the evolution of patients infected by the human immunodeficiency virus (HIV). Common clinical signs include fever, dry cough and dyspnea, in the presence of pulmonar interstitial affection with several degrees of hypoxemia. One hundred and sixteen patients with NPC and infection by HIV were diagnosed between December 1986 and January 1990. Criteria of persistent fever was established in 10 of them (8.7%), with normal thoracic radiography at the time of hospitalization. NPC in the adquired immunodeficiency syndrome (SIDA) may develop' with persistent fever, joining the large relation of entities manifesting in this way.

[Pulmonary tuberculosis with normal chest radiography and infection by human immunodeficiency virus].

We report 16 cases of pulmonary tuberculosis with normal chest x-ray film associated with infection by the human immunodeficiency virus (HIV). These patients belong to a sample of 125 cases of tuberculosis and HIV infection observed during the same study period. The clinical features and the diagnostic methods are described. The unusual character of the finding and its relevance are discussed, as the presentation of pulmonary tuberculosis with normal chest x-ray film was relatively common in patients with HIV infection (12.8%) in our series. This possibility should be considered in the whole population of patients with HIV infection.

Trichosporon beigelii pneumonia in a neutropenic patient.

A case of pulmonary infection caused by Trichosporon beigelii is reported in an asthmatic patient undergoing steroid treatment who developed fever and lung infiltrates. Arthroconidia and blastoconidia were isolated from repeated sputum, bronchial aspirate and telescopic catheter samples. The infection coincided with neutropenia resulting from pyrazolone treatment. The response to amphotericin B treatment was favourable.

T.I.A. revisited: new wine--old bottle.

TIA has been accepted as a concept of transient cerebral ischemia. There is an ever enlarging body of literature sparking controversies considering its natural history, prognosis, outcomes of clinical trials and usefulness of the concept. Much of the controversy is related to the diverse pathophysiology of TIAs, the selection of patients for clinical trials and the associated risk factor in the studied population. The advantages of the concept are a warning of the possibility of future irreversible pathology an opportunity to treat and/or remove risk factors an opportunity to establish a basis for monitoring the patient and making interim decisions concerning intervention. The risks of the concept are: administering a specific therapy to all patients, failure to fully evaluate the patient once the diagnosis is made, tendency to rely on "experts" for guidance. TIA is a useful concept if one recognizes: the pathophysiology is polymorphous and multiplex, the concept is a signal for diligent search for the underlying cause or causes, it is more important to treat the risk factors than to give a specific therapy.

The underlying problems resulting in controversies in the clinical management of transient ischemic attacks: an overview.

Of all the controversial topics in the management of cerebrovascular diseases the subject of transient ischemic attacks is perhaps the most controversial. A preponderance of literature related to the subject, reflecting a remarkable degree of interest in the topic, is a result of this controversy. Each conclusion serves to stimulate more interest, more evaluation, and more controversy. We will attempt to give an overview of the controversies in the management of transient ischemic attacks.

[Enzymatic activities of Campylobacter jejuni, C. coli, and C. lari].

Susceptibility to nalidixic acid has been considered for a long time as an important test in the identification of the different species of the genus Campylobacter. However, due to the increasing development of resistance new laboratory tests are needed to carry out an accurate identification to the species level in combination with other typing schemes. We have evaluated the enzymatic activity of 180 Campylobacter spp. strains isolated from clinical specimens performing the Api ZYM system (bioMérieux) in order to assay whether different enzymatic patterns could help to characterize these microorganisms. Thirteen of the 19 enzymatic activities detected by the system (lipase-C14, valine arylamidase, cystine arylamidase, trypsin, chymiotrypsin, alpha-galactosidase, beta-galactosidase, beta-glucuronidase, alpha-glucosidase, beta-glucosidase, N-acetyl-beta-glucosaminidase, alpha-manosidase and alpha-fucosidase) were negative for all the strains tested, whereas 3 enzymes (alkaline phosphatase, acid phosphatase and naphtol-A-S-BI-phosphohydrolase) were detected in 96.6% of the strains. Esterase-C1, esterase lipase-C8 and leucin arylamidase showed a variable reactivity depending on isolates. Enzymatic activity patterns clearly differentiate all the C. lari strains from other Campylobacter species. No significant differences were detected among the enzymatic activities of C. jejuni and C. coli strains. Our study suggests that the Api ZYM system is easy to perform and a valuable method to be applied in the characterization of the campylobacteria as a complement to other biotyping and serotyping schemes.
