Shoulder arthroscopy in the athlete. Practical applications.

Shoulder arthroscopy may be a useful tool to the surgeon in confirming a diagnosis for therapeutic treatment of a diagnosed condition. Shoulder arthroscopy has provided valuable information in establishing a diagnosis in the athletic individual when overlapping symptoms make a clinical diagnosis difficult. Shoulder arthroscopy for shoulder instability is in its developmental stages with early studies showing a high redislocation rate. Impingement syndrome is initially treated with conservative methods; however, if this is unsuccessful acromioplasty can be considered. Arthroscopic acromioplasty allows a thorough examination of the glenohumeral joint. Patients with rotator cuff tears may get pain relief with arthroscopic debridement. Inflammatory diseases such as rheumatoid arthritis can be treated with arthroscopic synovectomy. Shoulder arthroscopy is a valuable tool in the treatment and diagnosis of shoulder disorders.

The usefulness of MRI in evaluating menisci after meniscus repair.

We reviewed magnetic resonance imaging (MRI) scans of 15 asymptomatic patients at 6 months to 1 year following meniscal repair procedures. All of the patients were found to have persistent MRI signals in the region of the repair. Nine of the 17 meniscal repairs in the 15 patients had persistent grade 3 signals. Arthrograms were performed on six patients (eight meniscal repairs). Seven of these postoperative menisci showed grade 3 signals on MRI. Only one meniscus was shown to have a complete tear by arthrogram. There were no false negative scans. The overall accuracy, defined as the percentage of postoperative menisci correctly diagnosed by MRI, was 38% (3/8). MRI is a useful preoperative diagnostic study following knee injury. This report demonstrates, however, that present MRI modalities are unable to distinguish between scar tissue of healed meniscal repairs and meniscal tears. Therefore, it is not a useful diagnostic tool in evaluating reinjury following successful meniscal repair.

Dislocations following arthroplasties of the hip. Incidence, prevention, and treatment.

Three methods of avoiding postoperative prosthetic dislocation following total or bipolar hip arthroplasty are described; each approach is recommended for a specific population. The anterior surgical approach was utilized for bipolar hip endoprosthetic arthroplasty in 62 minimal or bed-to-chair ambulators with established flexion contractures about the hip. Two of these patients subsequently experienced a prosthetic dislocation, one of which was secondary to infection. An additional 98 patients who underwent arthroplasty using the posterior approach wore a knee immobilizer postoperatively, without incidence of prosthetic dislocation. A modification of Wilke boot casting was utilized for four patients following reduction of a dislocated hip prosthesis. The cast consisted of a short cylinder cast on the affected extremity connected by a cross bar to a short leg cast on the contralateral leg. The casts controlled rotation, were well tolerated, and resulted in no redislocations. We therefore recommend the anterior approach in minimal ambulatory patients who are to undergo total bipolar hip arthroplasty. When the posterior approach is used in hip replacement procedures, we have found that postoperative use of a knee immobilizer helps prevent dislocation. Following reduction of a dislocated hip prosthesis, a modification of the Wilke boot cast that does not immobilize the affected ankle can be used.

Multi-centre clinical investigation of indapamide in the United States: a review.

Indapamide, a new indoline antihypertensive agent, has been the subject of a worldwide programme to develop this drug for general clinical use. The results are described of the multi-centre U.S. clinical programme demonstrating the effectiveness and tolerance of indapamide for the treatment of hypertension. All work was conducted under U.S. Federal Food and Drug Administration guidelines, and resulted recently in a New Drug Application. A total of 1891 subjects or patients participated in 27 separate studies conducted by 91 investigators. In controlled clinical trials comparing 2.5 mg indapamide once daily with 50 mg hydrochlorothiazide once daily for 40 weeks in patients with mild to moderate essential hypertension, indapamide produced a reduction of supine blood pressure of -9.5/-14.3 mmHg as compared with -7.6/-11.4 mmHg for hydrochlorothiazide. In combination with methyldopa, propranolol, clonidine, guanethidine and hydralazine, indapamide consistently produced a greater decrease in arterial pressure than did those agents given alone. Indapamide added to these step-care agents did not result in a meaningful increase in adverse reactions. Indapamide has been the subject of a long-term safety study in which over 100 hypertensive patients have been followed up for 2 years or longer. During this period of time, indapamide was well tolerated and remained effective. No biochemical, electrocardiographic or ophthalmological changes were associated with its use. Other studies with indapamide are discussed describing the systemic and renal haemodynamic effects, pharmacokinetic properties and special safety studies conducted with this agent. The use of indapamide in patients with hepatic or renal impairment is reviewed in detail.

[Splenic emergencies and pregnancy].

The authors report a personal case of spontaneous rupture of the splenic vein during pregnancy, already published elsewhere [6], and study again spontaneous rupture of the splenic vessels, artery or vein, and rupture of the spleen in relation to pregnancy, either during or after labour. In all, 150 cases were found in the world literature. This is an extremely dangerous surgical emergency, and the mother's life is in danger immediately. We attempted to determine the clinical and paraclinical factors which would permit one to lay down the early indications for operation in these two emergencies. Splenectomy remains, in all cases, the basis of surgical treatment. In cases of maternal survival, the late, general and obstetric prognosis remains good.

Incidence of exercise-induced bronchospasm in chronic non-allergic rhinitis in children.

Out of 20 children diagnosed as suffering from chronic non-allergic rhinitis, only one (5.0%) suffered from exercise-induced bronchospasm, as compared with 47.2% of children with allergic chronic rhinitis, who showed bronchospasm after running. These findings support the view that mediators of hypersensitivity are released in E-IB and may play an important role in its pathogenesis.
