Acetabular labral tears: evaluation with MR arthrography.

PURPOSE: To determine the usefulness of magnetic resonance (MR) arthrography in the diagnosis of acetabular labral tears. MATERIALS AND METHODS: MR arthrography of the hip was performed in 10 patients who underwent subsequent surgical evaluation. Eight arthrograms were obtained with intraarticular administration of gadolinium solution and two with intraarticular administration of normal saline. T1-weighted spin-echo (intraarticular gadolinium) or T2-weighted gradient-echo (intraarticular normal saline) images were obtained in the axial, sagittal, and coronal planes with use of a surface coil. Criteria for labral tears included labral blunting, absence, displacement, intrasubstance contrast material, and contrast material at the acetabular-labral junction. Labra with enlargement, intrasubstance intermediate signal intensity, or irregular margins were interpreted as degenerative. RESULTS: Labral tears were diagnosed in eight hips. Tears included six labra with contrast material that tracked at the acetabular-labral junction, one of which had associated intrasubstance extension. One tear was confined to the labral substance. The other tear exhibited absent labral tissue and an irregular remnant. One degenerative labrum and one normal labrum were identified. All MR arthrographic findings were confirmed at surgery. Extension of one anterosuperior tear into the posterosuperior labrum was not prospectively appreciated. CONCLUSION: In this preliminary study, MR arthrography appears to be a promising imaging modality for accurate diagnosis of acetabular labral tears.

Metal levels in cemented total hip arthroplasty. A comparison of well-fixed and loose implants.

In a prospective study, synovial fluid metal levels from stainless steel, cobalt-chromium, and titanium-alloy cemented total hip implants were measured. There were 37 well-fixed and 44 loose hip arthroplasties. Tissue-metal levels were quantitated in the cases revised for loosening. Retrieval analysis for implant wear was performed. Synovial fluid analysis showed a fivefold increase in metal levels of loose compared with well-fixed stainless steel implants. There was a sevenfold increase in metal levels of loose compared with well-fixed cobalt-chromium implants. There was a 21-fold increase in metal levels of loose compared with well-fixed titanium-alloy (Ti-6Al-4V) implants. Tissue-metal levels from revised cobalt-chromium implants averaged 45 micrograms/g dry tissue weight compared to 4,470 micrograms/g dry tissue weight from revised titanium-alloy implants, a 100-fold increase. Implant retrieval analysis showed severe burnishing and scratching in all titanium-alloy femoral heads and extensive burnishing and scratching in the majority of the femoral stems. Well-fixed cemented implants have similar low synovial fluid metal levels. However, when loosening of implants occurs, titanium-alloy implants release disproportionate levels of metal into synovial fluid and local tissues compared to stainless steel or cobalt-chromium.

Biological Properties of d-Amino Acid Conjugates of 2,4-D.

Some d-amino acid (glutamic acid, valine, or leucine) conjugates of 2,4-dichlorophenoxyacetic acid (2,4-D) at 10(-5) molar, stimulated elongation of Avena sativa L. var Mariner coleoptile sections and growth of soybean (Glycine max. L. var Amsoy) tissue as much as did the l-amino acid conjugates at 10(-6) molar. The d-methionine conjugate did not stimulate growth of soybean root callus tissue but did stimulate Avena elongation. The d-aspartic acid conjugate did not stimulate elongation of Avena coleoptiles but did stimulate growth of root callus tissue.Pretreatment of root callus tissue with 100 micromolar 2,4-D-(d) -or (l) glutamic acid for 18 hours prior to incubation for 6 hours in [1-(14)C]2,4-D resulted in a greater uptake of [1-(14)C]2,4-D than did pretreatment with 2,4-d. Qualitatively the metabolites were similar in all preincubation treatments. Preincubation with 2,4-D-(d)-glutamic acid also resulted in larger free [(14)C]2,4-D pool and a significant increase in water-soluble conjugates when compared with 2,4-D, or 2,4-D-(l)-glutamic acid preincubations.
