[Pyelonephritic scar formation].

Pyelonephritic scars almost never develop after urinary tract infections alone, but require in addition certain risk factors. The most important risk factors are: vesicoureteral reflux, intrarenal reflux, obstructive uropathy, and nephrolithiasis. Most of the pyelonephritic scars develop in early childhood, where we find the combination of vesicoureteral reflux, intrarenal reflux, and urinary tract infections. Urinary tract infections alone almost never result in pyelonephritic scars. After the age of 3 years the risk to develop pyelonephritic scars is not more than 10% in those with the combination of urinary tract infections and vesicoureteral reflux. In this age group the risk to develop pyelonephritic scars is not altered by antireflux operation.

[Sonography in the diagnosis of pyelonephritis scars].

Radiological and ultrasound examinations were performed in 34 women with recurrent urinary tract infection, as part of a prospective study of pyelonephritis. Ultrasound was found to be valuable in the recognition of new pyelonephritic scars and for screening. In assessing ultrasound and radiological findings the criteria of Hodson are equally valuable in differential diagnosis. Taking into account radiation exposure, ultrasound should not replace intravenous urography in this field, but if regular serial examinations are necessary it can be used alternately with urography.

[Significance of the urodynamic study in the diagnosis of urinary incontinence in women].

Comparing the results of clinical evaluations and urodynamic studies in 175 consecutive incontinent females, we came to different diagnoses in 12.6%. If we rely only on the clinical diagnosis, this means that at least 1 out of 10 incontinent females with no urodynamic work-up will have a wrong or incomplete therapy. In respect to the exact indication, especially for operative therapy, urodynamic work-up seems therefore necessary in different forms of female urinary incontinence.

[Comparison of bacteriologic urine studies following bladder puncture and bladder catheterization].

In 108 female patients with history of urinary tract infections bacteriologic work-up was done in urine taken by catheterisation. 85% of the urine samples were sterile, 15% had bacterial counts less than 10(4) ml. The incidence of contaminated urines was the same when the urine was controlled one and three weeks later again by catheterisation. When urine samples were taken by suprapubic bladder puncture at the time when the catheterisation was done, a significant higher percentage of the punctured urine was contaminated. The results were discussed in terms of the interpretation of bacteriologic data obtained by suprapubic bladder punction.

[Comparison between sonography and intravenous pyelography in follow-up controls in recurring urinary tract infections].

Real-time sonograms and IV pyelograms were compared with one another in 34 female patients of 6 to 36 years of age with relapsing urinary tract infections. All scar tissue formations detected by x-ray film were also identified via sonography. In five further cases, sonography showed scar formation three times and one each concrement and a trabeculated bladder that were not seen--or only retrospectively seen--roentgenologically. Other x-ray film findings, such as a dichotomic pelvi-calyceal system, renal duplication, malrotation, concrements, calcified calyceal diverticle, retention of urine leading to obstruction, resection of renal pole and an "empty renal bed", were also confirmed by sonography. The physician who performed and assessed the sonographic examination had been unaware of the x-ray and clinical findings or anamnesis prior to sonography.

[Congenital heart defects as a cause of still birth and infant mortality].

Among 4,686 still-births and children died from 0 to 1 a in the German Democratic Republic in 1979 there were 307 (6.6%) isolated congenital heart diseases. We only analyzed 294 died infants. The first 5 diagnoses were: Hypoplastic left heart (all infants died within the first 28 d) 9.9%, Ventricular septal defect 9.9%, Transposition of great vessels 8.8%, Coarctation aorta 7.1%, Common truncus arteriosus 5.8%. The most important results are an interesting frequency with advancing years of mothers (3.4% in 10,000 mothers of live-births aged 16 a and less to 20.6% at the age of 40 a and over (see Fig. 1), 13% premature children (under 37 weeks of gestation), 23% children under 2,500 g birth weight, 55% boys and 45% girls. 80% of the above 294 infants died within the first 3 months.

Correlation of symptoms and results of direct bacterial localization in patients with urinary tract infections.

During the last 8 years we localized 725 urinary tract infections in 213 patients via the bladder washout test. With this test the infections were located in the upper urinary tract in 11.9 per cent of the patients without vesicoureteral reflux, 27.1 per cent with reflux and 25.3 per cent without reflux after a successful antireflux operation. Of all supravesical and vesical bacteriuric episodes 57.6 and 67.1 per cent, respectively, were asymptomatic. The presence of symptoms differed only slightly among patients with and without reflux, and with and without pyelonephritic scars. Vesical symptoms, such as dysuria and frequency, were seen in 18.8 per cent of the patients with supravesical and 26.2 per cent with vesical infections, while fever and/or loin pain occurred in 23.6 and 6.7 per cent, respectively. However, urinary tract infections with loin pain and/or fever were localized evenly between the bladder and upper urinary tract. We conclude that factors other than the site of infection determine the presence or absence of symptoms.

Pyelonephritic scarring in 213 patients with upper and lower urinary tract infections: long-term followup.

Of 213 patients more than 3 years old with recurrent urinary tract infections new pyelonephritic scars formed only among those with a combination of urinary tract infection and vesicoureteral reflux. Of 61 patients with that combination new renal pyelonephritic scars developed in 7 (11.48 per cent) and severe clubbing in 2. Of 40 patients with vesicoureteral reflux who underwent an antireflux operation new scars formed in 5 and progressive renal damage occurred in 1. Most renal scars (58) were present at initial examination. Neither number of urinary tract infections, number of upper urinary tract infections localized by the bladder washout technique nor attacks of flank pain and fever were seen more often in patients who suffered new scars, and grade of reflux was not a factor. Patients in whom new scars developed during our prospective followup were younger than those who did not have new scars. However, new scars formed after an initial normal excretory urogram in only 1 patient with vesicoureteral reflux and urinary tract infection compared to 4 after an antireflux operation.

[Therapy of lymphangioma in the head and neck region].

20 patients with a lymphangioma of the head and neck were observed after surgery over follow-up intervals of 3 to 13 years. In six children the tumor removal had to be subtotal in order to preserve vital organs. Though small areas of tumor were left no recurrent tumor growth was observed in this group. In 14 patients the tumor was surrounded by a capsule, in six newborns and children the tumor showed an infiltrating growth, which led to difficulties in breathing and swallowing. Out of the 14 radically operated patients, three had a recurrence after 1-2 years. In the six function conservating and not radical operated children, in whom small areas of tumor were left, no recurrence was noticed within 3-13 years.

[Mucoviscidosis (cystic fibrosis), a disease of an unclear structure until recently].

Cystic fibrosis (mucoviscidosis) is described in a review article. The cause of this common lethal hereditary disease of white people is hitherto unknown. The early dead of patients with cystic fibrosis, a genetic paradoxon, a lot of hypotheses and very high demands in treatment should challenge more scientists for research. There are added some short notes about the authors work in history of cystic fibrosis.

[Sterile vesiko-ureteral reflux. Radiological kidney surface measurements as follow-up criteria].

33 patients with 46 sterile vesicoureteral refluxes were followed by repeated clinical-urological and radiological studies over a period from 1 to more than 6 years. The results of the evaluation, obtained by only looking at the urograms, were compared with the following measurements: the length of each kidney, the length ratio right to left, and the parenchymal area. At the end of the follow-up period, 2 out of 32 initially normal kidneys (about 6%) had pathologically low values, whereas 3 out of 14 already initially damaged kidneys (21,5%) showed a further decrease of these values. Preceding urinary tract infections with consecutive pyelonephritis, leading to a progressive shrinkage of the organ over many years, are thought and discussed to be more responsible for these results than the sterile reflux itself.
