[Skin manifestations in renal transplants].

The authors studied the skin disorders in 50 patients who have undergone renal transplantation. They observed: -- Viral infections (herpes simplex, herpes Zoster, warts) in 56% of the patients. -- Bacterial infections in 36%, resulting in septicemia in 8% of the cases. -- Fungal infections in 26% of the patients. These infections appeared more severe than usual and recurred frequently. The occurence of several infections processes in the same patient was not uncommon. The clinical aspect and high incidence of various infections is related to immunosuppresive therapy. However, there is no clear-cut correlation between the type of infection and the type of treatment used. -- Squamous cell carcinoma occured in one patient. A high incidence of malignancies is known to occur in immunosuppressed patients. -- Skin signs related to hemodialysis (pruritus, hypermelanosis, skin dryness, vascular disturbances) regressed. -- The incidence of adverse reactions to drugs was high. -- 4 cases of ulcerations of the oral mucosa probably related to Azathioprine were observed.

[Immunopathological study of Gougerot-Ruiter's vasculitis (author's transl)].

The authors report 10 cases of selected Gougerot-Ruiter's disease and 13 cases of various vasculitis. Direct immunofluorescence studies were performed on the skin lesions and the results were correlated to various biological parameters such as dosage of serum IgA, IgG and IgM, antinuclear antibodies, Australia antigen. Complement (beta-1-C/beta-1-A) deposition was found in 9 out of 10 cases of Gougerot-Ruiter's disease as opposed to immunoglobulin deposition found only in 4 out of 10 cases. Both C3 and immunoglobin were regularly found in the vessel walls of 2 cases of cryoglobulinemia and 1 case of SLE. In the cases of thrombopenic purpura, simplex purpura. Schoenlein Henoch purpura and drug induced purpura, complement (beta-1-C/beta-1-A) and immunoglobulin were absent in the lesions. The authors discuss the different physiopathological mechanisms which may be involved in the development of Gougerot-Ruiter's vasculitis.

[Skin manifestations in patients with renal chronic renal failure on regular hemodyalysis].

The authors studied the skin disorders in 50 hemodialyzed patients. Pruritus appears to be the main dermatological feature by the frequency of its occurrence and by its unpleasant and even intolerable effects. This sign is more frequent among long time dialyzed patients and seems to be due to 2 factors: high urea blood concentration and secondary hyperparathyroidism. Calcinosis cutis is less frequent than pruritus. It seems to have the same origin and can be considered in the more general picture of the metastatic calcinosis in patients with renal failure and secondary hyperparathyroidism. Hypermelanosis, exceptional in the chronic renal insufficiency patients before hemodialysis, is present in 41% of our group. It is more obvious in the long time dialyzed patients. The nail disorders are mostly the absence of lunula (30%), related to the anaemia, and the half and half nail (36%) that seems specific of the severe azotaemia. Skin dryness (30%) and ichthyosis (10%) can be related to the pruritus. Alopecia, drug reactions and prurigo seems to have a particular indidence. Two patients presented bullous eruptions localized in sunlight exposed areas of skin. The clinical, histological and immunological aspect was identical to that observed in the Porphyria Cutanea Tarda but all the porphyrin levels in the urine and faeces were normal.

[Specific markers of myocardial injury in acute stroke].

INTRODUCTION: It is thought that any acute damage to the central nervous system and, more particularly, acute cerebrovascular disease (ACVD) can give rise to a myocardial lesion. Our aim is to apply the latest biochemical markers (troponin T, troponin I and myoglobin) to the study of this problem. PATIENTS AND METHODS: We conducted a retrospective study of 42 patients who were consecutively admitted to hospital with ACVD. The pathological antecedents and the clinical and electrocardiographic variables were considered in each case. A single determination of CK, CK MB, myoglobin, troponin T and troponin I was performed for each patient. RESULTS: The determination of the new biochemical markers was positive in a higher number of cases than CK and its MB fraction, or electrocardiographic alterations. This positivity, together with the troponin T and troponin I values correlate with mortality. CONCLUSIONS: We present the first research work to be published in Spanish that studies the new biochemical markers of myocardial damage in ACVD. We urge researchers to carry out further analyses on more extensive series in order to determine the influence myocardial damage has on mortality and to establish suitable therapeutic measures. This will also allow us to find out whether a certain location or size of the lesion can give rise to a higher predisposition to this kind of damage

[Immunological and immunocytochemical studies of the inflammatory infiltrating cells of cutaneous tumors].

The identication of mononuclear cells extracted from various inflammatory infiltrates of cutaneous tumours has been investigated and the relative amount of B and T cells determinated according to various assays. It has been found that there were two different patterns of distribution of the cells in the infiltrates: one with a E/EAC ratio similar 5 (squamous and basalcell carcinomas), the other with a E/EAC ratio similar 1(malignant melanoma). The identification in situ of Ig-producing cells has been developed in the same tissues fixed in Bouin's solution. The relative frequency of these cells has been determinated and appears very low in the malignant melanoma.
