Survival of an upper lip aesthetic complex using arterial reanastomosis only.

Venous congestion after microvascular replantation remains a formidable problem even under ideal circumstances. The patient reported is a young woman with a traumatically avulsed upper lip aesthetic complex. Blood flow was reestablished using microanastomosis of the labial artery, however, no veins of adequate size could be recovered. With systemic administration of heparin, allowing the flap to bleed freely in addition to the use of medicinal leeches, we were able to salvage an avulsed upper lip aesthetic complex with the use of a single arterial microanastomosis. This patient report exemplifies alternative methods used in dealing with venous inadequacy in a composite replant.

Multiple rhabdomyomatous mesenchymal hamartomas of skin.

A case of multiple rhabdomyomatous mesenchymal hamartomas is presented. The patient is a black male infant, the product of an uncomplicated term gestation and delivery. At birth, there were numerous polyps distributed over the periorbital and periauricular areas bilaterally. Some appeared fingerlike with constrictions below their tips. Others were branched or globular in shape. These projections showed spontaneous and independent movement, particularly during feedings. On histopathologic examination, the polyps were covered by squamous epithelium and contained normal follicular units. Bundles of skeletal muscle were present in the reticular dermis, extending into the subcutis. Regular cross-striations were seen in these muscle fibers. In some specimens, the muscle bundles formed a solid, central core. Skeletal muscle histochemical stains confirmed the presence of both types 1 and 2 muscle fibers. Electron microscopy revealed a normal skeletal muscle banding pattern. This case is the first report of multiple rhabdomyomatous mesenchymal hamartomas of skin. Functional skeletal muscle with spontaneous movement is part of the clinical picture.

Primary closure of the split-thickness donor site.

A simple method of improving the donor site of split-thickness skin grafting is presented. Advantages include fewer wound-care problems, decreased pain, and a more aesthetic donor site. The disadvantage of increased operating time and expense are well compensated for by this method.

Augmentation mammaplasty by means of the transrectus route.

A new operative technique has been developed for augmentation mammaplasty. Through an inframammary incision, the anterior rectus sheath is entered, and the pocket is dissected in an entirely submuscular plane. We have performed this procedure in 112 patients to date. Complications have been few. The capsular contracture rate in 90 patients followed for greater than 1 year is 7 percent. The inframammary crease can be lowered using this technique, making mastopexy unnecessary in most patients with moderate ptosis.

Preoperative and postoperative considerations in elective breast operations.

The number of young women undergoing elective breast operations is increasing rapidly. The lifetime breast cancer risk in the female population is 1 in 10. As this group ages, it is obvious that surgeons will encounter carcinoma of the breast in these patients more frequently. The surgical implications of breast cancer in these patients are discussed, and guidelines for preoperative and postoperative management in patients being considered for augmentation mammaplasty, mastopexy, and reduction mammaplasty are suggested.

Peripheral in-continuity tissue examination.

When treating skin cancers, it is essential to remove the entire neoplasm if possible. Immediate reconstruction is most helpful in returning the patient to a useful and satisfactory life. The Mohs histologic technique can be time-consuming and cumbersome. Reconstruction can be delayed. A modification of Mohs technique, peripheral in-continuity tissue examination (PITE), is described in which the surgeon and the pathologist combine their talents to remove the tumor. All margins are evaluated, and the surgical defect is closed primarily. Larger and more complex tumors can be removed and defects immediately reconstructed using this efficient technique, obviating the inconvenience, pain, and expense of multiple, separate procedures.

A fasciocutaneous transposition flap for coverage of defects of the lower extremity.

During a three-year period, eleven fasciocutaneous transposition flaps were used in ten patients to cover a defect in the lower extremity that was due to trauma or infection. The patients were subsequently evaluated with respect to survival of the flap, sensation in the flap, healing of the donor site, and postoperative morbidity. Two cases are presented for illustration. Coverage with a fasciocutaneous transposition flap proved to be safe and simple. This technique offers several advantages compared with reconstruction using microvascular free muscle transfer, and it may be preferable for many patients who have a defect distal to the knee.

Reduction mammaplasty: central cone technique for maximal preservation of vascular and nerve supply.

Many types of breast reduction procedures have been described and are now being used. Before 1984, we used variations of the Wise technique, with free grafting of the nipple-areola complex; because of problems with inadequate nipple projection, squareness of the breasts, and decreased nipple sensation, however, we have subsequently used the central cone technique, as advocated by Hester et al. We present 44 cases with follow-up periods extending up to three years.

The perineal artery axial flap in reconstruction of the vagina.

We present two patients in whom an aesthetic and functional vagina was re-created using a perineal artery axial flap, based on the terminal vessels of the internal pudendal artery. This flap provides thin, supple skin for reconstruction of moderately sized vaginal defects leaving a minimal donor defect.

Tissue expansion in the treatment of hypertrophic scars and scar contractures.

The temporary tissue expander can generate an excess of full-thickness skin that can be recruited into an adjacent area of tissue deficiency caused by scar contracture. The creation of a pseudobursa enhances the functional improvement gained by this technique. Now in its developmental stage, tissue expansion carries an alarming complication rate (33%), but we expect refinements in technique to diminish this percentage.
