Artificial urinary sphincter cuff erosion. Experience with reimplantation in 38 patients.

Artificial urinary sphincter cuff erosion has become rare but remains a dreaded consequence of sphincter implantation. Our experience indicates that with rational and systematic management of this problem, sphincter reimplantation can restore continence in many of these patients. During a ten-year period, 412 artificial sphincters were implanted at the Mayo Clinic. Bladder neck or urethral erosion occurred in 64 patients (15.5%). Ultimately, 38 patients had reimplantation, and the outcome was successful in 32 (84%). After the advent of delayed activation, the incidence of cuff erosion decreased greatly--from a peak of 18 percent (12 of 66 cases) in 1977 to 1.3 percent (1 of 79 cases) in 1985.

Clinical experience implanting an inflatable penile prosthesis with controlled-expansion cylinder.

Ninety-four patients with erectile dysfunction underwent implantation of controlled-expansion inflatable penile prosthetic cylinders. Most of these patients had aneurysmal dilatation of the corpora cavernosa after prior implantation of inflatable devices. Follow-up of forty-six months has shown their efficacy to be excellent for management of primary impotence, penile straightening procedures, and revision after cylinder leaks. None of the patients has had recurrence of the primary condition that required revision.

Augmentation cystoplasty utilizing de-epithelialized sigmoid colon: a preliminary study.

The use of bowel segments for bladder replacement or augmentation has been associated with metabolic complications and obstruction due to mucus production. Establishment of a transitional epithelium over the de-epithelialized surface of a segment of intestine might alleviate these complications. Twenty Holstein bull calves underwent sigmoidocystoplasty. Fourteen experimental animals had the epithelium of the sigmoid removed before augmentation. Six calves with intact mucosa served as controls. Fifteen calves survived the study: 11 experimentals and four controls. Cystectomies were performed at four, six, eight, or 12 weeks. Ninety-one percent (10/11) of the experimental calves had almost complete epithelialization of the de-epithelialized graft. All experimental animals had residual colonic mucosa or mucoceles. Nine of 11 experimental calves (82%) had greater than 25% contracture of the sigmoid graft. Two animals had less than 25% graft contracture (1) or formed a wide-mouthed true diverticulum (1) in the grafted segment. All control animals formed a wide-mouthed true diverticulum and had no graft contracture.

The inflatable penile prosthesis: clinical experience with a new controlled expansion cylinder.

Controlled expansion inflatable penile prosthetic cylinders were implanted in 63 patients with erectile dysfunction from September 1983 through January 1986. The majority of these patients previously had received standard inflatable cylinders. To study the efficacy of these new cylinders, revision with the controlled expansion cylinder was used in various conditions, with aneurysmal dilatation of the corpora cavernosa being the most common cause for cylinder replacement. Controlled expansion cylinders were used for revision after cylinder leaks and for penile straightening procedures as well as in the primary treatment of impotence. During our 3-year followup of these patients there have been no recurrences of the primary problem that necessitated the revision.

Testicular seminoma metastatic to the prostate.

A 43-year-old man had clinically manifest prostatic involvement by a metastatic testicular seminoma. The signs, symptoms, diagnosis and treatment of this entity, as well as a hypothesis regarding its development, are discussed.
