Alpha-feto-protein-(AFP-)-secreting intra-pericardial teratoma--report of a case diagnosed on CT scanning.

A three-month-old boy presented with acute tamponade and respiratory distress. Only CT scanning of the thorax suggested the presence of an intra-pericardial tumour compressing the heart and causing congestive cardiac failure. A large bilobed pedunculated tumour arising from the root of the ascending aorta was surgically excised. Initial serum AFP level was elevated. Histology revealed the tumour to be a teratoma with foci of yolk sac carcinoma. Alpha-feto-protein was demonstrable by immuno-histochemical staining in the yolk sac carcinoma component. The child remains well one year after removal of the mass.

Duodenal ulceration presenting in childhood longterm follow-up.

Forty-one children diagnosed as suffering from duodenal ulceration by endoscopy and barium studies have been reviewed 5 to 15 years (mean 8.6 years) later. Mean age at diagnosis was 8.6 years. All were managed medically as children. Fifty-one percent are now asymptomatic, off all treatment; the rest have chronic symptoms typical of duodenal ulcer. Six patients (15%) required surgery, four as emergencies for complications and two for failure of medical management.

Esophageal anastomosis--an experimental model to study the anastomotic lumen and the influence of a transanastomotic tube.

The influence of an indwelling transanastomotic silicon tube on healing esophageal anastomosis in puppies was studied by making resin casts of the reconstructed esophagus. These casts demonstrated stenosis but no difference in the group with a transanastomotic tube was seen. Stenotic index of the puppies varied from 0.7 to 0.97 (no stenosis = 1). An important observation made during the study was the presence of shelfing of scar tissue on the posterior esophageal wall. This was the site of mucosal ulceration, granuloma formation, and fibrous scarring where interrupted 0000 silk knots had been tied intraluminally. The anterior wall had a linear scar, where the knots were tied extraluminally. Extraluminal knots would be part of the technique to achieve a minimal amount of scar tissue at the site of the esophageal anastomosis.

Thoracic neural crest tumors. A clinical review.

The experience of a regional children's hospital in the management of intrathoracic neural crest tumors over a period of 20 years is presented. Of 145 children with neuroblastoma between 1961 and 1980 inclusive, 115 had primary abdominal neuroblastoma (including pelvic) with a mortality of 86%; 30 patients had a thoracic neuroblastoma with a mortality of 16.5% (five patients). The more favorable outlook in this group should imply a more cautious approach to the initiation of treatment regimens which have their own morbidity and mortality.

Polyamines in colorectal cancer--a clinical and experimental approach.

The urinary polyamines putrescine, spermidine and spermine were measured prior to operation in 10 patients with colorectal cancer and 10 control subjects. Carcinoembryonic antigen assays were also performed in an attempt to correlate these values with polyamine excretion. The total polyamine rates in patients with colorectal cancer were 3.2 +/- 1.5 (SD) mg/24 h and 2.6 +/- 1.2 (SD) mg/24 h in the controls. The difference between the group with colorectal cancer and the controls was not statistically significant. Urinary polyamines were also measured in an experimental animal model for colorectal cancer in which tumour cell mass could be assessed. Only marginal differences occurred in polyamine rates between animals with extensive tumours and controls. These findings suggest that urinary polyamine measurement is unlikely to be a useful procedure to assess tumour cell mass in patients with colorectal cancer.

Cryptorchism in cerebral palsy.

This study examined an association between cerebral palsy and cryptorchism. Twenty-one (53.8%) in a group of 39 severely disabled males exhibited cryptorchism to a varying degree. A striking feature was the high proportion with an impalpable testis on one (15.4%) or both (7.7%) sides. Early damage to the fetal hypothalamo-pituitary axis is suggested as the likely causative factor.

Neurofibrosarcoma of the duodenum in von Recklinghausen's disease.

The clinical features and surgical management of 2 patients with neurofibrosacroma of the duodenum in association with von Recklinghausen's disease are described. The tumours were excised and the resulting duodenal defects closed satisfactorily using the jejunal serosal patch technique. Three such neurofibrosarcomas reported previously in English are reviewed.

Diagnosis and management of post-cholecystectomy symptoms: the place of endoscopy and retrograde choledochopancreatography.

Endoscopic retrograde choledochopancreatography (ERCP) was employed in 52 patients symptomatic after cholecystectomy in whom barium meal studies and intravenous cholangiography had failed to yield a diagnosis. Cannulation of the papilla of Vater was carried out in 50 of the patients in a mean time of 10 min. Diagnostic information demonstrating a pathological lesion or a normal biliary and pancreatic ductal system was obtained by means of endoscopy and ERCP in 48 cases. There were no serious complications. A pathological lesion was demonstrated in all but one of the patients presenting with jaundice, cholangitis or pancreatitis but the diagnostic yield was not as high in patients with vague upper abdominal symptoms. Endoscopy and ERCP are the investigative procedures of first choice in complex post-cholecystectomy cases in whom intravenous cholangiography fails, gives incomplete information or suggests normality in the face of continuing symptoms or clinical evidence of residual biliary disease.
