We describe a two-step predictive method to assess the probability of bacterial etiology in cases of diarrhea. The patients are divided into three probability groups (high, intermediate, low) according to a clinical score. Thereafter the patients in the intermediate group are further assigned to the high or low probability group according to the presence or absence of fecal leukocytes. We developed the method in an initial series of 157 children with acute diarrhea; the reproducibility of the method was tested in a second series of 180 cases. Eighty-six percent of patients in the first series and 81% in the second were correctly classified by our method. We suggest its use, especially in pediatric practice, to identify children with the highest probability of bacterial diarrhea, thus reserving stool culture to a few selected cases because only a minority of children with diarrhea have a recognized bacterial etiology.

A conjugate has been constructed consisting of diphtheria toxin fragment A (DTA) linked to a monoclonal antibody, BLMRL-HMFG-Mc5 (MC5), directed against a mammary cancer antigen. The conjugate retains both binding and DTA enzymatic activity when tested against target MCF-7 cells, although the conjugate binds less well than the unconjugated antibody. The conjugate is toxic to MCF-7 cells. Toxicity is both dose- and time-dependent. Half-maximal toxicity is observed with 2-5 X 10(-8) M conjugate after 5 days of incubation. However, the conjugate need only be in contact with the cells for 1 day in order to effect complete killing by 5 days. The earliest that an effect can be seen on cell growth is 2 days. When tested against WRK-1 rat mammary tumor cells, the conjugate is inactive.

The use of molecular components from both normal and neoplastic breast tissue as markers for breast tumors has long been recognized as of major potential for breast cancer diagnosis, and has been an area of active research. Recent developments in immunological approaches have further stimulated this field by permitting new specificity in the identification and characterization of such markers. Breast tumor markers are now being explored in immunohistochemistry, radioimaging, immunodiagnosis, and even in attempts at their utilization in therapy. To bring together an update on research in this area, a workshop was held in March at the National Institutes of Health, sponsored by the advisory Breast Cancer Working Group and the Breast Cancer Program of the National Cancer Institutes (Dr. Elizabeth Anderson, Program Director) through the auspices of the Organ Systems Coordinating Center (at Roswell Park Memorial Institute, Dr. Clement Ip, Scientific Administrator). The 1 1/2 day workshop included three sessions: Circulating Tumor Markers, Breast Cancer Antigens, and Estrogen Metabolites and Estrogen-Induced Proteins.

A case of thyroid lingual ectopy is reported. Medical therapy was at first sufficient to control the progressive increase in thyroid volume, but adolescence and pregnancies induced a further increment in the volume of the lingual goiter, accompanied by an impairment of swallowing and respiration. Such obstructive symptoms were resolved by surgical intervention followed by continued substitutive medical therapy.
Membranes isolated from mouse and human milk fat globules were found to contain the enzymes responsible for the synthesis of dolichol monophosphate mannose and dolichol monophosphate glucose as well as those involved in the transference of the glycosyl residues from the two dolichol derivatives to dolichol diphosphate oligosaccharides. The levels of most of the enzymes were comparable to those found in mouse mammary gland microsomes. The presence of enzymes involved in protein glycosylation via dolichol derivatives in the milk fat globule membrane provides evidence in favor of an outward flow of membrane components from the rough endoplasmic reticulum, where these enzymes are active in vivo, towards the cell surface.

We have developed a method for clonal growth of human mammary epithelial cells of both nonmalignant and malignant origin. Plating efficiencies of 1 to 50% were obtained by seeding second-passage mammary epithelial cells on fibroblast feeder layers in an enriched medium composed of various hormones and growth factors, as well as conditioned media from three specific human cell lines. Single mammary epithelial cells seeded sparsely onto the fibroblasts underwent at least eight population doublings to form large, readily visible colonies. Optimal colony formation required both feeder cells and the enriched medium. Epithelial colonies containing at least 16 cells were visible 5 days postseeding, and these colonies continued to grow progressively. Plating efficiency and colony size were similar on ultraviolet-irradiated or nonirradiated fibroblasts. The number of colonies formed was proportional to the number of epithelial cells plated. The colonies were identified as epithelial by the presence of human mammary epithelial antigens.
Of 19 primary human breast carcinomas implanted into noninbred female nude mice, 3 produced transplantable tumors. Membrane components specific for human mammary epithelial cells were demonstrated in the cells from heterotransplants even after four or five passages in nude mice.
