Experimental examination of the direct damaging effects of Giardia lamblia on intestinal mucosal scrapings of mice.

Giardia lamblia is known to produce functional and structural derangement of the small intestine but the pathogenesis of this defect is not clear. To examine this, mucosal scrapings from the small intestine of mice were incubated with human G. lamblia trophozoites. The integrity of the mucosal cells was assessed by their ability to exclude trypan blue, and by the levels of brush border lactase, sucrase and maltase. As judged by the trypan blue test, more mucosal cells incubated with G. lamblia were found to be damaged than were in the control groups I (mucosal cells alone) and III (mucosal cells plus Giardia culture media). Similar results were obtained with disaccharidases where again the mucosal cells incubated with G. lamblia showed a statistically significant reduction in the activity of lactase, sucrase and maltase compared to that in the control groups. These findings suggest that G. lamblia causes direct damage to the small intestinal epithelial cells and that this effect is not mediated through factors such as bacterial proliferation, bile salt deconjugation and immunological reactions.

Congenital lobar emphysema: a review of 10 cases.

We reviewed the clinical features and management of 10 patients with congenital lobar emphysema (CLE) treated in our institution over a period of seven years, from September 1988 to September 1995. The age of onset of symptoms ranged from few days from birth to six months. Correct diagnosis was made in only 4 patients before referral, even though diagnosis could have been arrived at from the chest radiograph in all of them. Lobectomy was done in seven patients, while one patient had vascular ring resected. Of the three patients managed conservatively, in one lobectomy was performed at the end of 5 months conservative course as her clinical condition deteriorated during an intercurrent chest infection. Of the remaining two, one has mild episodic reactive air way disease and the other patient is asymptomatic.

Genetic variation at three VNTR loci (D1S80, APOB, and D17S5) in two tribal populations of Andhra Pradesh, India.

This study reports the genetic variation at three variable number of tandem repeat (VNTR) loci (APOB, D17S5 and D1S80) in two tribes (Thoti and Kolam) of Andhra Pradesh, India. Kolams constitute 1% of the total scheduled tribal population of Andhra Pradesh, while Thoti is a numerically small tribe. All three genetic loci were genotyped using the polymerase chain reaction (PCR) technique and were polymorphic in both populations. At the D1S80 locus, both populations showed higher frequencies of allele *31 (9-14%) than other Indian populations. In the APOB system, Thoti showed a very high frequency of allele *37 (54%) and for D17S5 system allele *4 was the most common in Thoti (32%) and allele *2 in Kolam (28%). Both tribes differed statistically significantly from other tribal populations of the region. The level of gene differentiation was low (GST = 0.038) for Indian tribal populations. The allele frequency distribution, heterozygosity and genetic diversity analysis shows that the observed genetic variation is socially and geographically structured.

Prognostic relevance of micro-vessel density in cancer of the urinary bladder.

Angiogenesis is an important factor in tumour growth and metastasis. Degree of angiogenesis (microvascular density-MVD) has been found to correlate with tumour progression and disease outcome in a number of different malignancies. We studied 88 patients undergoing cystectomy for transitional cell bladder cancer to determine if angiogenesis was associated with cancer specific survival. Microvessels were identified by immunostaining of endothelial cells for CD31. Active areas of angiogenesis ("hot spots") were selected using low magnification. The vessel count was performed using a Chalkley point graticule. The mean of 3 counts was used for statistical analysis. The median count was 3.4. Univariate analysis revealed that higher MVD was associated with worsening prognosis (p = 0.02). When adjusted for clinical stage MVD continued to predict worsening prognosis (p = 0.02). MVD was not affected by age or sex or by previous radiotherapy. MVD was associated with the risk of patients dying following pelvic recurrence (p = 0.03) and MVD was significantly higher in patients with lymph node metastasis at surgery. In conclusion, microvessel density proved to be an independent prognostic marker in transitional cell carcinoma of bladder.

Eosinophilic gastritis--an unusual cause of gastric outlet obstruction.

Eosinophilic gastroenteritis is a rare entity. We report a 41-year-old man who presented with features of gastric outlet obstruction due to a submucosal lesion in the distal end of the stomach. Distal gastrectomy with Billroth II reconstruction was done. Histology showed eosinophilic gastritis infiltrating the muscular and serosal layers of the pylorus and antrum.

Massive lower intestinal bleeding--a decade of experience.

BACKGROUND: Lower gastrointestinal bleeding is defined as hemorrhage orginating distal to the ligament of Treitz. Its diagnosis is difficult, often requiring endoscopy, angiography and isotope scans. METHODS: All patients with massive lower gastrointestinal bleeding seen in three gastroenterology centers of Armed Forces during an 11 year period between 1988 and 1999 were retrospectively analysed. RESULTS: A total of 91 patients with massive lower gastro intestinal bleeding were seen during 11 years. The mean age of the patients was 38.9 years and 64 were males. Aetiological diagnosis was: Non-specific ulcers--11, Ileal tuberculosis--8, NSAID enteropathy--8, Enteric fever--7, Meckel's diverticulum--7, Polyps--6, Ulcerative colitis--5, Carcinoma colon--5, Colonic diverticulosis--5, No cause found--5, Polyposis coli--5, Jejunal diverticulae--3, Angiodysplasia colon--3, Radiation colitis--3, Ischaemic colitis--3, Ileal tumor--2, Ileal angiodysplasia--2, Intestinal Lymphoma--2, Bechet's syndrome--1. Bleeding stopped on conservative therapy in 18 patients (including 5 where no diagnosis could be made). Diagnosis was made in 36 patients on fiber-optic colonoscopy done during active bleeding. Remaining patients were subjected to emergency laparotomy and diagnosis was obvious on inspection of abdominal contents in 25 cases. The diagnosis was finally made in remaining 12 cases by intraoperative endoscopic examination through an ileotomy. Four patients died, only one of them due to rebleed after surgery from an additional lesion. CONCLUSIONS: Compared with experience in Western countries, massive lower gastro-intestinal hemorrhage in India affects younger patients, has different causes and carries a lower mortality. Colonoscopy is useful in making diagnosis during active lower intestinal bleeding in about one-third cases. Exploratory laparotomy and Intra-operative endoscopy are complimentary to above examination and can make the diagnosis in most of the remaining cases.
