[Present value of tuberculin tests in sarcoidosis].

Under present epidemiological conditions of tuberculosis in our country tuberculin skin tests were done in 103 patients suffering from sarcoidosis. These results were compared with those of patients suffering from other lung diseases. The patients with sarcoidosis show more negative tests (66%) than the others. In inactive cases of sarcoidosis especially in cases with an acute course in the beginning conversion of tuberculin skin test may be seen. This effect may be used for evaluation of the activity of the disease.

[Rapid in vitro diagnosis of sarcoidosis using a RIA test].

The in-vitro test developed needs only 100 microliters serum and allows a diagnostic valuation within 3 days. Using special sarcoidosis antigen preparations the test showed better results than the Kveim test. There was not a false positive result in patients with tuberculosis and cancer.

[Opiate receptors].

In view of the basic principles of the receptor theory, one should expect all opioids to exhibit principally the same spectrum of effects, as they exert their actions by interaction with "the opiate receptor." Clinical experience shows, however, that the opioids in clinical usage in fact exhibit a variety of spectra of effects. This discrepancy is explained by introducing the "multiple receptor theory," as developed by Gilbert and Martin. This review attempts to class practical clinical consequences with those more recent pharmacological findings.

[Value of bronchoalveolar lavage in the diagnosis and follow-up of lung diseases].

In a group of 100 patients with different lung diseases the diagnostic value of BAL was estimated comparing with conventional broncho-bioptical methods. Final diagnosis was obtained by BAL only in 3% of all cases in contrast to conventional broncho-bioptical methods with 49% positive findings. In a further group of 77 patients with acute and chronic forms of sarcoidosis in stage I and II a significant difference in BAL-lymphocytosis between acute and chronic forms was observed. Our findings demonstrate the limited value of BAL for diagnostic purpose but its valuability in estimation of the activity and the course of different lung diseases especially of granulomatous and fibrotic origin.

[Tolerance and immunogenic effect of preventive influenza vaccination in patients with chronic lung diseases].

Seventy eight patients with chronic lung diseases were once or repeatedly vaccinated against influenza with a bivalent inactivated vaccine. The vaccination was well tolerated. Systemic reactions and transitory deterioration of the lung disease were exceptional. Fourfold rise of antibodies occurred in 1/3 (H1N1) and 1/2 (H3N2) of the patients. After one year, this increase was no more demonstrable. Annual revaccination is recommended.

[Serum angiotensin convertase activity and clinical course of sarcoidosis. Course studies in Löfgren syndrome].

Serial measurements of SACE were performed in 24 patients with Löfgren's syndrome during a period of 32,4 +/- 8,0 months. Initially only 21% (5 patients) had elevated SACE, but the percentage increased during the first weeks of disease reaching 46% (11 patients). After regression SACE activities were in the normal range. In two patients (8%) 18 and 21 months, respectively, after onset of disease pathologically elevated SACE values were measured. Clinically relapses were observed. Our observations demonstrate that serial measurements of SACE are suitable for controlling patients with Löfgren's syndrome, indicating relapses and change to chronic course of sarcoidosis.

[Serum angiotensin convertase activity and clinical course in sarcoidosis. Course studies in primary chronic sarcoidosis].

The relationship between the clinical course and SACE activity was analyzed by observing 133 patients with histologically confirmed sarcoidosis (74 stage I, 59 stage II) at intervals of 6.2 +/- 2.6 months during a period of 32.9 +/- 11.9 months. At the end of the follow-up period the patients were grouped into 4 categories concerning their chest x-ray changes: (a) improved (53); (b) unchanged (38); (c) progressive (9); (d) relapses (33). In group (a) initially elevated SACE always decreased, in (b) it often remained above the normal range; (c) and (d) always coincided with rising SACE. Out of 56 relapses observed 53 (95%) were associated with pathologically elevated SACE. There were significantly more relapses (35) in stage II compared to 21 in stage I sarcoidosis. SACE activities were measured in 109 patients placed on steroids. As a rule initially elevated SACE concentrations fell rapidly after initiation of steroid therapy. However SACE was found to be above the normal range after four weeks of therapy in 21 patients (19,3%), 5 in stage I and 16 in stage II, indicating an unfavourable course of the disease. Serial measurements of SACE are a very useful indicator of the activity of sarcoidosis and are helpful for monitoring the effects of corticosteroid therapy, having also some prognostic significance.

[Lymphocyte populations in bronchoalveolar lavage fluid in various interstitial lung diseases].

Differential cell counts, T-, Tactive-, Tstable-, and B-lymphocytes were studied in bronchoalveolar lavage (BAL) fluid and in peripheral blood from patients with thoracic sarcoidosis (S), extrinsic allergic alveolitis (EAA) and idiopathic pulmonary fibrosis (IPF). In patients with S and EAA the total number of lymphocytes, the percentage of T-, Tactive-, and Tstable-lymphocytes were increased in BAL-fluid, especially in cases of active S and of acute EAA. Contrary to the BAL-fluid the peripheral blood values for T-lymphocytes were decreased in S and in the range of the control group (healthy volunteers) in EAA. The BAL-fluid of patients with IPF is characterized by a high number of polymorphonuclear cells, especially neutrophils. Current concepts of pathogenesis of interstitial lung diseases are discussed and the possible key role of alveolitis in this process is elucidated.

[Histiocytosis X--a comparative 2-country study of East Germany and Yugoslavia].

In a retrospective study 25 patients with histologically confirmed histiocytosis X were investigated in the GDR and in Yugoslavia. No differences between the clinical course was observed in the two countries. The disease was found somewhat more frequent in men, and relative to age groups the age group 18-39 prevailed. Only one fourth of the patients were detected by radiophotography. Lung function was impaired in 2/3 of the patients. In the lung radiogramm reticular and micronodular changes predominated at the beginning of the disease while cystic lesions appeared in late stages of the disease. The diagnosis was established on the basis of biopsy (most often transthoracic lung biopsy) and pathohistologic examination. The course of the disease was unfavourable because of the 18 observed patients 3 died and in 6 progression of the disease was noted on average within several years.

[Value of computer tomography in the diagnosis of thoracic sarcoidosis].

The paper deals with our experience with computed tomography in thoracal sarcoidosis. This method allows a good judgement of lymph nodes in mediastinum and statements, which stand out from conventional roentgenologic diagnostic. In recording of enlarged hilar lymph nodes these methods are equal. As a rule X-ray films and conventional tomography are for planning of therapy so much evident, that in our opinion it is not necessary to use computed tomography in routine diagnostic of sarcoidosis.

[Sarcoidosis in children in East Germany compared to its frequency in the Yugoslavian Socialist Republic].

Sarcoidosis in children is very rare. Especially asymptomatic cases are scarcely known. Sometimes extrapulmonary cases are found. These need a therapy with cortisone derivates. The danger of a false diagnosis is very high if it is based only on a X-ray film. A histological examination seems important. The cooperation between pediatrician and pneumologist is considered as important especially when the grown up patients pass on from pediatric care to the supervision of the pneumologist.

[A contribution to bullous emphysema].

Bullous pulmonary emphysema is a disease predominantly occurring uni- or bilaterally in men. While smaller localized bullae in the apex of the upper lobes may be the cause of spontaneous pneumothorax in younger asthenic patients, it may be the cause of fatal situations in older patients by rupture of giant bullae. False diagnosis between partial spontaneous pneumothorax and bullous emphysema are common, even now. The therapy of choice in all patients with bullous emphysema and still sufficient pulmonary function is surgical treatment with taking away the bullae. Only in patients older than 70 years with cardiorespiratory failure a longer lasting drainage therapy seems to be indicated. The results of all cases with bullous emphysema (47) treated in the thoracicosurgical department of the Research Institute for Pulmonary Diseases and Tuberculosis within 27 years are reported.

[Sarcoidosis - diagnosis and therapy today].

The etiology of sarcoidosis is still unclear. It belongs to the most frequent lung diseases; but also nearly all other tissues and organs may be established. One third of the diseases runs under the picture of the acute form of the course (Loefgren's syndrome). A histologic ascertainment is to be striven for. The tendency to spontaneous remission is great. This is still additionally supported by the application of cortison derivatives. In the individual case the indication to medicamentous therapy results from the tendency of the course of the disease, from the degree of the affection of organs and the clinico-functional disturbance. In 80 patients with Loefgren's syndrome the author adopts a definite attitude to diagnostics and therapy.
