Length of stay. It's not just an inpatient concern.

This article outlines five strategies that can help reduce LOS, a key indicator for improving both service to the patient and cost reduction for the hospital. 1. Reduce the Roles Within the Emergency Department. Expand staff job descriptions, and as a result, the ED's flexibility by reducing the number of separate and distinct roles in the ED. 2. Streamline the Admission Process. Avoid the backup in the ED by streamlining the admission process, paying special attention to ensuring that incentives are aligned to get the patients into inpatient beds. 3. Redeploy and/or Cross-train Key Ancillary Services. Cut down on the delays associated with coordinating with outside ancillary departments by bringing those services which make sense into the ED or at a minimum streamlining the process for accessing them. 4. Use Technology Wisely. Look at available technology as a means to put more information into the hands of your staff or reduce the overall work involved in the process. 5. Establish Team Accountability and Measure Performance. Develop a baseline LOS, measure it going forward, and most importantly, push down accountability for improving it to your clinical and support teams.

The value in "value added"--a tool for understanding your operations.

There are two roles a value added profile can play--as a communication tool and as a target for restructuring. As a communication tool, the profile very clearly shows the inefficiencies of the system and the resulting underutilization of employees. These messages establish for everyone the need for and urgency of change. If your organization is convinced of the need for change, the restructuring process is made easier. The other benefit of the profile is not as a benchmarking tool between institutions or even between bedside care areas of the same hospital. Rather, its real benefit is as a rule to measure progress towards a goal, and to identify when adjustments to the design need to be made.

Restructuring in a unionized environment. Selecting the right approach for union involvement.

Restructuring in a unionized environment doesn't have to be a nightmare. You must assess your current environment to be able to make smart strategic choices. Your strategic choices will lead you to an appropriate union involvement approach and guide you through the dozens of detailed design and implementation issues. However, don't worry so much about them being smart choices that you make no choices. When it comes to restructuring with unions, no plan is the worst plan.
