Familial histiocytic dermatoarthritis. Histologic and ultrastructural findings in two cases.

We report the clinical, histological, and ultrastructural study of two patients, mother and daughter, 32 and six years old, respectively, who presented with dermatitis and arthritis. From the clinical point of view, they appeared to be affected by Familial Histiocytic Dermatoarthritis (FHD), a diagnosis supported by the early age of onset, familial occurrence, and a typical ocular lesion in the mother (glaucoma, uveitis, and cataracts). However, histology revealed the presence of multinucleated giant cells that are usually associated with Multicentric Reticulohistiocytosis (MR). Unlike MR, in the cases here reported the PAS positive, intracytoplasmic material appeared to be glycogen. Electron microscopy disclosed pleomorphic bodies, which have also been described in MR. Thus, we conclude that FHD and MR probably do not represent separate entities, and that a spectrum of disorders exists between these two conditions.

Haemorheological parameters in alopecia areata.

Whole blood, plasma and relative viscosities and red cell filterability have been investigated in 15 patients with Alopecia areata (A.A.) and in 9, age and sex-matched, normal subjects. Erythrocyte filtration time resulted to be significantly increased in alopecic subjects as compared to the normal subjects. Such parameter also showed a good correlation (r = 0.752) with relative viscosity. Whole blood, plasma and relative viscosities were not significantly different from normal subjects.

Life events and alopecia areata.

48 patients with alopecia areata, 30 with common baldness, 30 with fungal infections underwent the Paykel's Interview for recent life events. In the 6 months preceding onset, alopecic patients reported 123 events (mean = 2.56), patients with common baldness 22 events (mean = 0.73), patients with fungal infections 15 (mean = 0.50) (p less than 0.001). Events with negative impact, exits from social fields, uncontrolled events and socially desirable and undesirable events were significantly more frequent in alopecic patients than in controls.

Sex hormone-binding globulin and saliva testosterone levels in men with androgenetic alopecia.

Sex hormone binding globulin (SHBG), plasma testosterone and saliva testosterone were measured in sixty-four men with androgenetic alopecia and in forty males within the same age range without alopecia. There was a significant reduction in SHBG levels in bald men, compared with controls. Plasma testosterone levels were not raised in bald men, but their salivary testosterone levels were significantly higher than in controls.

High glucose modifies heparansulphate synthesis by mouse glomerular epithelial cells.

BACKGROUND: Alterations in proteoglycan metabolism are involved in the pathogenesis of diabetic nephropathy. The aim of this study is to evaluate the effects of high glucose on proteoglycan production and to find a reliable in vitro model for the study of diabetic nephropathy. METHODS: A clone of mouse glomerular epithelial cells was cultured in media containing elevated (30 mmol) and physiological (5 mmol) glucose, or iso-osmolar (30 mmol) mannitol concentrations. We evaluated the synthesis of 35SO4-labeled molecules and the amount of proteoglycans by Sepharose CL6B and DEAE-Sephacel chromatographies. RESULTS: A clear decrease (56%) in total cell-layer proteoglycan synthesis was induced by 30 mmol glucose, in comparison with normal glucose. A reduction of 25% in medium associated proteoglycan synthesis was observed in high glucose cultured cells. After Sepharose CL6B, in cells cultured in high glucose, cell layer heparansulphate proteoglycan-I (Kav 6B 0. 04) synthesis was reduced by about 81%, heparansulphate proteoglycan-II (Kav 6B 0.21) by about 87% and heparansulphate glycosaminoglycan (Kav 0.4-0.8) by about 91%, respectively. In mannitol-incubated cells the reductions observed were less evident and not significantly different from those in normal glucose. CONCLUSIONS: These results indicate that (1) glomerular epithelial cells play a central role in proteoglycan synthesis, (2) high glucose modifies the amount and influences the different species production of these macromolecules, while osmotic forces seem to be only partially involved in these effects, and (3) this cellular clone of glomerular epithelial cells can represent a reliable in vitro model for the study of the mechanisms involved in diabetic nephropathy.

[Recurrent brown tumors as initial manifestation of primary hyperparathyroidism. An unusual presentation].

Primary hyperparathyroidism is a rather frequent pathology characterised by hypersecretion of parathormone (PTH) which is caused by adenomas in 85% of all cases. At clinical onset, the most common symptoms are hypercalcemia-related (pain due to kidney stones, polyuria, gastrointestinal and neurological disorders) while rarer symptoms are due to brown tumors, expansive lesions often found in fibro-cystic osteitis. A case in which the patient showed recurrent mandibular brown tumors as initial clinical symptoms of primary hyperparathyroidism is described. This patient was examined for hypercalcemia, and a tumor mass at the left inferior mandibular branch was found. The patient had undergone surgical removal of a tumor in the left mandibular some years before, which was diagnosed as osteoclastoma. Primary hyperparathyroidism was diagnosed during recovery, and surgical removal of the parathyroid adenoma and mandibular tumor was performed. A histological diagnosis of large cell brown tumor was made. A microscopic observation of brown tumors which are made up of large multinuclear osteoclastic cells can often be confused with other large cell tumors during diagnosis. It is therefore necessary to exclude the presence of hyperparathyroidism with ionised calcium and, in cases of high values, intact PTH (iPTH), before performing a histological diagnosis of a large cell bone tumor. Throughout the course of primary hyperparathyroidism, brown tumors might appear in the absence of other specific symptoms and localize at the level of a single bone segment.

Free microvascular flaps in oncological surgery of the limbs.

Surgery plays a role of primary importance in the treatment of sarcomas of the soft tissues. The first objective of surgical treatment is the local control of the disease. Local recurrence is a serious event, that often requires amputation, and favors metastatic progression of the disease. Currently, except in very rare cases, conservative treatment is the treatment of choice, as it has by now been demonstrated that its results are equivalent to those obtained when demolitive surgery is used. In the distal sites of the limbs, superficial localization of the tendons, ligaments, and joints, the absence of anatomical compartments, make it difficult to perform wide conservative surgery. To this must be added that patients are often sent to see a specialist after a previous inadequate operation, and the presence of a contaminated surgical scar often requires exeresis of a wide area of the skin, sufficient to obstruct healing by primary intention. The use of revascularized free flaps allows for oncologically adequate conservative surgery to be performed, even in anatomical sites where amputation alone traditionally provided a safe surgical margin.

[Atypical partial tracheolaryngectomy associated with circular pharyngectomy followed by flap reconstruction for dedifferentiated papillary carcinoma of the thyroid gland].

The management of the extensive malignancy of the hypopharynx generally combines with total laryngectomy. The authors report a case of scanty differentiated papillary thyroid carcinoma, extended to the hypopharynx, with partial involvement of thyroid, cricoid and first tracheal ring cartilage and unilateral vocal fold palsy treated with an "atypical" partial tracheo-laringectomy combined with total circular pharyngectomy. The defect has been replaced with an antero lateral tight free flap suitably modeled to restore the pharyngeal and laryngeal lumen. A modified Montgomery T- tube has been inserted to avoid larynges-tracheal stenosis. This unusual technique allowed to restore a stable airway and satisfying deglutition and an adequate voice. With this case presentation the authors want to subline that when the tumor type is less aggressive than squamous cell carcinoma, it is possible to perform a partial larynx preservation also in the case of circular pharyngectomy. Obviously the residual laryngeal skeleton have to be sufficient to restore laryngeal lumen.
