Elastic properties of the rectal wall in normal adults and in the patients with ulcerative colitis.

The elastic properties of the rectal wall have been studied in 16 normal adults (group I) and in 24 patients with ulcerative colitis, during a nonactive (13 cases, group II) or an active period (11 cases, group III). The rectal intraluminal pressures after accommodation to the distension were significantly higher in group III than in groups I and II. These pressures were not related to distending volumes in groups I and II but were correlated with the distending volumes in group III. The rectal elasticity coefficient was significantly greater in group III than in the other two groups. There was an inverse correlation between the rectal elasticity coefficient and the rectal area. These results suggest a loss of the rectal distensibility in active colitis.

Rheumatic symptoms in Crohn's disease and the HLA system.

The study of the distribution of the HLA phenotypes among 55 patients suffering from Crohn's disease did not show any significant difference compared to a control population. On the other hand, the same study limited to patients suffering both from Crohn's disease and peripheral articular manifestations showed a significant increase in the frequency of the HLA BW17.

[Respiratory function disorders during hepatopathies with immunity involvement].

A respiratory functional test was done systematically on 24 patients with chronic hepatitis and was found normal in only 2 cases. It showed an alteration of the transfer in 14 patients and a syndrom of hyperinflation in 8 others. Despite the histopathological proof it seems that the transfer alteration could be attributed to diffuse interstitial pulmonary lesions. They have already been noted in chronic hepatitis by several authors. The pathogeny of this interstitial involvement probably implied immunity phenomenons including the participation of the antigen Australia or others. As for the hyperinflation syndrome observed in 4 patients in the absence of any bronchopulmonary history, it could reveal an emphysema, which, so far, had not been seen associated to chronic hepatitis.

[The removal of gastrin secreting tissue in the treatment of Zollinger-Ellison syndrome (author's transl)].

In the light of 24 cases, routine total gastrectomy in the treatment of Zollinger-Ellison's syndrome should be abandoned in favour of the removal of gastrin-secreting tissue. This treatment which gives just as good results on the ulcer disease, also has the advantage of removal of the tumour tissue malignant in almost 2/3 of cases. Removal of the gastrin secreting tissue thus becomes the operation of choice. Total gastrectomy remains a last resort operation if it proves impossible to find or remove the gastrinoma, or, if after removal of the original tumour, there occurs a relapse of the ulcers or spread of the neoplasm.
