In vitro antibacterial activity of E-3604, a new 6-fluoroquinolone, on clinical isolates.

From a series of pyrrol-containing antibacterial agents, E-3604 was selected for development and compared with nalidixic acid, norfloxacin and ciprofloxacin against strains of clinical isolates. The in vitro activity of E-3604 was greater than that of nalidixic acid, similar to that of norfloxacin and lower than that of ciprofloxacin, except in the case of Staphylococcus where E-3604 showed the best in vitro activity of all the studied compounds, with a MIC range of 0.25-0.03 mg/ml. E-3604, like the other quinolones, presented a pH-dependent variation of activity, greater activity being observed in slightly acidic pH values.

[Farmer's lung. Report of a case (author's transl)].

Asthenia, anorexia, fever, weight loss, dyspnea and cough developed insidiously over 3 months in a patient working in a farm and exposed to moldy organic dusts (hay and alfalfa). Complete recovery occurred when exposure was stopped, but symptoms reappeared when the patient took the same job again. Pulmonary function tests showed reduced compliance and diffusion of gases. The demonstration of precipitating antibodies to moldy hay extracts confirmed the diagnosis of farmer's lung. Eight months after discontinuance of exposure and on steroid therapy the patient was asymptomatic with spirometric values within normal limits. Chest roentgenogram showed a residual finely reticular pattern in the superior lobules. The importance of an early diagnosis and subsequent change of job in order to avoid irreversible pulmonary fibrosis is stressed.

'Pseudocirrhosis' in hereditary haemorrhagic telangiectasia.

Telangiectasia-associated hepatic fibrosis (TAHF) in a 68-year-old woman with hereditary haemorrhagic telangiectasia (HHT) is described. The patient died of oat-cell carcinoma of the lung. In addition to the structural alterations which have been described previously in HHT, the liver exhibited focal midlobular hepatocytic necrosis and tumour metastases. The possibility that treatment of HHT was causally related to some of the hepatic abnormalities found in our patient and the differentiation of TAHF from true cirrhosis are discussed.

[Morbid obesity. Reflections on a surgical protocol (I). A clinical and peroperative protocol].

UNLABELLED: Obesity as a condition, and morbid obesity as a disease, have at present reached epidemic proportions. Bariatric surgery is the most effective treatment in those patients in whom medical, dietary, and/or behavioral treatments have failed. What is more, it is the initial treatment of choice in morbid superobese patients (> 50 kg/m2). The present study summarizes and criticizes the main surgical techniques used at present, and purposes an action protocol for anesthesia, as well as some norms and advice on control, action, and pulmonary and dietary rehabilitation, in the peri- and post operative period, based on more than 7 years' experience with a series of more than 150 operated and controlled patients. CONCLUSION: A restrictive bariatric technique is a model which allows the multidisciplinary obesity treatment team to achieve a definite and permanent modification of the eating habits of all patients who meet the criteria for inclusion in a surgical protocol.
