[Ketoprofen in the prevention of postoperative pain in abdominal surgery. A multicenter study].

Two-hundred-forty-eight patients undergoing abdominal surgery were admitted to a multicentric clinical trial. The patients were randomly assigned to a single i.v. dose of ketoprofen or acetylsalicylic acid, 15 minutes after the end of operation. Ketoprofen showed a better analgesic activity with a statistically significant difference at 2 and 4 hours after administration. Two patients treated with ketoprofen reported vomiting and skin rash respectively. The results of this study confirm the efficacy of ketoprofen for the prophylaxis of postoperative pain in abdominal surgery.

Antibodies to hepatocyte derived antigens and liver damage in patients after bone marrow transplantation.

The follow-up sera from 33 patients who received allogenic bone marrow transplantation (BMT) were tested by ELISA for antibodies to liver derived antigens present in a macromolecular, lipid associated, protein complex (LSP). Anti-LSP were found in 20% of patients with graft-vs-host disease (1 with acute GVHD, 3 with chronic GVHD), but in none of the 13 patients without GVHD. Lymphocytotoxic antibodies (LCTA) were frequently detected with similar prevalence in patients with and without GVHD. In the 4 anti-LSP +ve cases, LCTA serum levels did not follow the trend of anti-LSP antibody levels. Liver biopsy was performed in 10 patients with clinical and biochemical sign of GVHD and showed features of hepatic GVHD in six cases; although piecemeal necrosis was present in 3 cases, anti-LSP were detected only in one patient. An anti-LSP response can be elicited in patients after BMT only during GVHD, with no relationship to LCTA and without a close association with signs of periportal hepatocyte damage.

Factors leading to the appearance, after BMT, of cytotoxic antibodies against T, B and activated T lymphocytes: critical appraisal.

In order to investigate the role of lymphocytotoxic antibodies, acquired after allogeneic and autologus bone marrow transplantation, we studied 309 sera from 42 transplanted patients (16 adults and 26 children). We tried to correlate antibody elicitation towards T, B and activated T lymphocytes with the following parameters: genetic (recipient's and donor's sex, HLA profile), clinical (recipient's primary disease, GvHD, transplant outcome) and technical (bone marrow purging, auto-or allotransplant). There is evidence that anti-T and -B cytotoxic antibodies appear earlier than anti-activated T antibodies. Anti-HLA specific antibodies seem to be produced by the transfusional stimulus: they appear early after BMT and wane after the first year. Humoral responsiveness seems to be age related (adults are more responsive than children) and conditioned by GvHD (the level of cytotoxic antibodies decreases when GvHD is prevented by bone marrow purging). The level of cytotoxicity is significantly lower in the sera of autotransplanted patients compared with the allotransplanted ones. It appears that anti-activated T antibodies are produced by cell activation at different times in adults and children: in adults this occurs during GVHD and in children during the relapse of disease.

Mixed lymphocyte reactions evaluated by means of bromodeoxyuridine incorporation.

The mixed lymphocyte reactions are usually performed by the uptake of 3H thymidine (3H TdR) in the study of histocompatibility for allogeneic bone marrow transplantation. Bromodeoxyuridine (BrdUrd), an analogue of thymidine, can be used as an alternative marker of proliferation. In this study we compared the evaluation of the proliferative activity of alloreactive lymphocytes in MLR by 3H TdR and BrdUrd incorporation in flow cytometry. The results show that BrdUrd is able to recognize proliferative activity at least 24-48 hours before 3H TdR, and allows discrimination of proliferation of a few cells despite its proximity to the autologous controls. However, the large quantity of cells needed for every combination is presently a disadvantage of the method.

Bromodeoxyuridine

The distribution of HLA antigens in Italy.

Human histocompatibility (HLA) gene frequencies were studied in the Italian population. A random sample of healthy individuals born in several Italian regions and provinces was studied to estimate HLA-A, -B, -C and -DR gene frequencies using the maximum likelihood method. The goodness of fit to Hardy-Weinberg proportions was evaluated by the likelihood ratio statistics. Different Italian regions and provinces show significant differences in the HLA alleles, providing further evidence for the genetic heterogeneity in the Italian population. This heterogeneity is also displayed by a synthetic geographical representation which uses colour to map the most informative gene differences. Statistically significant gametic associations between HLA-A, -B, -C and -DR loci are reported. The difference between northern and southern Italy and between continental Italy and Sardegna is clearly shown also by their heterogeneous linkage disequilibria.
