BACKGROUND: Recent studies have caused much controversy about the prevalence of IgE antibodies to Hev b 1 among health care workers (HCWs) and patients with spina bifida (SB) who are allergic to latex. This investigation was carried out to verify the results reported. METHOD: Serum samples from 140 patients with SB as well as from 105 HCWs allergic to latex were tested by enzyme allergosorbest test (EAST) and EAST-inhibition assay to evaluate the rate and degree of sensitization to highly purified Hev b 1. RESULTS: Eighty-one percent of patients with SB who were allergic to latex had IgE antibodies against Hev b 1. The prevalence of anti-Hev b 1 antibodies among HCWs allergic to latex was 52.3%. In 15 of 33 serum samples from patients with SB that were randomly tested, the IgE binding to commercial latex allergens could be completely inhibited by Hev b 1; in only six cases was the maximum inhibition of IgE binding to latex by Hev b 1 less than 50%. Testing two monoclonal anti-Hev b 1 antibodies with extracts of five brands of latex gloves revealed a predominant presence of Hev b 1 protein as a monomer or its aggregates. Molecular analysis of human leukocyte antigen-D region genes DRB and DQB1 suggested no statistically significant correlation between the human leukocyte antigen alleles tested and IgE responsiveness to Hev b 1. CONCLUSIONS: Our results indicate that Hev b 1 not only makes significant contributions to the IgE binding to latex, but it is also the unique sensitizer in about 45% of patients with SB who are allergic to latex.

BACKGROUND: Myasthenia gravis is usually revealed by a ptosis or a diplopia. A respiratory muscle weakness often occurs during the course but an acute respiratory failure as initial feature is unusual. CASE REPORTS: Three girls, aged 8, 10 and 14 years, were hospitalised in an intensive care unit, along a 15 year-period, for an acute respiratory distress. The first two children suffered from skeletal and bulbar muscle weakness. The third, admitted with the diagnosis of unexplained pneumonia, was complaining of skeletal and bulbar muscle weakness for the last 18 months. Myasthenia gravis was confirmed with electromyography, and detection of the acetylcholine-receptors antibodies in all three cases. CONCLUSION: Any unexplained acute respiratory distress must lead to search for skeletal and bulbar muscle weakness, specially after muscular exercise or at the end of day, manifestations which characterize myasthenia gravis.

In this study we investigated the relationship between HLA class II alleles and the IgE-specific immune response to the 4.7 kDa polypeptide hevein of Hevea brasiliensis, a major latex allergen, 51 individuals with immediate-type latex allergy and 90 controls were examined for the polymorphisms in exon 2 of HLA-DRB1, 3, 4, 5 and DQB1 by sequence-specific oligonucleotide probe typing. 35 (69%) out of 51 latex-sensitized subjects showed positive hevein-specific IgE values. Analysis of the HLA data among these 35 subjects revealed increased phenotype frequencies for DR4 (22/35, 63%) and DQ8 (18/35, 51%) when compared with those in the 16 hevein-negative but latex-positive subjects (DR4: 2/16, 13%, p = 0.0009, Pc = 0.047; DQ8: 0/16, p = 0.0003, pc = 0.018) and with healthy controls (DR4: 22/90, 24%, p = 0.00012, pc = 0.013; DQ8: 16/89, 18%, p = 0.0003, pc = 0.036). Finally the DR4-DQ8 haplotype frequency was significantly elevated in hevein-positives when compared with hevein-negatives (51% vs. 0, p = 0.0003, pc = 0.034) or controls (51% vs. 18%, p = 0.0002, pc = 0.045) The present data suggest DR4 and DQ8 to be operating jointly as susceptibility factor for the allergy to hevein.

OBJECTIVES: To assess nosocomial infections in a burn care centre, to identify patients' infection risk factors at the time of admission and factors of monthly variations of infection incidence. STUDY DESIGN: Prospective survey, from October 1992 to September 1993. PATIENTS AND METHOD: The study included 140 patients staying for more than two days in a 22-bed burn unit. Nosocomial infection criteria were derived from the 1988 CDC criteria. Incidence rates of infection were calculated. Infected and noninfected patients were compared. Each monthly infection incidence was compared with six unit activity indicators. RESULTS: Fifty-six patients developed 132 infections. The overall incidence was 94%. Incidence density was 25 infections per 1,000 days of care. The distribution of infected sites was: skin (30%), intravascular catheters (25%), blood (22%), urinary tract (18%), respiratory tract (5%). The most frequent pathogens were Pseudomonas sp (49%), Staphylococcus sp (18%), Escherichia coli (18%), and Streptococcus faecalis (10%). They were characterized by a good antibiotic sensitivity. Each common burn severity index was predictive of nosocomial infections. Facial, perineal and respiratory lesions were also linked to infection. There was a positive correlation between the peak of nosocomial infections in the unit during a month and the peak of activity during the foregoing one. CONCLUSION: Incidence rates of infection were high, as 40% of the population was concerned. Choosing reliable infection criteria was the most difficult problem to solve.

Behavioral effects of cognitive aging and the meaning of experience are compared. Relating the process of aging to biological and functional decay does not do justice to the accumulation of knowledge and abilities over the active lifespan. Human development is a continuous process of growth and decline. It accounts for both gain and loss of cognitive abilities. From this perspective human capacities are not static but composed of a changing set of abilities which grow and decline over the life span. Despite decreasing capacity with age, reserve capacity may be sufficient to cope with normal task demands. Finally, it is illustrated how experience of older workers may contribute to the design of training programs.

The aim of this investigation was to study age-related differences in timing of position and velocity identification in a laboratory task. The skills required for performing this task are thought to be similar to those needed in real traffic situations. From this perspective, the results of this study may be taken into account in studies on accident analysis and prevention. To control for differences in the conscious experience of time and for simple reaction time to visual stimuli, young (25-34) and old (58-70) adults had to produce timed periods of 20 seconds and to time the arrival of a visible moving object at its goal. In these simple tasks no differences were found between young and old subjects. In the proper, more complex experiment, timing of position and velocity identification of a moving object were assessed with or without feedback on timing and velocity in the same old and young group. The object, moving from left to right at a constant velocity, was shown on a video screen during the first 40% of its trajectory. The moving object could have any of four (fixed) velocities which were presented in blocks with a regular or a mixed order. By pushing a button subjects estimated at which time the object would have reached the marked end of a trajectory. In a condition without feedback on timing of position, the timing error of older adults was higher than that of young adults and particularly with higher object velocities.(ABSTRACT TRUNCATED AT 250 WORDS)
The ambulant long term care for children with meningomyeloceles requires the interdisciplinary cooperation of pediatricians, pediatric neurologists, surgeons, orthopedists, radiologists and psychologists, social workers, physiotherapists and orthopedic mechanics. We present the concept for our out-patient consulting hour for 154 children and adolescents with meningomyeloceles, meningoceles and other forms of paraplegia, and the program of medical examinations for the detection of urologic malformations and the consequences of neurogenic disturbance of bladder function, neurologic complications (seizures, tethered cord etc). and dysfunction of liquor shunts. Additional items are the control of physiotherapy and the regular and adequate prescription and inspection of orthoses. The rate of complications is presented: revision of shunts 60%, seizures 17%, cerebral abnormalities 16%.
