The effectiveness and tolerance of betablockers in elderly patients (> or = 70 years) with heart failure.

UNLABELLED: The beta blockers (BB) being less prescribed in elderly patients (P) with heart failure (HF), the aim of this study was to assess the effectiveness and tolerance of BB (meteoprolol, bisoprolol or carvedilol) given on the top of the conventional therapy in HF due to LV systolic dysfunction in P > or = 70 year (n=57, group 1) and < 70 year (n=101, group 2). Differences in baseline clinical characteristics between the 2 groups were not significant. The BB doses given in group 1 P was lower but the difference was significant for bisoprolol only. Intolerance to BB imposing withdrawal occurred in 12% of group 1 P and in 10% group 2 P (p>0.05). Symptomatic improvement expressed as a significant decreases in NYHA class was observed in both groups. Readmission for worsening HF was needed in 42% vs. 39% while 1 year mortality rate was 11.4% vs. 10.4% in group 1 and 2 P respectively (p>0.05). CONCLUSION: BB are tolerated and seem to be effective in most elderly P with HF. Therefore, BB should be tried in all HF P without contraindication irrespective of age.

Clinical and echocardiographic spectrum of pericardial effusions in routine M-mode echocardiographic examinations of hospitalized patients.

In 123 (13.2%) out of 926 hospitalized patients pericardial effusions were diagnosed during routine M-mode echocardiographic examination. 53 (43.0%) presented a small (group 1), 31 (25.3%) a moderate (group 2) and 38 (31.7%) a great amount of fluid (group 3). Pericardial effusion could be diagnosed only by echocardiographic examination in 18 patients (34.0%) of the first group, in 8 (26.0%) of the second and in 7 (18.0%) of the third group. Routine M-mode echocardiographic examination in hospitalized patients allows the detection of small pericardial effusions often missed by conventional methods. This method also makes possible rapid diagnosis of pericardial effusion in cardiac patients in a critical condition or with haemodynamic disturbances sometimes difficult to identify.

The sick sinus syndrome. Clinical and electrocardiographic spectrum of nine patients.

The paper presents a report on the clinical and electrographic spectrum of nine patients with the sick sinus syndrome with severe symptoms of clinical disturbances and serious disorders of impulse formation and conduction. Seven patients had syncope associated with sinus bradycardia, sinoatrial block or atrial fibrillation with slow ventricular rate. Prolonged episodes of atrial and ventricular asystole occurred and were interrupted by junctional escape beats. Paroxysmal atrial fibrillation and flutter were foun in three patients and infrahisian block in four. Atropine induced a slight acceleration of the heart rate in all the patients studied while the post-suppression S-A node recovery time was prolonged in four.

Clinical significance of ventricular dyskinesis in acute myocardial infarction assessed by apexcardiography.

Apexcardiograms [ACG] recorded in 64 patients with anterior acute myocardial infarction disclosed in 36 cases [56.2%] paradoxal systolic precordial pulsation considered as an expression of ventricular dyskinesis [VD]. The functional significance of VD was assessed by the left ventricle systolic time intervals. There was a significant shortening of the left ventricular ejection time (LVET) and a significant increase in the pre-ejection period (PEP) and of PEP/LVET ratio in patients with VD. The heart/chest ratio was greater in patients with VD but the difference was not statistically significant. The condition of the heart after the acute phase of myocardial infarction was much better in patients without VD or with transient VD in than in those with permanent VD. 91.3% of the patients with permanent VD presented heart failure and belonged to the functional class III-IV. These findings indicate that patients presenting ACG-assessed VD have frequently an altered cardiac performance and a worse long-term prognosis.

Bidirectional tachycardia a study of five cases.

Five patients with bidirectional tachycardia due to digitalis toxicity associated with severe organic heart disease were studied. The origin of the abnormal rhythm was established with the aid of His bundle recordings in three cases and by indirect clues in the others two. In three cases the origin of bidirectional tachycardia was suprahisian while in two patients it was infrahisian. In one patient the transition from junctional to ventricular tachycardia could be observed. Bidirectional tachycardia appears to be a complex arrhythmia in which similar electrocardiographic configuration can be due to different mechanism. Digitalis toxicity was often a causal factor.
